REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec. R806. The installation of ridge venting requires the installation or existence of soffit venting. For calculation purposes, one hat or turtle vent equal to one-half of one square foot of opening. Effective December 1, 2014, asphalt shingle installations require an approved midroof inspection, conducted when 25-75 percent of the roof covering is installed, prior to final approval. Installation of roof sheathing (new or overlay) is required on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and mid -roofs may be called in at the same time, one hundred percent of the sheathing must be complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave and secured in place is required to be provided for all roof inspections. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. In order to pass a final inspection of elastomeric or similar type roof coverings, a letter of inspection and approval from the manufacturer technical representative stating that "the application of the roof at (project address) has been applied in accordance with the installation instruction for (roof material brand name) roof covering" is required to be on site at the time of final inspection. DATE: Tuesday, April 28, 1998 SUBJECT: 4088 Oak St. In an effort to expiain the problems that have arisen with this particulaz permit and the associated deed that is required, I will tell you the scenazio from my perspective and recollection. On April 15', I was asked if we had a Quit Claim or Warranty deed form on disk that could be used to help Mr. Yang prepaze his deed for the necessary dedication of right of way. Mr. Yang brought in a piece of paper that was purported to be a plot plan that he had prepazed and a legal description he had written for the area to be dedicated along with a copy of a portion of what appeazed to be an Improvement Location Certificate or a Survey Plat with neither a stamp nor a signature by a licenced surveyor. The legal description was for a portion of two sepazate pazcels, only one of which was owned by Mr. Yang. I told him that we would also need a copy of his deed of ownership and with those documents we couid probably help him with the prepazation of the deed for dedication. As I began to review the materials that he had provided at our first meeting and the deed of record for the property I noticed that the legal descriptions did not match one another and the dimensions shown on the drawings provided didn't match the deed of record. At that time I decided it would not be appropriate for me to attempt to author a legal description for the azea needed for right of way dedication. It has always been my understanding that the Building permit process requires the applicant to provide all the necessary documentation to the city prior to issuance of a permit. These would include a recorded deed for the property, and an Improvement Location Certificate with the proper stamp and signature. If dedication is required, it is the applicants responsibility to provide those documents as well. Z'he Director of Public Works shall give written notice and an estimate of costs to the Owner 30 days prior to advertisement of any major improvements to 4088 Oak St. which would affect the Property and will submit to the Owner a detailed cost breakdown and billing after Award of the construction contract by City Council. The detailed cost estimate will include the Owner's portion of the cost of curb, gutter and sidewalk and associated pavement improvements and other items listed in Paragraph 4. 6. Upon receipt of the detailed cost estimate or billing, the Owner hereby agrees to pay to the City, within 45 days, the full amount of the detailed cost estimate or billing. The Owner also agrees to pay to the City any extra costs associated with the Public Improvements to the Property due to variations in estimated contract quantities or as a result of change orders within 30 days of written request for payment by the City. The City will pay to the Owner any overcharges paid to the City by the Owner within 45 days after final acceptance of the Public Improvements. 7. Owner agrees to pay for all public improvements or billings within 45 days of written notice; Owner further agreeing that any amount resulting from his failure to pay for any improvement or billing this improvement completed shall constitute a lien upon the property above described in the actual amount of the public improvements or billing completed, less any payments made by Owner, plus interest on the amount unpaid at 10 percent (lo%) per annum from the date of final acceptance or billing, plus any costs incurred by the City in collecting same, including court costs and attorney's fees. 8. In the event an Improvement District or a Special Zmprovement District is created by the City to improve or provide those requirements as set out in the SUBDIVISION REGULATIONS as they exist at the date of execution of this Covenant, for curb, gutter, sidewalk related street and drainage improvements, solely relating to those streets fronting on the property and neighboring tracts of land, the Owner or his assigns and successors agree not to oppose its creation, or subsequent reasonable assessment of the costs to the property. 9. Zn the event an Improvement District is created by the City to provide the improvements as provided by the SUBDIVISION REGULA- TIONS, the costs assessed against the property shall not be disproportionate with costs assessed to other nearby and like properties. 