Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

subaect to the compliance with those documents, and all applicable statutes, ordinances, regulations, po icies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the original permit fee.
3• If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This contract includes all materials and labor to complete work per price and scope of the
customers insurance claim.
All the scope listed in this insurance claim will be completed unless otherwise noted here:
Excluding line items 25, 26, 27, 28, 30, 31, 32, 33, 34, and 35 totaling $6120.45 for personal contents, family room
carpet, fence repair, and shed repair. Insured also will remove and manipulate all furnishings and contents.

All permit fees are in addition to the contract amount and will be supplemented (billed) to the insurance company for
reimbursement.
Customer Agreement: We have partnered with GCV Servicing to handle Administrative work for us to make restoring
your home a smooth process. They will be working directly with your insurance and mortgage companies to make
sure that payments associated with your project are released, and received, in a timely fashion, which allows me to
focus on making sure your project is completed as quickly as possible and to the quality standards you expect and
deserve. GCV Servicing will be calling you to introduce themselves. By signing our contract, you are also authorizing
your insurance and mortgage carriers to speak directly with them regarding your project, and to make payments
directly to GCV Servicing. As part of the service they provide to us, they will also be contacting you once all work
has been completed to make sure you are satisfied with the work we have done. If you have any concerns or
problems, please let them know and they will contact us directly to take care of your issues. Lastly, by working with
GCV Servicing we are also affiliated with StormScamHelp.com, a nonprofit company that only partners with
qualified, trustworthy contractors, to ensure that your project is completed in a professional manner and to your
satisfaction. If you have any questions, please contact EXG at the office or call your EXG represenative. or you can
contact GCV directly.
I/we, the homeowner and the insured, authorize representatives of Traveler's Home and Marine Insurance Company
to speak with Genesis Contractor Solutions d/b/a GCV Servicing ("GCV Servicing") on matters involving the claim
number set forth above (the "Claim"). The above insurance company is also authorized to make checks payable to
GCV Servicing and mail them directly to GCV Servicing. My claim number is SNY3961001 H. I/we, the homeowner
and mortgage holder, authorize representatives of (Mortgage Company) to speak with GCV Servicing on matters
involving the Claim, the loan associated with the loan number referenced below, and the release of funds. The above
mortgage company is also authorized to make checks payable to GCV Servicing and mail them directly to GCV
Servicing. My loan number is Loan# I/we, the homeowner, authorize GCV
Servicing, their representatives and affiliates to communicate with Exterior Guru LLC and my Insurance Carrier
regarding the Claim, any supplements, and any additional damage discovered during the construction process, which
also includes General Contractor's Overhead and Profit. I/We, understand that all right, title, and interest in any and
all insurance proceeds due from the Insurer identified above regarding my claim number listed above is the property
of GCV Servicing. This is in accordance with the contract between Exterior Guru LLC & GCV Servicing. I/We, do
hereby grant a Limited Power of Attorney to GCV Servicing to endorse Insurance and Mortgage checks as needed.
This attached document is a partnership agreement with Genesis Contractor Solutions and PinPoint Claims to provide
a smooth transaction. Genesis Contractor Solutions (also GVGC) is a 3rd party quality control and administrative
assistance.
Insurance Claims: This attached document provides legal information on how the State of Colorado requires Exterior
Guru to handle insurance claims as well as other information. The owner grants permission for Exterior Guru to speak
with their Insurance Carrier. All insurance claims are subject to final agreement with the carrier for final costs as
they pertain to the scope of work. Work is to be completed per scope and price of insurance claim. All insurance
proceeds released to owner for the scope listed in this contract are due to Exterior Guru upon completion of work and
upon funds received by customer from their insurance company. Detailed information regarding an Insurance Claim
can be found on the attached Insurance Claim document.
Product Selection: This attached document will be used to verify your product and color selections if not stated in
this contract.
Warranty - Shingle Template: This attached document is a copy of Exterior Guru's warranty. We offer a Life -Time
Labor and Material Re -Roof warranty. This document will be provided to the home owner once the work is complete
and payment in full has been received.
Pardon Our Dust: This attached document may be shared with your neighbors to let them know exterior work will be
taking place at your home and provides them with a phone number to call if any debris gets on their property.
How To Prepare: This attached document informs the customer how to prepare for the day(s) work will be
completed.
Payments for Insurance Claims: Customer's AVC check is due at the time of contract and will be deposited into
an escrow account to be held to purchase materials for their project. All proceeds from the customer's insurance
claim in this contract are due to Exterior Guru. Recoverable depreciation (RCV) is due upon customer receipt from
their insurance company.
Terms and Conditions: This attached document informs the customer of the terms and conditions of the contract.
Upon signing this contract I/We, acknowledge that I/We have read the attached documents and agree to their terms
and conditions.
