Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come

to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

By signing this agreement the homeowner authorizes North West
Roofing to represent the homeowner's best interest in pursuit of
roof replacement or repair due to storm damage. This contract does
not obligate the homeowner or North West Roofing until the claim is
approved by the insurance Company and accepted by North West
Roofing. Upon Approval of the claim, North West Roofing will
complete specific work at the price agreeable to the insurance
company and to North West Roofing. There is no additional cost to
the homeowner except for their deductible. The agreed upon price
will become the final contract price and North West Roofing will
receive all insurance proceeds and supplements for the work
completed by North West Roofing.

THIS CONTRACT BETWEEN NORTHWEST ROOFING (THE "COMPANY") AND THE CUSTOMER(S) (THE "CUSTOMER-) WiLL BE SUBJECT TO ALL APPROPRIATE LAWS. REGULATIONS AND
ORDINANCES, AND THE FOLLOWING SPECIALTERMS AND CONDITIONS: _.
This contract is subject to the approval of Company's Management & Credit Department without exception.
ALL CHECKS FOR PAYMENT MUST BE MADE PAYABLE TO NORTH WEST ROOFING THERE WILL BE NO EXCEPTIONS. HOMEOWNER WILL STILL BE LIABLE TO NORTH WEST ROOFING. FOR
PAYMENT OF THIS CONTRACT IF CHECK IS MADE OUT TO ANY OTHER ENTITY OR PERSON.
If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the remaining provisions of this agreement shall not be affected.
The customer agrees that the equity in this property is security for the contract.
The company shall not be liable for the failure of performance due to labor controversies, strikes, fire, weather; inability to obtain material from usual sources, acts of God, or any other
circumstances beyond the control of the company.
If the customer defaults in payment of this contract, charges shall be added from the dates of the default at the rate of one and one half percent per month (18% per annum) with a
minimum charge of $2.00 per month. If placed in the hands of an attorney for collection. Company shall be entitled to collect all attorneys' fees, and costs expended in enforcing the
collection of any charges payable under the contract.
You may cancel the contract and obtain an in full refund of any deposit within seventy- two hours after entering the contract. In addition, you may resend the roofing contract pursuant to
C.R.S. 6.22-104.
If this contract is cancelled by the customer later than seventy-two hours from receipt of the written notice identified in 6-22-104 or if the customer cancels the contract later than 72 hours;
for a reason other than the receipt of written notice Identified in 6.22-104. The customer shall pay The Company twenty percent (20%) of the contract prices as liquidated damages, not as a
penalty, and the Company agrees to accept this amount as a reasonable and just compensation for cancellation and the efforts that the Company expended in relation to the subject of this
contract.
If material has to be reordered or restocked because of a cancellation by the customer other than permitted by 6.22-104. Company will charge a restocking fee equal to fifteen percent (15%)
of the contract price.
The contract does not include the replacement of deteriorated decking, roofing jacks, ventilators, Flashing, or other materials unless Included in the contract and will then be charged on a
time and material basis.
The Company's Limited Workmanship Warranty (3 years) covers the labor to correct problems with the installation of the roof. The warranty does not cover damage to roofs caused by
lightning, gales (SOMPH), hurricane, tornado, hailstorm, impact of foreign objects, or damage to roofs due to the settlement, distortion, failure or cracking of the roof deck, walls or the
foundation of a building.
The Company is not responsible for any damage below the roof due to leaks caused by excessive wind, ice, or hail during the period of the warranty. The Company's warranty covers roof
only.
The Company is not liable for any structural movement of the foundation or walls, settling, Cracks in dry -wall, cracking or breaking of the driveway during the roofing or loading processes, of
minor scratching or denting of rain gutters.
Any pictures, wall hangings, lights, and collectables, both indoors and out MUST be secured by homeowner. Any damage to personal items during installation will not be the responsibility of ,
the company. The Company will also not be responsible for nail damage to people, animals, pets, or property. Pets should not be left unattended during installation.
Any alteration or deviation from scope of work involving extra cost will be executed only upon written orders, and become an extra charge to the above estimate.
The Customer agrees to be solely responsible for carrying fire, tornado and other necessary insurance.
This contract and labor warranty shall not be assigned and are nontransferable unless specified in writing.
The Customer will not receive a labor, material, or manufactures warranty if the contract is not paid in full.
This contract constitutes the entire understanding of the parties, and no other understanding, collateral or otherwise, shall be binding unless in writing signed by both parties
This agreement is composed of front and back pages and all materials incorporated herein by reference and shall be considered the entire agreement by the parties. Any representation,
statement, or other communications not written in this contract are agreed to be immaterial, and not agreed on by either party, and do not survive the execution of this contract.
The company has the right to order excess materials. All excess materials belong to the Company.
The Company reserves the right to require Customers to file for supplemental insurance claims if insurance adjustor measurements are used and prove to be incorrect, the Company
discovers unknown conditions, or due to an increase in cost.
The Customer has sole responsibility to provide safe -keeping areas for all pets. Company shall not be responsible for gates left open during or after Company's work.
Supplements paid by the insurance company for additional labor and/or materials needed beyond the original scope of repairs are part of this contract as if contained herein and will be paid
to Company either by the Homeowner or Homeowner's Insurance Company.
The final full payment shall not be delayed while waiting for city, county or other building inspection department to inspect the work.
Homeowner acknowledges Company is a General Contractor and as such will be entitled to 10% profit and 10% overhead if approved by Insurance company as part of the claim.
Homeowner is solely responsible for Homeowners Association (Covenant approval) of roofing products and color.
Company is not responsible for damage to stucco, siding, and wall abutment due to re -Flashing, as a result of Instailing new Flashing.
Company is not responsible for satellite dish realignment.
North West Roofing reserves first (1) and second (2) right to correction of defects or resulting damage with ample time for correction. North West Roofing will not be responsible financially if
homeowner calls a private contractor. "
The waiver of a breach of a provision of this contract shall not operate as a waiver of any subsequent breach by any party.
Company reserves the right to bill for percentage of work complete. Customer understands that a stop work order may be issued If requested progress payment is not met.
Company reserves the right to bill on a pay per trade policy. Customer agrees to pay in full at the completion of each trade, prior to the Company starting anew trade.
A maximum allowable retainage of 5% of original contract price shall be held for any punch -out items. Customer agrees to pay in full at completion of each trade.

