inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Budding
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements; fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Subject: Observations at Mrs. Pastore Residence, 3985 Miller Street, Lot 87,
Cambridge Park, Wheat Ridge, Colorado. i
Deaz Mr. Odean:
As requested I visited the subject house on December 10, 1997, Stan
McDaniel from Cambridge Park accompanied me. The purpose of the visit was to
observe the conditions in the crawl space and the outside drainage.
For the purpose of discussion, it is assumed the house faces south. The crawl
space is about 3 feet below the outside grade. The soil on the bottom of the
crawlspace had shrinkage cracks in the middle and was very wet and muddy toward
the west and north sides. A sump has been installed in the northwest corner. The
water level in the sump had risen to about 15 inches below the bottom of the crawl
space and the imert of a drain line entering the sump was about 20 inches dowa The
pump tumed on while I was [here and the water was drawn down to below the inlet.
After stabilizing, a small trickle of water was coming out of the pipe. It is not known
how the inlet is connected to the underdrain. It appeared that there may be a small
leak in a drain pipe below the shower in the master bedroom. Some areas of the
wood plate and Tll's were damp.
Outside, on the west side the drainage is poor. The ground surface slopes
down from the house on the west. An underdrain has been installed on the exterior
close to the west wall. Backfill and gravel have been placed up to the level of die top
of the foundation walL On the north side, backfill is in contact with the wood siding.
The drainage is flat on the north. The drainage around the east and sou[h is fair to
good.

It is important that good surface drainage be provided azound the house. It is
critical that the backfill not be above the 6-inch limit, as specified in the code. Water
is wetting the wood members and must be corrected. In order to create proper
drainage it may be necessary to provide a grade separation between this house and the
one to the west. A swale may be needed on the north to provide the necessary slope
away from the house. Judicious lawn irrigation is needed. Outside drainage
improvements should be completed first. If these do notimprove the moisture in the
crawl space, then interior underdrains and more ventilation may be required.

Dear Mr. Odean:
We observed the excavation for the subject lot on October 25, 1996. The
proposed house will have a crawl space.
At the time of our visit tiie excavation had been completed except for a 10 foot
wide strip on the nonh that was not excavated. The excavation was dug to a depdi of
3 to about 41h feet deep. The soils exposed on the bottom consisted of sandy clay,
clayey sand and sand and were similar to that encountered in our borings. In our
opinion the house should be founded on a Type 4 foundation consisting of spread
footings with a maximum soil pressure of 2,500 psf and a minimum dead load
pressure of 800 psf. Footings should be placed on firm, undisturbed soil. The other
recommendations in our report 296 131, dated August 8, also should be observed.
