1. This permit was issued bas the information provided in the permit application and accompanying plans and
specifications and is subject to the compliance with those documents, and all applicable statutes, ordinances,
regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must
be made in writing and received prior to the date of expiration. An extension of no more than 1B0 days made be granted
at the discretion of the Chief Building Official and may be subject to a fee equal to one- half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to
the standard requirements, fees and procedures for approval of any new permit. Re- issuance or extension of expired

No. 8, Part B, adopted December 21, 2007, and effective January 30, 2008 and the Colorado Air
Pollution Prevention and Control Act C.R.S. (25 -7 -101 and 25 -7 -501 et seq). This notice signifies
that the structure was inspected for asbestos, luminous exit signs (containing radioactive material),
and Ozone - Depleting Refrigerants and the demolition contractor has properly notified the
Colorado Department of Public Health and Environment pursuant to Regulation No. 8, Part B.
As a contractor, you maybe subject to other demolition licenses and permits, depending on the
requirements of the county and municipality in which the work is being performed. The Colorado
Department of Public Health and Environment, Air Pollution Control Division, strongly suggests
that you check with county and municipal authorities in order to determine any other local
building /permitting requirements that must be meta
Please note that certain asbestos - containing materials (ACM) may remain in the structure
during demolition. Therefore, any demolition debris left behind after the completion of post -
demolition site cleanup may constitute a "reason to know of asbestos - contaminated soil' at
the site, subject to the requirements of Section 5.5 of the Solid Waste Regulations (6 CCR
1007 -2, Part 1).
THE ORIGINAL APPROVAL NOTICE MUST BE POSTED ON SITE AT ALL TIMES.
Immediately notify the Asbestos/IAQ Unit of project modifications by fax (number above) or e-mail
(address above) and the appropriate county health department by fax. Project modifications include
changes in the scope of work or the scheduled work dates, etc.

This wood has been preserved by pressure treatment with an
EPA-registered pesticide containing chromated copper arsenate (CCA)
to protect it from termite attack and decay. Wood treated with CCA
should be used only where such protection is important.
CCA penetrates deeply into and remains in the pressure-treated wood
for a long time. Exposure to CCA may present certain hazards.
Therefore, the following precautions should be taken both when
handling the treated wood and in determining where to use or dispose
of the treated wood.
USE SITE PRECAUTION FOR CCA PRESSURE-TREATED WOOD
* Wood pressure-treated with CCA preservatives may be used inside
residences as long as all sawdust and construction debris are
cleaned up and disposed of after construction.
* Do not use treated wood under circumstances where the preservative
may become a component of food or animal feed. Examples of such
sites would be structures or containers for storing silage or food.
* Do not use treated wood for cutting-boards or countertops.
* Only treated wood that is visibly clean and free of surface
residue should be used for patios, decks and walkways.
* Do not use treated wood for construction of those portions of
beehives which may come into contact with the honey.
* Treated wood should not be used where it may come into direct or
indirect contact with public drinking water, except for uses
involving incidental contact such as docks and bridges.
HANDLING PRECAUTIONS FOR CCA PRESSURE-TREATED WOOD
* Dispose of treated wood by ordinary trash collection or burial.
Treated wood should not be burned in open fires or in stoves,
fireplaces or residential boilers because toxic chemicals may be
produced as part of the smoke and ashes. Treated wood from
commercial or industrial use (e.g., construction sites) may be
burned only in commercial or industrial incinerators or boilers in
accordance with state and Federal regulations.
* Avoid frequent or prolonged inhalation of sawdust from treated
wood. When sawing and machining treated wood wear a dust mask.
Whenever possible, these operations should be performed outdoors to
avoid indoor accumulations of airborne sawdust from treated wood.
* When power-sawing and machining, wear goggles to protect eyes from
flying particles.
* After working with the wood, and before eating, drinking, and the
use of tobacco products, wash exposed areas thoroughly.
* If preservatives or sawdust accumulates on clothes, launder before
reuse. Wash work clothes separately from other household clothing.
* Treated wood should be dry before handlinq.
