1. This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, anew permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, anviolation of any provision of any

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

6. Replace (4) autocaulk pipe boots and (8) painted attic vents. Replace (1) damaged vent cap.
7. Install Malarkey modified asphalt ridge shingles (64')
8. Clean up repair area upon completion.
9. Prices shown are good for 30 days from the proposal date. After this time, prices may increase.
10. Warranties offered apply only to the roofing work that we install.
11. Work can begin 2-3 weeks following approval and completed in 1-2 days, weather permitting.
12. Workers Compensation, Liability Insurance and State Sales Tax are included.
13. Payment Schedule: 45% of contract due when we order your materials. If this down payment is not received as agreed, the scheduled

Thank you for the opportunity to observe the grading and drainage for your project at the
referenced location. We also reviewed the drainage study associated with the site titled "Final
Drainaee Report 45`h and Parfet Citv of Wheat Ridee Colorado" prepared by Foothills
Engineering Consultants, Inc. dated February 15, 2001. Based on our visual observations of
the site it appears that the ground surrounding the building at 4465 / 4467 Parfet Street
has been graded in accordance with the aforementioned drainage study. Following is a
list of issues of which you should be aware:
1. We did not complete any surveying to determine if the grading meets the requirements
of the drainage study and our opinion is based only upon visual observations.
2. We did not review the rest of the site for compliance with the drainage study and this
letter applies only to the ground surrounding the building at the referenced address.
3. The Drainage Study that we reviewed was not signed by either the engineer who
prepared the study of the City of Wheat Ridge. This could indicate that it is not the
final study completed for this site but we aze assuming based on conversations with
you that it is.

Dear Vince,
As you requested, we have reviewed the loads on the one-foot cantilever at each
side of the duplex. We have made the following assumptions from the architectural
drawing in our files:
1) The first floor joists run left to right with a maximum span of 16' and a one-
foot cantilever.
2) The second floor joists run le$ to right with a maximum span of 17'.
3) The roof trusses bear on the common wall between the units and have a
maximum span of 17' and a 2' overhang.
4) The load on the cantilever consists of two walls, the second floor, and the
roof.
5) The floor joists are 9'/z" TJI 150's at 16" O.C. or equal.
Reinforcement of each floor joist at the first floor cantilever is necessary. Please
see attached drawing for reinforcement details. Blocking between eacn floor joisis and a
rim board on the end of the joists is still required. Also, the cantilevered area must be
insulated and the bottom enclosed with treated plywood.

Yesterday I received the C.O. checklist for a duplex located at 4465-4467 Parfet Street (I believe this is for
only the northerly duplex unit). The applicant, Mr. Vince Hopkins, has not completed Ihe requirements per
City Code for this duplex. I cannot sign the C.O. request until the outstanding issues have been resolved.
Section 26-415-B of the City Code of Iaws requires that all developments other than single-family or
duplexes (i.e., commercial and subdivision developments), subsequent to final grading, but prior to the
issuance of C.O., must submit a grading certificate from the engineer-of-record who did the original Final
Drainage Report. This ensures that the work was performed in a satisfactory manner and that the drainage
will function properly. Therefore, the grading certificate serves to protect to not only the future occupants but
the surrounding neighborhood as well.
Back in April, Mr. Hopkins begged and pleaded with me to sign off on the C.O. (the Grading Certificate had
not yet been submitted to the City) for the southerly half of the duplex because he had a closing date set "and
really needed to close the dea] so he can sell the duplex" unit. Mr. Hopkins stated to me at that time that the
original engineer-of-record was out of business, and he would have to hire a new firm to obtain the Grading
Certificate. Knowing that Mr. Hopkins has been working on a"shce-string" budget (while also keeping in
mind his reputation), I said I would sign off on the southerly duplex under one condition:
• Mr. Hopkins must hire a new engineering firm (since he has another unit to build, he will need to hire
a firm anyway) and obtain the required Grading Certificate for the above-mentioned duplex p ~nor to
any additional Building Pernvts or Certificates of Occupancv bein¢ issued. Mr. Hopkins was
ageeable to this condition of approval.
As of today, July 29, 2004, I am still holding a Building Pernvt Application for 4475-4477 Parfet Street (the
last unit to be constmcted in this subdivision). Mr. Hopkins submitted this to the Ciry on April 20, 2004
immediately following the issuance of the above-mentioned Certificate of Occupancy. Kristy has made
several attempts to contact Mr. Hopkins regarding this issue I have yet to receive the certificate for the
duplex I approved for C.O. 3 months ago.
In conclusion, without some cooperation from Mr. Hopkins in this matter, I cannot approve any further
Building Permit or C.O. requests until Mr: Hopkins has complied with standazd City Code requirements.

