REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec. R806. The installation of ridge venting requires the installation or existence of soffit venting. For calculation purposes, one hat or turtle vent equal to one-half of one square foot of opening. Effective December 1, 2014, asphalt shingle installations require an approved midroof inspection, conducted when 25-75 percent of the roof covering is installed, prior to final approval. Installation of roof sheathing (new or overlay) is required on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and mid -roofs may be called in at the same time, one hundred percent of the sheathing must be complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave and secured in place is required to be provided for all roof inspections. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. In order to pass a final inspection of elastomeric or similar type roof coverings, a letter of inspection and approval from the manufacturer technical representative stating that "the application of the roof at (project address) has been applied in accordance with the installation instruction for (roof material brand name) roof covering" is required to be on site at the time of final inspection. Exceptional Exteriors, LLC and the Client agree to the terms and conditions set forth on both front and back of this contract. Exceptional Exteriors, LLC (hereinafter "Exceptional Exteriors" and "EEL") will perforin roof replacement and exterior repairs to the Premises for the Client as approved and specified by Client's insurance company (collectively, the "Scope of Work") for the replacement cost value (RCV) of the insurance claim. This contract shall be binding upon the parties and their respective successors, heirs, legal representatives and permitted assigns. 16. An account is past due when more than thirty (30) days have passed after the roofing work was completed. If an account is past due by 60 days a late charge of $120 will occur. If an account goes to into collections, Client shall pay EEL interest at twenty- one (21 %) percent per annum for all past due accounts. If Client does not pay all sums when due under this contract to EEL, then Client agrees to pay all costs and fees incurred by EEL for its collection, including reasonable attorney fees and costs. EEL reserves the right to place a mechanic's lien on Client's property for any default in the timely payment of amounts due under this contract. . Client shall pay EEL interest at twenty-one (21%) percent per annum for all accounts 90 days past due. 18. The parties waive the right to a jury trial in any action or proceeding based upon or related to any aspect of the transaction in connection with this contract, or any document executed or delivered in connection with such transaction. 19. Venue for any legal action brought pursuant to or involving this contract shall be in the City and County of Denver, State of Colorado, or any other location mutually agreeable to the parties. 20. On December 8, 2010, EVstudio observed the reported structural concerns at the notched and bored stud packs. The structural concerns observed and possible solutions are based on a limited observation without any prior information of the structural issues. This report will provide a summary of observed damage and recommendations to correct structural concerns. Observations Interior — Frame Walls (roughly middle of dwelling) • The existing stud wall supporting the upper floor has a few notched and bored studs to allow for new electrical wiring. • The holes into the studs were less than 1" wide. • The holes were located at roughly the center of the studs and not in a manner to critically damage the studs. Recommendation Based on the observations and existing load conditions the observed studs were structurally sufficient. No repairs are needed. Limitations • The recommendations of this report are based on the available data and the limited accessibility of the elements and components of the structure. • The conclusions and recommendations submitted in this report are based on accepted general practice and are limited to the data available to our office.
