Effective December 1, 2014, asphalt shingle installations require an approved midroof inspection, conducted when 25-75 percent of the roof covering is installed, prior to final approval. Installation of roof sheathing (new or overlay) requires an approved inspection prior to installation of ANY roof coverings and is require on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave and secured in place is required to be provided for all roof inspections. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. Bulk Sample Analytical Procedures Bulk samples of construction materials are analyzed according to the Methodfor the Determination of,,lsbestm in Bulk Building Materials (EPA/600/R-93/116) which the EPA recommends as the "preferred substitute" to the old interim Method, In areas where the "new" Method is silent, FRS follows the "old" Interim Method and/or specific rulings issued by the various regulatory agencies. Each separable layer or portion of the sample is individually analyzed and reported, along with each layer's volume percent as a part of the total sample. Additionally, an arithmetic composite for the total sample, as received by the lab, is reported for layered samples. Information on the nature and quantity of materials at the sampling site, which is unavailable to FRS, must be used by the client to determine whether or not this arithmetic composite of the sample is representative of the materials, as they exist in place. This same information must be used by the client to determine whether or not any layered material, as it exists in place, should be treated as several different materials or as a composite material. Each sample layer is prepared and analyzed in refractive index oils that are certified by the manufacturer, Cargille Laboratories. Refractive indices are measured by the Dispersion Staining, Becke Line, or Oblique Illumination methods, according to which method is most applicable to the sample material. Asbestos content is reported as a percent of the sample, and is a visual estimation based upon gross sample examination and comparisons with published calibration charts during PLM analysis. Sample Ashing Ashing is a preparatory procedure carried out to eliminate many interferents and asbestos "look-alikes." A portion of the sample is placed in a furnace at 500 degrees Celsius for between i and 4 hours: the actual time varies according to sample type. The amount of asbestos present in the original sample is calculated, based upon the weight loss due to asking. If the sample had a 501/o weight loss, and the visual estimation of asbestos present was 20%, the amount of asbestos reported would be I0%. All ashed samples are so noted on the Final Report. Non-Asbestiform Amphibole Minerals Particles of NON-ASBESTIFORM anthophyliite, tremolite, and actinolite with length to width ratios equal to or exceeding 3:1 and overall lengths greater than 5 microns are occasionally detected in some balk samples. While such particles do meet the "old" legal definition of fibers, they do not meet the classical definition of asbestos because they do not occur as bundles of parallel fibers (often exhibiting splayed ends), matted masses of individual fibers, or long, flexible, and/or curved individual fibers. The majority of mineral fibers in a sample must display these characteristics as well as exhibit a length to width ratio of at least 20:1 for the mineral to be designated "asbestos." 'these guidelines are specified in the "New" Method: EPAt6Q0/R-93/116. Continued water intrusion was apparent throughout the basement level of the residence, resulting in water damage to the flooring and walls as well as visible mold growth on the walls throughout the basement. 2.0 Scope of Work The purpose of the inspection and sampling was to determine the level of airborne mold spores within the basement and main level of the residence when compared to the naturally occurring ambient levels outside as well as to identify the areas that are impacted by mold growth. No other areas within the residence were included in the visual inspection or the air quality sampling. 3.0 Certifications Mr. Kreuscher is a Residential Mold Inspector with DS Consulting, Inc (see Appendix for certification). 4.0 Site Description The residence is a single -level, single-family residence. It has a finished basement and a two -car attached garage. 5.0 Overview of Findings The results of the inspection and sampling indicate that the level of airborne mold spores within the basement level was significantly higher than the outdoor ambient concentrations at the time of inspection and sampling and visible mold was observed. The sampling also indicates that the level of airborne mold spores within the main level was higher than the outdoor ambient concentrations at the time of inspection most likely due to the significantly elevated levels throughout the basement. Visual Inspection The visual inspection within the residence focused on the basement. The sampling also indicates that the level of airborne mold spores within the main level was higher than the outdoor ambient concentrations at the time of inspection most likely due to the significantly elevated levels throughout the basement. There are no established regulations designating a safe or normal concentration of airborne mold within indoor air, therefore; the interpretation of indoor air quality samples utilizes a comparison between the samples collected indoors to those collected outdoors. Using this comparison, if the indoor mold concentrations are higher than the outdoor concentrations by a factor of 10 or more, then the indoor airborne mold levels are considered "elevated" and indicate that a contributing mold condition may be present. This comparison