PLANNING DEPARTMENT REOUIREMENTS•
1. Compliance with all relevant ordinances, resolutions, variances, approved site plan, etc.
2. Completion of all landscaping per plan and/or Zoning Code.
3. Completion of all pazking improvements and driveway connections per plan and/or zoning code.
4. Completion of handicapped pazking spaces and signage.
5. Removal of all construction work trailers, materials and debris.
6. Removal of construction debris from neighboring and/or surruunding sites.
7. Removal ofall constrvction signs, banners, etc.
8. Separate permits must be obtained for permanent signs with the permit being approved prior to installation of signs.
9. Separate permits shall be obtained for fences aud/or retaining walls exceeding 48" in height, if not indicated on
approved plans.
PUBLIC WORKS DEPARTMENT•
1. Compliance with all relevant ordinances, resolutions, vaziances, special exceptions, etc.
2. Site grading to be completed for positive drainage away from building and if applicable, in compliance with the
overall grading plan(s).
3. A Drainage Certifica6on Letter is to be fumished from the engineer of record stating that the paving and drainage
improvements have been installed per approved plans and speci5cation.
4. If the property is affected by Streeucape requirements, any required Maintenauce Agreement needs to be sigied prior
to requesting a certiScate of occupancy and any required escrow in lieu of the construction of such items needs to be
paid in full.
5. lf applicable to the site, streetlight requirements per Excel Energy design bcation shall be completely installed.
6. If applicable, all Ciry of Wheat Ridge approved surveying dceuments, Such as the Final Plat and/or required separate

As you know, your request for zone change and outline and final development plan were
approved by City Council on February 26, 2001. There were no additional conditions of
approval.
A motion was made and carried for approval of the combined preliminary and final development
plan with the following conditions:
1. The private drive be constructed to the City's standazd for pavement thickness.
2. An equivalent caliper inch of trees be replaced on the property.
3. The developer be required to contribute 5% of the land value for park land
purposes and that this be paid prior to the recording of the development plan.
4. The fence along the west property shall be required to be eight feet in height
unless, at the time of construction, the property owners adjacent to the west of this
pro erty unannnously agree that it can be reduced to six feet in hei ht.

7. The homeowners association's covenants be revised to address maintenance of the

An additional condition of approval by the Planning Commission which did not have to be added
to the review set but which still will be required is that the license agreement with Public Service
company be signed prior to issuance of the building permit.

The preliminary and final development plan sheets referenced above need to be renumbered 1 of
3, 2 of 3 and 3 of 3 so that they make up a single, recorded plan set. A blackline photographic
mylar of each of the five sheets will need to be submitted for recording. Of the conditions of
approval, the following condition numbers must be added to the development plan: #'s 1, 2, 4, 6,
8, and 9. Item #3 (5% of the purchase price of the property) must be paid prior to the issuance of
building permits. Item #'s 2 and 5 must be added to the landscape plan in the recordable plan
set.
I have provided Barbara Newman with information regarding the different varieties of
ornamental trees which can be used. I would like to set up a meeting with you, Barbaza, myself
and the city forester to discuss the desired tree vaziety. Item #7 must be addressed and reviewed
by public works prior to the issuance of a building permit.
Once the conditions have been added into the plan set, please submit two blueline copies of all
the sheets for our review prior to the production of a recordable, mylaz set.
Keep in mind that prior to the issuance of a building permit, a complete landscape plan consistent
with Section 26-32 of the Wheat Ridge Code of Laws needs to be submitted and approved.
Attached is a copy of the landscaping regulations for your review.