10. Job Number: 95-142 17ris report presents the resulu of a soil and foundation investigation for the ProPosad siugle familY residence to be consWcted at 4088 Oak StreeR, located in Wheat RidBe, Colorado. The project site is shown on Figure 1. The study was condacted in acco*dance w►d► our proposal to Mr. Steve Muriha dated 7anuary 19, 1995. A field investigation program was conducted to obtain mfornnaUa° oa subsurface wnditions. Material samples obtained during the subsurface investigation were tested in the laboratory to pmvide data on the classification and eogineeriog characteristics of the on-site soil and bedrock. The results of the Seld and laboratory investigations aze presentad herein. Tlus report has becn prepared to sumniarize the data obtained and to present our conclusions and recommendations based on the proposed construction and the subsurFace conditions encountered. Design parairieters and a discussion of geotechnical eogineering considerarions related to consWction of the proposed residence are included. Prouosed Construction: We understand that the proposed consuuction for the residence will consist of demolishing an existiag shed on the site and the construdion of anew wood-frame, multi-level structure of approximately 2,000 to 2,500 squue fcet with a full basement and an 7393 Dahlia Street, Commerce Ciry, Colorado 80022 (303) 289-1989 • Fax (303) 289-1686 4088 Oak Street attac6ed garage. The northwest comer of the new structure is to be located approximately 30 feet east of Oak Street and approximately 30 feet south of 41° Avenue. For purposes of this report, we bave assumed that foundation loads will be light as is typical for residentia► structums. If loadings or conditions are signiScandy different from thase described above, we should be notiSed to re-evaluate the recommendations contained in tlus report. Site Conditions: At the time of our investigatioq the site was a vacant lot at 4088 Oak Street in Wheat Ridge, Colorado with the exception of being occupied by an exis[ing wood and metal shed. The site was also being used as a storage location for a pick-up, a utility uailer, and a travel trailer. Vegetation at ihe site was natural grasses and wceds. The site was bounded by Oak Street to the west, 41" Avenue to the norfh, and existing single family residences to the east and south. Topography at the site was slighdy sloping toward the south-east at approximately three (3) percent. The maximum elevation difference across the site was approximately tluee (3) to four (4) feet. The surrounding area is occupied by several a single family residences that appeared to be of wood frame construction approximately 20 to 50 years of age. Field Investisation: The field investigation for the project was conducted on January 26, 1995. One (1) test hole was drilled at the location shown on Figure 1 to explore subsurface wnditions. The location of the testboring was referenced to local features and elevations were determined by estimation. The test holes were advanced through the overburden soils and underlying bedrock with 4-inch diameter continuous flight augers. The test holes were logged by a representative of Ground Engineering Consultants, Inc. Samples of the subsurface materials were taken with a 2-inch I.D. "Califomia Liner" sampler. The sampler was driven into the various strata with blows from a 140-pound hanmer falling 30 inches. This test is similar to the standard penetration test described by AST'M Method D-1586. PenetraAOn resistance values, when properly evaluated, indicate the relative density or consistency of the soils. Depths at which the samples were taken and the penetration resistance value aze shown on the Log of Exploratory Hole, Figure 2. Job No. 95-142 Gound EnAncering ConsuNents, Inc. 2 4088 Oak Street Measurements of the waYer level were made in ihe test holes by lowering a weighted tape into the open hole shortly after completion of drilling. The location of the water level measured, if present, is shown on ihe Log of Exploratory Hole. Laboratorv Investiaation: Samples obtained from wr eYploratory hole were eacamined by the project engineer and classi5ed in the laboratory. Laboratory testing included standard proPectY tests, such as natural moisture contents, dry unit weights, grain size analyses and liquid and plastic limits. A swell-consolidation testing was conducted on the fourtcen (14) foot sample of the claystone bedrock to deternune the compressibility or swell chazacteristics of ihe claystone at the site under loading and saturation. Results of the laboratory testing program aze shown on Figure 4 and are summarized on Table 1. The laboratory testing was conducted in general accordance with applicable AS1'M specifications. Subsurface Conditions: The subsurface soils encountered at the site consisted of a thin veneer of topsoil, underlain by silty sand overburden to approximately seven (7) feet, underlain by interbedded sandstone and claystone to approximately thirteen (13) feet, undedain by claystone bedrock to the maxiinum eactent of the exploration at riventy-five (25) feet. The sand overburden encountered in the test boring was silty, nonplastic, fine grained, medium dense to dense, slightly moist, and light brown in color. The interbedded sandstone and claystone bedrock encountered in the test boring was nonplastic to medium plastic, occasionallY hiBhlY Plastiq fine grained to coarse grained, liard to very hard, slightly mois[ to very moist, and olive-brown. The claystone bedrock encountered in the test boring was medium to highly plastic, hard to very hazd, very moist, and gray in color. Free groundwater was observed at the temiination of the test boring at approximately twenty-Sve (25) feet. Foundation Recommendation: Based on the data obtained during the field and laboratory investigations, we believe straight-shaft piers drilled into the bedrock beneath the site should be utilized to support the proposed residence. Job No. 95-142 Cxound Engneering Consoftanls,lnc. 3 4088 Oak Street Shallow foundations placed on swelling soils and/or bedrock similaz to those encountered at tlris site, can experience sUuctural defortnation if the soils and/or bedrock are subjected to changes in moisture content. The drilled pier foundation will place the bottom of the piers in a zone of relatively stable moisture content and make it possible to lead the piers sufficiendy to resist uplift movements. Utilizing