Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those

For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

General Project Terms

This proposal and contract (hereinafter also referred to as "Agreement") between Axe Roofing, LLC (the "Company') and the property owner(s) (the "Customer")
supersedes any understanding, written or oral agreement between the parties and will be subject to applicable laws in the State where work is performed, with the
following conditions:
1.Company shall assist Customer in submitting, processing, negotiating and settling claims with its property insurance coverage for damage to Customers property. If
Customer chooses not to have Company perform work, after claim has been settled, Customer shall pay Company 15% of the total amount approved by the
insurance company for payment of the repairs and the deductible, including any supplemental claims, overhead and profit as consideration for Company's assistance
with the claim.
2. All Agreements subject to review and approval by Company management. Any prices quoted will remain valid for thirty (30) days from the date offered. The
Customer will indemnify and hold the Company harmless for any proposed or understood changes to the Agreement not written on both signed copies of the
Agreement. The Customer is responsible for obtaining any and all applicable product and related color approval(s) from any relevant Homeowners Association(s)
(HOA) and Historical Society.
3. Customer hereby agrees: (i) to fully cooperate with Company in Company's efforts to negotiate with Homeowner's insurance company; (ii) to provide complete and
accurate information in order to assist Company in its efforts; (iii) to continually provide information should additional information become known to Customer; and (iv)
to allow Company to file supplemental insurance claims.
4. The Company is acknowledged to be a general contractor and is entitled to receive all overhead and profit charges applicable to an insurance claim. All payments
should be made payable to Axe Roofing, LLC. with no payments to be made in cash. The Company's insurance is produced by Moody Insurance and can be
contacted at 303-824-6600.

