Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

CONTRACT CONDITIONS
Payment to Dutch Brothers, Inc. - (hereinafter called DBI) as provided in the Contract shall be due
and payable in FULL upon substantial performance of work by DBI. Owner agrees that no part of
the contract price will be held pending city inspections or work orders thereof. This Contract shall
become binding only upon written acceptance by DBI or by an authorized agent, upon
commencement of the work. All legal collection charges resulting from lack of payment will be the
responsibility of the customer. Owner further agrees that there may be a mechanic's lien applied
to this property if payment is not made upon installation of the roof. In the event of nonpayment, a
service charge in the amount of 1 and Y2% per month may be added to the contract amount until
the account is paid in full.
The Limited Warranty provided by DBI pertains to the labor only. Any warranty pertaining to the
materials is the warranty of the material manufacturer. The DBI labor warranty is void if the
consideration due DBI remains unpaid ninety (90) days following the completion of the installation,
or upon the occurrence of any event rendering the material warranty voidable, including any and
all acts of God. DBI will not be responsible for interior damage beyond expiration of labor
warranty.
1) All proposals are subject to approval of DBI Management.
2) Replacement of deteriorated decking, facia boards, roof stacks, ventilators, flashing or other
materials, unless otherwise stated in this contract, are not included and may be charged as an
extra on a time and material basis.
3) This contract constitutes the entire understanding of the parties, and no other understanding,
collateral or otherwise, shall be binding unless in writing and signed by both parties.
4) This proposal will expire in thirty (30) days from date unless extended in writing by the
Company. Due to fluctuations in the cost of materials, we reserve the right to revise our price as
necessary with costs in effect at that time.
5) If material has to be reordered or restocked because of a cancellation by the customer, there
may be a restock fee up to fifteen percent (15%) of the contract price.
6) Labor warranty does not cover damage to roofs caused by lightning, 45 mph wind, hurricane,
tornadoes, hail storm, impact of foreign objects or other violent storm or casualty, or damage to
roofs due to settling,distortion, failure or cracking of the roof deck. DBI is not responsible for
damage to walls, ceilings, sheetrock or any pre-existing construction flawed areas which may
result from repairs or installation in the normal course of business.
7) DBI will not be responsible for cracked cement, punctured a/c lines, or damage which could be
averted by customer's due dilligence with reasonable notice.
8) DELAYS, ETC. Purchacer hereby acknowledges that DBI may be subject to delays
occasioned by inclement weather, labor disputes, and material supply shortages which are
beyond the control of DBI, and Purchaser hereby accepts any delays occasioned by one or all of
these circumstances in the installation of the roof or roof repair.