was developed by the American Conference of Governmental Industrial Hygienists (ACGIH) and the Indoor Environmental Standards Organization (IESO) for the interpretation of airborne mold spore sampling results. The following mold spores were found at elevated levels within the basement and main level of the residence: • Penicillium/Aspergillus at a ratio of 219:0 in basement, 13:0 on main level • Chaetomium at a ratio of 78:1 in basement, 7:1 on main level • Stachybotrys at a ratio of 41:0 in basement Please see Appendix B for the full Laboratory Report 6.0 Sampling & Analytical Procedures The survey and assessment were conducted by a Residential Mold Inspector qualified by experience, education, and training in the recognition and management of elevated indoor mold levels. The survey was performed in accordance with recommended procedures and approved sampling techniques. These procedures call for the visual inspection of the building for suspect mold areas and the collection and analysis of representative samples of suspect areas. A non-destructive building inspection was conducted. Walls, columns, perimeter pipe chases, and other inaccessible areas were not broken into in order to locate hidden mold. It should be noted that additional mold might be located in such inaccessible areas. Roofs and exterior areas were not included in the survey. As mold conditions can continuously change, the quantities and locations of mold identified in this report are approximations and subject to field verification. The results contained within this report are limited to the date and time the inspection was conducted. DS used a high-volume pump manufactured by GAST, calibrated to 15.0 liters per minute with a high- volume rotameter prior to collecting the first air quality sample. Each air quality sample was collected by drawing air through spore -trap cassettes for a total of 5 minutes per sample for a total volume of air equal to 75.0 liters per sample. The calibration of the pump was also checked after the last air quality sample was collected. Collected samples were analyzed by a Colorado Registered Microbiological Laboratory. Analysis of the samples was performed on the "date reported," as listed in the sample analysis report. 7.0 Laboratory Analysis Explanation For each sample, the first column is labeled "Raw Count." This is the actual number of each mold spore found for the amount of air collected. The next column, labeled "Spores/M3," is mathematically extrapolated from the "Raw Count" column and the volume of air collected. For example, assume that one sample has collected 0.1 cubic meters of air, and one spore is found in that sample. Assuming an even distribution of spores, 1.0 cubic meter of air would then contain 10 spores. 8.0 Conclusion & Recommendations Due to the existence of significant amounts of visible mold and the high levels of airborne mold spores present throughout the basement, it is recommended that area undergo a professional mold remediation. The main level should also be a part of this remediation however; no visible mold was observed during the limited inspection on the main level. Cleaning and air scrubbing should be conducted throughout the main level. As some people are more susceptible than others to mold -related health problems due to factors such as age, relative overall health, allergies, existing respiratory ailments, etc., even small levels of mold spores can be potentially dangerous. Potential health effects and symptoms associated with mold exposures include allergic reactions, asthma, and other respiratory complaints. There is no practical way to eliminate all molds and mold spores in the indoor environment; the way to control indoor mold growth and prevent mold growth is to control moisture levels and fixing the source of the water problem. Taking measures to reduce indoor humidity to 30%-60% will reduce the chances of mold growth. The underlying moisture problem within the residence/building must be corrected to prevent recurring mold growth. Indoor moisture can result from numerous causes, such as: facade and roof leaks; plumbing leaks; condensation; and high relative humidity, etc. An emphasis should be placed on ensuring proper repairs to the cause of the moisture problem so the water intrusion and moisture accumulation is stopped and does not recur - resulting in additional mold growth. 9.0 Disclaimer & Limitations Currently there are no government regulations for evaluating potential health effects of mold contamination and remediation. This information is subject to change as more information regarding mold becomes available. This report, the information and the interpretations contained within, are designed to follow currently known industry guidelines and recommendations for the interpretation of microbial sampling, analysis, and remediation. The sewage screening was attempting to locate any bacteria that were indicative of human feces. The screening was cultured to find Fecal Coliforms, E. Coli. Fecal Coliform bacteria live in the digestive tracks of warm-blooded animals, including humans, and are excreted in the feces. E. Coli is a part of the group of Fecal Coliforms. The purpose of the inspection and sampling was twofold. One was to determine if any sewer residue/bacteria was present within the air of basement level. Two was to determine if any sewer residue/bacteria were present on the impacted surfaces of the basement following the recent sewer loss. Air Samples A single air sample was collected from the basement level bathroom (where the pipe break took place). The screen was cultured to find Total Coliforms and E. Coli within the air, which none were found. Surface Swab Samples One surface swab sample was collected from the basement level bathroom floor. The sample collected from the bathroom was negative for Fecal Coliforms and E. Coli. 2.0 Scope of Work The scope of the limited inspection and sampling was limited to specific areas of the residence as affected by the original sewer loss/pipe break. These areas included the basement level bathroom where the break took place underneath the shower pan. 3.0 Site Description & Observations The floor was removed within the bathroom shower to expose the pipe and one foot flood cuts were made around the shower pan. Small amounts of debris were found within the bottom plates of the walls in the bathroom shower pan and were also found sporadically throughout the bathroom. 