5. Customer and Company agree that Customer is not assigning to Company any interest in a legal claim, but instead agrees to pay Company the full amount
allowed by the Customers insurance company scope of loss/estimate, any agreed to upgrades and the Customers deductible.
6. This Agreement shall survive the death, incapacity, and/or bankruptcy of Customer and shall be binding on Customers heirs, administrators, estate, successors or
assigns. Customer agrees not to contract with any other company, to process and/or submit any claims unilaterally, to cause others to submit claims on Customers
behalf or in any manner attempt to bypass or circumvent this agreement during the time period as identified above. This Agreement may only be modified by written
agreement signed by both parties.
7. Customer agrees Company may enter into such contracts with other companies or individuals as it deems necessary to complete the Scope of Work. Any
materials not used in the completion of the Scope of Work shall be the property of Company.
8. This Agreement shall be governed by the laws of the State of Colorado. This Agreement comprises the entire agreement between Company and Customer. No
oral or prior written agreements shall have any binding effect and this agreement shall supersede all prior writings, statements or representations, whether written or
oral.
9. If Customer breaches this agreement or if Company engages an attorney to enforce the agreement or collect the amounts due under this agreement, Customer
shall pay to Company Company's reasonable attorneys fees and all costs of collection or enforcement. Should default be made in payment of this Agreement, the
Company reserves the right to place a mechanic's lien upon the property and pursue collection of funds from the Customer. Default is defined as having thirty (30)
days passed from completion of stated work by the Company. The Customer agrees to pay interest of eighteen percent (18%) per annum applied to all unpaid
amounts due to the Company until the Agreement is paid in full. The Customer also agrees to pay all costs incurred by the Company for the collection of the
Agreement or insurance settlement amount including reasonable attorney fees.
10. The Customer acknowledges that work processes can be noisy, messy and can cause vibrations/shifting of the property's structure and its contents. The
Customer will indemnify and hold the Company harmless for pre-existing damage or deterioration, improperly installed concrete and/or soils, minor interior damage
(i.e. nail pops, cracks in plaster, drywall or paint) and minor exterior damage (i.e. slight scratching/denting of gutters or paint, minor landscaping damage, hairline
fractures or minor chipping in concrete). Rotten, brittle or improperly maintained or installed siding/trim/stucco may be damaged during installation of wall flashings
(when applicable) for which the Company shall be indemnified and held harmless. The Company may repair such damage with additional labor and materials costs
charged to the Customer if requested by the Customer, and the Customer agrees to not withhold any payment to the Company for appearance or resolution of any
aforementioned issues. The Customer shall indemnify and hold the Company harmless for the proper functioning of any evaporative coolers, HVAC units, solar
systems, skylights and ventilators installed prior to work being performed by the Company. The Company agrees to inspect and pay for time and materials necessary
to repair substantial damage (if caused and inspected by the Company) to Customer's property during the repair/replacement process as long as such items are not
already excluded above.
11. The Company shall be indemnified and held harmless for failure to perform due to: A) the Customer and/or their insurance company's or mortgage company's
delays, omissions or errors B) weather C) inability to acquire material from normal sources D) labor strike or controversies E) lack of access or additional damage to
the property for which stated work is to take place, and F) any other circumstance beyond the control of the Company whether of similar or dissimilar nature. If any
provisions of this Agreement should be held invalid or unenforceable, the validity and enforceability of the remaining provisions of this Agreement shall not be
affected.
12. Company shall not assume or be held liable for any damages to personal property or physical injury as a result of vibrations caused during the execution of the
work described in this Agreement.
13. Company will make all reasonable efforts to protect the property from any further damages. However Company shall not be held liable for wear and tear to
driveways, parking lots, walkways, lawns, shrubs, or other floral or vegetation caused by truck, equipment, materials, work force or debris.
14. ANY WARRANTIES ON THE PRODUCTS USED ARE THOSE MADE BY THE MANUFACTURER. COMPANY DISCLAIMS ALL WARRANTIES, EITHER
EXPRESS OR IMPLIED, INCLUDING ANY IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. COMPANY DOES NOT
ASSUME NOR AUTHORIZE ANY OTHER TO ASSUME FOR IT ANY LIABILITY IN CONNECTION WITH THE SALE OR USE OF ANY PRODUCTS. CUSTOMER
SHALL NOT BE ENTITLED TO RECOVER FROM COMPANY ANY CONSEQUENTIAL DAMAGES, DAMAGES TO PERSON OR PROPERTY, DAMAGES FOR
LOSS OF USE, LOSS OF TIME, LOSS OF PROJECTS OR INCOME, OR ANY OTHER INCIDENTAL DAMAGES.
15. Company and its insurers shall be held harmless for alleged or actual damages/claims as a result of mold, algae or fungus. Company and its insurers shall
exclude all coverage, including defense, damages related to bodily injury, property damage and clean-up directly or indirectly in whole or in partfor any action brought
by mold, including fungus and mildew, regardless of the cost, event, material, product or workmanship that may have contributed concurrently or in any sequence to
the injury or damage that occurs.
16. Company's performance under this Agreement shall be excused in the event of fire, explosion, flood, severe weather, accident, strike, governmental act,
embargo, shortages of materials, computer system failure, war or military action, riot, civil disturbance or any other event beyond the control of Company or which
materially affects the economic basis of the bargain ("force majeure"). Company's performance in the event of a force majeure shall be excused for as long as the
cause continues, without liability. If the force majeure continues unabated for ninety (90) consecutive days, Company may, at its option, cancel this Agreement
without liability.
17. Customer agrees that Company's performance under this Agreement shall not commence until the Homeowner pays the insurance proceeds and deductible to
Company for the Scope of Work. Customer agrees to limit any and all damages to the Company to the amount of the contract.
18. Customer, may cancel this transaction at any time prior to midnight on the 3rd business day after the date of this agreement.
19. 1 (We), the Customer(s), have read and fully understand this Agreement. I have been provided with a copy of this Agreement. I have also been notified that I may
cancel the Agreement within three (3) days from the date hereof.

Objective: Remove the paneling in the exi[ing lower level bedroom and family room.
Re-model the SW lower level room into a Family Room and SE lower level room into a
bedroom with a closet. Finish and complete a bathroom using the existing plumbing
rough in.
1. Heating Duct work already finished
2. Electric outlets already exist. Add at least 1 additional electric outlet unless more
are required by code. Add other electrical outlets as required by code. Add on/off
switch for over head lighting needs to be installed at each room entrance (
presently pull switch.) Provide light fixtures. Permit required.
3. Replace doors with new hollow core doors.
4. Drywall rooms floor to ceiling, with R-13 insulation between dry wall and
exterior wall.
5. Put in a drop ceiling 2'x4' sections (or similar)
6. Frame and drywall a close[ in second bedroom with sliding doors. Closet should
have one top shelf and hanging rod.
7. Pull permit for work if necessary
8. Exclude flooring
9. Complete bathroom including fixtures including shower stall, toilet, 1 sink,
