2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding R1/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for

Saturday, February 28`h, since I never received this form at the time of closing on the
above-referenced property. In addition, in going through the closing papers, I never
received a copy of the soil inspection and am hereby requesting you to furnish me with a
copy.
Please find attached to the 11-Month Warranty Letter a copy of an inspection on the
above-referenced home performed by Coyle Inspection Engineers, Inc. on March 2, 1998.
A review of this inspection will reveal that there are several major structural probtems
which need immediate attention, plus many cosmetic items.

Note:The key to preventing or minimizing structural movement is to prevent moismre accumulation in the foundation
subsoils. The following recommendations should be observed as a minimum:
1. Maintain or provide positive slope away from the foundation. 1" of fall per foot for the 6rst 5 feet is
recommended. [f soil is to be added around the foundation it should be impermeable, clay type material.
1 Maintain or provide splash blocks or downspout extenders at all gutcer downspouts to discharge at least 5 feet
away from the foundation.
1 Maintain or create a non-irriga[ed zone around the foundation for a distance of 3 feet min., 5 feet preferred.
4. Do not use plastic weed barriers adjacent to the foundation; use a mesh fabric weed barrier that allows the
moisture in the soil to evaporate.

Note: Heat exchanger cracks and/or flue blockage can resul[ in carbon monoxide poisoning or death. Since cracks or
blockage can develop at any time, annual inspection and testing ofthe heat plant is recommended. Headaches or
diuiness may be symptoms indicating possible carbon monoxide leakage that should be investigated. The visual
inspection of the heat exchanger is an attemp[ to identify cracks, holes or other defects. However, it is generally not
possible ro view 100% of the heat exchanger without disassembling the heat plant, which is beyond the scope of this

Dear Mr. Odean:
This letter will confirm our visual observations made on October 6, 1996. We
previously reported on a visit to this house in a letter dated September 27, 1996. The
proposed house will have a walkout basement.
At the time of our October 6th visit the excavation had been completed and the
drilled piers were in place. The excavation was about 8 feet deep on the west and
was daylighted on the east. Claystone bedrock was exposed in the high wall and a
stiff sandy clay was exposed on the ]ower side. The bedrock and soil are similar to
that encountered in our borings. In our opinion the house should be founded on a
Type 5 foundation consisting of piers. A structural floor system should be used. Slab
on grade floors wiil have a high risk of movement. The other recommendations in
our report 296 131, dated August 8, 1996 also should be observed.