3 4.0 Certifications The sewer release testing was conducted by Mr. Rich Kreuscher with DS. Mr. Kreuscher is an Industrial Hygienist (IH) with DS. 5.0 Sampling & Analytical Procedures A non-destructive building inspection was conducted. Walls, columns, perimeter pipe chases, and other inaccessible areas were not broken into in order to locate hidden damaged or other visual evidence. It should be noted that additional sewage might be located in such inaccessible areas. Roofs and exterior areas were not included in the survey. The results contained within this report are limited to the date and time the inspection was conducted. DS used a high-volume Thomas pump manufactured by Asbestos Analytics for the air-quality samples. Mr. Kreuscher calibrated the pump to 28.0 liters per minute for the bacterial sample with a primary standard device prior to collecting the sample. The calibration of the pump was also checked after the sample was collected. The swab samples were collected using approved swabs for sewage screening purposes. The swabs were wetted using the reservoir at the base of the provided sampling tube, then ran along the surface of the floors in each area sampled covering approximately one square feet of area each. Collected samples were analyzed by Aerobiology Laboratory Associates, Inc., a Colorado -State -Certified Microbiological Laboratory, with EMLAP Accreditation #102977. Analysis of the samples was performed on the "date reported," as listed in the Aerobiology Laboratory sample -analysis report. Penicillium/Aspergillus group spores are characterized by their small size, round to ovoid shape, being unicellular, and usually colorless to lightly pigmented. There are numerous genera of fungi whose spore morphology is similar to that of the Penicillium/Aspergillus type. Two common examples would be Paecilomyces and Acremonium. Although the majority of spores placed in this group are Penicillium, Aspergillus, or a combination of both. Keep in mind that these are not the only two possibilities. 2. Ascospores are sexually produced fungal spores formed within an ascus. An ascus is a sac-like structure designed to discharge the ascospores into the environment, e.g. Ascobolus. 3. Basidiospores are typically blown indoors from outdoors and rarely have an indoor source. However, in certain situations a high basidiospore count indoors may be indicative of a wood decay problem or wet soil. 4. The colorless group contains colorless spores which were unidentifiable to a specific genus. Examples of this group include Acremonium, Aphanocladium, Beauveria, Chrysosporium, Engyodontium microconidia, yeast, some arthrospores, as well as many others. 5. Hyphae are the vegetative mode of fungi. Hyphal elements are fragments of individual Hyphae. They can break apart and become airborne much like spores and are potentially allergenic. A mass of hyphal elements is termed the mycelium. Hyphae in high concentration may be indicative of colonization. 6. Dash (-) in this report, under raw count column means 'not detected (ND)'; otherwise 'not applicable' (NA). 7. The positive -hole correction factor is a statistical tool which calculates a probable count from the raw count, taking into consideration that multiple particles can impact on the same hole; for this reason the sum of the calculated counts may be less than the positive hole corrected total. 8. Due to rounding totals may not equal 100%. 9. Analytical Sensitivity for each spores is different for Non-viable sample when the spores are read at different percentage. Analytical Sensitivity is calculated as spr/m3 divided by raw count. spr/m3 = raw counts x (100/ % read) x (1000/Sample volume). If Analytical Sensitivity is 13 spr/m3 at 100% read, Analytical Sensitivity at 50% read would be 27 spr/m3, which is 2 times higher. 10. Minimum Reporting Limits (MRL) for BULKS, DUSTS, SWABS, and WATER samples are a calculation based on the sample size and the dilution plate on which the organism was counted. Results are a compilation of counts taken from multiple dilutions and multiple medias. This means that every genus of fungi or bacteria recovered can be counted on the plate on which it is best represented. 11. If the final quantitative result is corrected for contamination based on the blank. the blank correction is stated in the sample comments section of the report. 12. Analysis conducted on non-viable spore traps is completed using Indoor Environmental Standards Organization (IESO) Standard 2210. 13. The results in this report are related to this project and these samples only. 14. For samples with an air volume of < 100L, the number of significant figures in the result should be considered (2) two. For samples with air volumes between 100-999L, the number of significant figures in the result should considered (3) three. For example, a sample with a result of 55,443 spr/m3 from a 75L sample using significant figures should be considered 55,000. The same result of 55,443 from a 150L sample using significant figures should be considered 55,400 spr/m3. 15.