this type of foundation, each column is supported on a single drilled pier and the building walls aze founded on a grade beam supported by a series of piers. Load applied to the piers is transmitted to the bedrock partially through peripheral sheaz stresses which develop on the sides of the pier and partially flvough end bearing pressure. The design and construction criteria presented below should be observed for a straight-shaft pier foundation system. The construction details should be considered when preparing project documents. (1) Piers should be designed for an allowable end bearing pressure of 20,000 psf and a skin friction of 2,000 psf for the portion of the pier in bedrcek. The upper eight (8) feet of pier penetration into bedrock should be ignored in all load calwlations. (2) Piers should be advanced to a minimum pier length of 20 feet. (3) Piers should be designed to resist lateral loads assunung a modulus of horizontal subgrade reaction in the overburden soils of fifty (50) tcf and a modulus of horizontal subgrade reaction of 300 tcf in the bedrock. (4) Piers should be reinforced their full length with one #5 reinforcing rod for each sixteen (16) inches of pier perimeter to resist tension created by the swelling materials. (5) A 4-inch void should be provided beneath grade beams to prevent the swelling soil from exerting uplift forces on the grade beams and to concentrate pierloadings. A void should be provided beneath necessary pier caps. (6) The minimum spacing requirements between caissons on the site should be three (3) diameters from center to center. At this spacing, no reduction in axial or horizontal soil modulus values is required. Piers grouped less than three (3) diameters center to center Job No. 93•142 Cround Engineering Consuftents, Irc. 4 4088 Oak Street should be studied on an individual basis to determine the appropriate reduction in both lateral and =al capacity. (7) A minimum pier diameter of ten (10) inches is recommended to facilitate proper cleaning and observation of the pier hole. (8) Concrete utiliud in the piers should be a fluid mix with sufficient slump so it will fill the void between reinforcing steel and the pier hole and have a minimum slump in ihe range of Sve (5) to seven (7) inches. (9) Pier holes should be propedy cleaned prior to placement of concrete. (10) The presence of water at the termination of the exploration and the relatively abundant moisture content in some of the samples taken from the test boring indicates that use of casing in the pier holes may be required to reduce water infiltration. The requirements for casing can sometimes be reduced by placing concrete immediately upon cleaning and observing the pier hole. In no case should concrete be placed in more than three (3) inches of water. (I1) The pier drilling contractor should mobilize equipment of sufficient size and operating wnditions to achieve the required bedrock penetration. in the claystone bedrock. If refusal is encountered in these materials, our office should be notified to evaluate the conditions and to establish that true refusal has been met with adequate drilling equipment. (12) Caze should be taken to prevent fomvng mushrooms at the tops of the piers, since this can increase uplift pressures on the piers. (13) A representative of the soil engineer should observe pier drilling operations. Floor Slabs: Floor slabs present a very difficult problem where swelling materials aze present near floor slab elevation because sufficient dead load cazinot be imposed on them to resist the uplift pressure generated when the materials are wetted and expand. Based on the moisture-volume change characteristics of the materials encountered at this site, we believe the only way to eliminate damage as a result of floor slab movement will be to wnstruct a structural floor above a well- Job No. 95-142 Grouod EnAneering ConsultaMs, Ina 5 4088 Oak Street ventilated crawl space. The floor should be suppor[ed on gade beams and piers the sacne as the main structure. Slab-on-grade construction may be considered as an altemate for the floor slabs, provided the risk of distress resulting &om floor slab movement is recogized and precautions are taken to reduce the effects of movecnent. If slab-0n-grade construction is opted for, the following measures should be taken to reduce the damage wluch would resuk from movement should underslab materials be subjected to moisture changes. (1) We recommend that in the basement level the interbedded sandstone / claystone beneath the floor slab be overexcavated to a depth of 3 feet and replaced with overburden sand materiaL The overexcavation should extend 3 feet outside the building limits. The overburden sand material should be stockpiled for reused as fill beneath the floor slabs. The overexcavation maybe eliminated by the soils engineers discretion upon observation of the foundation excavation. (2) Floor slabs should be separated from all bearing walls and columns with an expansion joint which allows unrestrained vertical movement. (3) Interior partitions resting on floor slabs should be provided with a slip joint so that if the slabs move, the movement cannot be transmitted to the upper structure. This detail is also important for wallboards and door frames. A slip joint which will a11ow at least two (2) inches of verticai movement is recommended. (4) Floor slabs should be provided with control joints to reduce damage due to shrinkage cracking and should be adequately reinforced. We suggest that joints be provided on the order of twelve (12) feet on centers; however, the actual joint placement should be determined by the reinforcement design. (5) A minimum 4-inch layer of free-drainage gravel should be placed beneath the slabs. This material should contain less than 5% passing the #200 sieve and more than 50% retained on the #4 sieve. The granulaz layer will help distribute floor slab loadings, ease construction, prevent capillary water rise and aid in drainage. ]ob