As requested, A. G. Wassenaar, Inc. (AGW) visited the residence located at 4335 Otis Street in Wheat
Ridge, Colorado on July 18, 2018. AGW was asked to collect bulk samples of suspect building materials
which may be impacted by future renovation activities. The following report outlines the results of AGW's
limited asbestos survey.
EXECUTIVE SUMMARY
The limited asbestos survey was conducted on July 18, 2018. The survey included the partition wall
between the living room and kitchen of the residence. The purpose of this asbestos survey was to identify
the presence of asbestos containing building materials (ACBMs) present prior to renovation. This survey
was performed in general accordance with the Colorado Department of Public Health and Environment
(CDPHE) Regulation No. 8. AGW collected six (6) samples for this project which were submitted for analysis
at an independent, third party laboratory. None of the samples collected for this project were found to
contain asbestos.
SCOPE OF WORK
AGW's scope of work for this project included collection of bulk samples of suspect asbestos containing
building materials by personnel certified as Asbestos Inspectors by the State of Colorado Department of
Public Health and Environment (CDPHE) and the Environmental Protection Agency (EPA).
The scope of work for this assessment included:
• Conduct a visual inspection of identified areas to locate suspected asbestos containing building
materials (ACBMs);
• Collect bulk samples from each suspect material for asbestos content analysis by Polarized Light
Microscopy (FLM)atanindependent laboratory;
• Prepare a written report which presents the results of the sampling conducted; and
• Provide recommendations for the abatement or appropriate management of any asbestos.
METHODS
Sampling Procedures
Suspect materials were grouped into homogeneous areas based on their physical characteristics, color,
texture, and functional application. Suspect materials were divided into one of three categories based on
the following homogeneous area classification schemes recognized by regulatory agencies:
1) Thermal systems insulation (TSI): any type of pipe, boiler, tank or flue insulation.
2) Surfacing material: sprayed or troweled onto structural building members.
3) Miscellaneous: all other suspect materials, including flooring, ceiling tiles, floor coverings, other
insulations, and finishing materials.
Random bulk samples were collected according to the guidelines published as Environmental Protection
Agency Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654.
All samples collected were placed in sealed bags for delivery to the laboratory, and samples were hand -
delivered to the laboratory immediately following sample collection. Samples collected were given unique
identification numbers in accordance with established AGW protocols for sample numbering.
Analytical Method
All bulk samples collected were analyzed by Polarized Light Microscopy (PLM) for asbestos content in
accordance with guidelines established by the EPA's Method for the Determination of Asbestos in Bulk
Building Materials, method EPA/600/R-93/116.
Reservoirs Environmental, Inc. (Reservoirs) was contracted to perform laboratory analysis of the bulk
samples collected according to methods required by the United States Environmental Protection Agency.
Reservoirs is fully accredited by NVLAP for asbestos analysis (Lab Code #101896). This laboratory is also
AIHA accredited (Lab ID#101533, Certificate #480). The results of the laboratory analysis for asbestos are
summarized in Table 1 below, and laboratory data is included as an attachment to this report.
Assessment
An assessment of all identified ACBMs was performed based on the material's friability, current condition,
and potential for disturbance. Friability is defined as the ability of the material, when dry, to be crumbled,
pulverized or reduced to powder by hand pressure. The current condition of each ACBM was also
determined. Materials were identified as being in Good, Damaged, or Significantly Damaged condition
based on the amount of damage observed. A material in Good condition does not have physical damage
or deterioration observed, or has very limited damage or deterioration. Materials which are placed into
the Damaged category, have less than 10% distributed or 25% localized damage observed. Physical
damage greater than 10% distributed or 25% localized results in the material being described as
Significantly Damaged. Finally, the potential for disturbance was determined for each ACBM identified
during this survey based on its potential for damage due to contact with the material, vibration, and air
erosion. Materials were placed into one of three categories, either Low, Medium or High potential for
disturbance.

LIMITATIONS
This limited asbestos inspection included only the living room and kitchen dividing wall within the
residence. Other suspect materials exist within the building which were not sampled during this visit. If
additional suspect materials not identified in this report are to be impacted, AGW recommends that all
work cease until the material can be evaluated by a certified Asbestos Inspector.