This letter will confirm our visual observations made on October 6, 1996.
The proposed house wiil have a crawl space.
At the time of our October 6th visit the excavation had been dug to a depth of
11/2 to 3 feet and drilled piers were in place. The soil exposed on the bottom
consisted of slightly moist, stiff sandy clay. The soil is similar to that encountered in
our borings. In our opinion the house should be founded on a Type 5 foundation
consisting of piers. A structural floor system should be used. Slab on grade floors
will have a moderate risk of movement. The other recommendations in our report
296 131, dated August 8, 1996 also should be observed.

and garage addition inspections were performed on the single family residence located at
3960 Miller Street, Wheatridge, Colorado. The pertinent information regarding the
inspecfion is as follows:
PIER DRILLING INSPECTION - OCTOBER 4, 1996
Soil Pro ide
The soil prof:de encountered during drilling consisted of 0' to 8'-0 ° of cday and 8' to 18'-
0" of claystone bedrock. No water was found in the holes at the time of drilling. Soils
report requirements were met at all pier locations inspected
Reinforcement Concrete
All of the piers were reinforced with three no. S bars, grade 60, for their entire length.
The concrete used consisted of a 3, 000 psi, 314 " stone aggregade mix. All of the piers
were properly corrstructed accordirrg to our recommendations and the recommendaiions
in the soids report.
FOUNDA770N WALL REINFORCING STEEL INSPECTION - OCTOBER 11. 1996
The foundation walls were reinforced horizontally with two no. 5 bars, grade 60, at the
top and bottom. All of the bars were properly lapped and ded The fourrdation wall
forms were properly centered over the dridled pier foundation system, and the reinforcing
steel from the piers extended to the top row of reinforcing in the foundation wall. Paper
void material was properly installed between the pier locations in the bottom of the
forms. Reinforcing steel was properly placed at the foundation wall step downs and
window openings.
GARAGE ADD7770NINSPECTION - OCTOBER 17, 1996
The foundation walls were reinforced horizontalty with rivo no. 5 bars, grade 60, at the
top and bottom. All of the bars were properly lapped and tied. The foundation wall
forms were properdy centered over the pad foundation system. Paper void material was
properly installed between the pier locations in the bottom of the forms.