No. 95-142 Ground Engincering ConsuNsnts, Gw. 6 4088 Oak Street (6) The fill beneath the floor slabs should be placed in uniform lifts at maximum 8-inches loose and compacted to at least 95 % of the maximum standard Proctor density, (AS'I'M D- 698), and within 2% of the optimum moisture content. (7) Some of ihe bedrock encountered during tlus investigation will be expansive when placed in a compacted condition. Consequendy, it should not be used as Sll beneath floor slabs. The bedrock can be used for fill near the bottom of fills outside building azeas. (8) All plumbing lines should be carefully tested before operation. Where plumbing lines enter through the floor, a positive bond break should be provided. Flexible connections should be provided for slab-bearing mechanical equipment. The precautions and recommendations itemized above will not prevent the movement of floor slabs if the undedying expansive materials are subjected to altemate wetting and drying cycles. However, the precautions will reduce the damage if such movement occurs. Foundation Walls: Foundation walls and retaining structures which are laterally supported and can be expected to undergo only a moderate amount of deflection may be designed for a lateral eacth pressure computed on the basis of an equivalent fluid unit weight of 50 pcf for the on-site soils. All foundation and retaining structures should be designed for appropriate surcharge pressures such as adjacent buildings, traffic and construction materials. M upward sloping backfill surface also increases the earth pressures on foundation walls and retaining structures. backfill should be carefully placed in uniform lifts and properly compacted. Care should be taken not to overcompact the backfill since this could cause excessive lateral pressure on the walls. Some settlement of deep foundation wall backfills will occur even if the material is placed correctly. Underdrain Svstem: Our experience indicates ihat local perched water table conditions can develop on top of the bedrcek after landscape ircigation starts; therefore, the lower level of the structure should be protected by an underdrain system. Job No. 95-142 Cround Engineering Cona+ilenta,lnc. 7 4088 Oak Street An underdrain system should consist of a layer of free draining granulaz material beneath the floor slab connected to perimeter drains. Free draining granulaz material used in the drain system should contain less than 5% passing ihe #200 sieve and more than 50% retained on the #4 sieve. 'Iye fiee draining granular maferial used in the drain system should mcet graded filter criteria so that it is capable of removing watec &om beneath the structure without becoming clogged by particles of natural soils which migrate into ihe filter. The drains should consist of 4-inch drain pipe surrounded above the invert with a minimum of six (6) inches of free draining granular material. The drain lines should be placed at least one (1) foot below the floor level and graded to sumps at a minimum slope of 1 percent. 'I'he granulaz underdrain system should be sloped to multiple sumps where water can be removed by pumping or gravity drainage. Surface Drainase: The following drainage precautions should be observed during construction and maintained at all times a8er the facility has been completed: (1) Excessive wetting or drying of the foundation excavations and underslab azeas should be avoided during construction. (2) Eacterior backfill should be moistened and propedy compacted. (3) The ground surface surrounding the exterior of the building should be sloped to drain away from the foundation in all directions. We recommend a minimum slope of six (6) inches in the first ten (10) feet. (4) Roof downspouts and drains should discharge well beyond the limits of the backfill. (5) Landscaping which requires watering and lawn sprinkler heads should be located at least ten (10) feet from foundation walls. (6) Plastic membranes should not be used to cover the ground surface adjacent to foundation walls. Limitations: This report has been prepazed in accordance with ganerally accepted soil and foundation engineering practices in this azea for use by the client for design purposes. The conclusions and recommendations submitted in this report are based upon the data obtained from lob No. 95•142 Ground Engineering ConsuitaMS, lna 8 4088 Oak Street the exploratory hole drilled at the location indicated on the exploraYory hole plan. The nature and extent of variations betwcen ihe actual conditions and the exploratory hole may not become evident until excavation is performed. If during constructioq soil, rock and groundwater conditions appeaz to be different from those described herein, this office should be advised at once so that reevaluation of the recommendations may be made. We recommend on-site observation of excavations and foundation bearing strata by a soil engineer. Swelling soils occur on this site. These soils aze siable at their naturel moisture content but can slirink or swell with changes in moisture. The behavior of swelling soils is not fully understood. The swell potential of any particular site can change erratically both in lateral and vertical extent. Moisture changes also occur erratically, resulting in conditions which cannot always be predicted. The recommendations presented in this report are based on the current state of the art for foundations and floor slabs on swelling soils. The owner should be aware that there is a risk in construction on these types of soils. Perfonnance of the building will depend on following the recommendations and in proper maintenance after coustruction is complete. As water is ihe main cause for volume change of these soils, it is necessary ihat the changes in moisture content be kept to a minimum. This requires judicious irrigation and providing positive surface drainage away from the building. Any distress noted in the building should be brought to the attention of a professionalengineer.