As requested, A. G. Wassenaar, Inc. (AGW) visited the residence located at 4335 Otis Street in Wheat
Ridge, Colorado on July 18, 2018. AGW was asked to collect bulk samples of suspect building materials
which may be impacted by future renovation activities. The following report outlines the results of AGW's
limited asbestos survey.
EXECUTIVE SUMMARY
The limited asbestos survey was conducted on July 18, 2018. The survey included the partition wall
between the living room and kitchen of the residence. The purpose of this asbestos survey was to identify
the presence of asbestos containing building materials (ACBMs) present prior to renovation. This survey
was performed in general accordance with the Colorado Department of Public Health and Environment
(CDPHE) Regulation No. 8. AGW collected six (6) samples forthis project which were submitted for analysis
at an independent, third party laboratory. None of the samples collected for this project were found to
contain asbestos.
SCOPE OF WORK
AGW's scope of work for this project included collection of bulk samples of suspect asbestos containing
building materials by personnel certified as Asbestos Inspectors by the State of Colorado Department of
Public Health and Environment (CDPHE) and the Environmental Protection Agency (EPA).
The scope of work for this assessment included:
• Conduct a visual inspection of identified areas to locate suspected asbestos containing building
materials (ACBMs);
• Collect bulk samples from each suspect material for asbestos content analysis by Polarized Light
Microscopy (PLM) at an independent laboratory;
• Prepare a written report which presents the results of the sampling conducted; andCity of

METHODS
Sampling Procedures
Suspect materials were grouped into homogeneous areas based on their physical characteristics, color,
texture, and functional application. Suspect materials were divided into one of three categories based on
the following homogeneous area classification schemes recognized by regulatory agencies:
1) Thermal systems insulation (TSI): any type of pipe, boiler, tank or flue insulation.
2) Surfacing material: sprayed or troweled onto structural building members.
3) Miscellaneous: all other suspect materials, including flooring, ceiling tiles, floor coverings, other
insulations, and finishing materials.
Random bulk samples were collected according to the guidelines published as Environmental Protection
Agency Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654.
All samples collected were placed in sealed bags for delivery to the laboratory, and samples were hand -
delivered to the laboratory immediately following sample collection. Samples collected were given unique
identification numbers in accordance with established AGW protocols for sample numbering.
Analytical Method
All bulk samples collected were analyzed by Polarized Light Microscopy (PLM) for asbestos content in
accordance with guidelines established by the EPA's Method for the Determination of Asbestos in Bulk
Building Materials, method EPA/600/R-93/116.
Reservoirs Environmental, Inc. (Reservoirs) was contracted to perform laboratory analysis of the bulk
samples collected according to methods required by the United States Environmental Protection Agency.
Reservoirs is fully accredited by NVLAP for asbestos analysis (Lab Code #101896). This laboratory is also
AIHA accredited (Lab ID#101533, Certificate #480). The results of the laboratory analysis for asbestos are
summarized in Table 1 below, and laboratory data is included as an attachment to this report.
Assessment
An assessment of all identified ACBMs was performed based on the material's friability, current condition,
and potential for disturbance. Friability is defined as the ability of the material, when dry, to be crumbled,
pulverized or reduced to powder by hand pressure. The current condition of each ACBM was also
determined. Materials were identified as being in Good, Damaged, or Significantly Damaged condition
based on the amount of damage observed. A material in Good condition does not have physical damage
or deterioration observed, or has very limited damage or deterioration. Materials which are placed into
the Damaged category, have less than 10% distributed or 25% localized damage observed. Physical
damage greater than 10% distributed or 25% localized results in the material being described as
Significantly Damaged. Finally, the potential for disturbance was determined for each ACBM identified
during this survey based on its potential for damage due to contact with the material, vibration, and air
erosion. Materials were placed into one of three categories, either Low, Medium or High potential for
disturbance.

This is a letter regarding the proposed remodel of the kitchen for the residential
structure located at 4335 Otis Street, Wheat Ridge, CO. The wall separating the
kitchen and the living room will be removed and replaced with 3-1%"X11%" BCI Versa
Lam 2.0 engineered beam to carry loads from the ceiling joist as well as two roof
kickers. The beam will be located in line with the north kitchen wall and will have a
maximum span of 15'-6". The beam may be installed above the ceiling joists with
hangars used to support the ceiling joists to the beam. A minimum 2-2x4 posts should
support the ends of the beam in the walls shown. Continuous support will need to be
provided to the concrete foundation below the main floor or with sufficient blocking. The
loads used for the design is 10 pounds per square foot (psf) dead, 20 psf live (attic) and
30 psf roof (snow). Design is based on 2012 IRC.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.
