Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

required. Ice dam membrane is required from eave edge to 2' inside exterior
walls. Board sheathing with any gap greater than 1/2" requires panel sheathing

Contractor shall provide ladder for inspections. Scheduled inspections that

a.m. on the day of inspection or a re- inspection fee will be assessed.

1. This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the
State of Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable
ordinances of the City.
2. This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at the discretion of the Building Official.
3. If this permit expires, a new permit may be acquired for a fee of one -half the amount normally required, provided no

Dear Mr. and Mrs. Wells:
In accordance with your request,.l visited the property at the above location on June 9,
2009 to review the west wall of the house, without regard to other components of the
property.
Following is a report of my findings:
The property is a one story house of frame construction. Exterior sidewalls are frame
with composite siding surfaces.
An impact (I understand from a car) has broken and pushed in the siding and
underlying OSB wall sheathing (see Photo 1).
The interior surface has been removed from the wall interior, exposing the framing and
broken OSB sheathing piece (see Photo 2).
The 2x6 at 16 inch center to center wall studs are not damaged and are
realigned/vertical (as per four foot level). There are not signs of related structural
damage.
The damaged OSB sheathing and composite siding areas need to be replaced.
An inner vapor barrier needs to be added, and R19 value Batt insulation need to be
installed in between the wall studs.
The interior sheetrock can then be replaced.

Dear Mr. Cavander:
As requested, Hollingsworth Associates, Inc. performed a subsurface study for your new resiJence
planned at 4470 Lee Street, Wheat Ridge, Colorado. The study was conducted in accordance with
our verbal proposal to you.
Prnnnsed Constructinn: The proposed residence will be a single story, modidar structure.
Foundation loads are expected to be light as is t}pical for this type of construction. IF the design
varies from the project description above, the recommendations presented in this report should bc
re-evaiuated.
Site Conditions: At the time of our field investigation on June 30, 1999, the site of the proposed
residence was vacant. The site had a grass and weed cover. The ground surface slopect downward
from south to north with a difference in c]evation of approximately 3 fcct.
Suhsoil Conditions: In general, the subsoil conditions encountered in the exploratory boring
consisted of6 inches of topsoil and 8%: feet of loose to medium densc, silty sand ovcrlying medium
dense, silty sand and gravel with cobbles and boulders, for the depth drilled, 13 feeL No free water
was encountered in the exploratory borings at the time ofdrilling. The gradations of typical samples
ofthe silt sand are presented on Fig. 3. The upper silty sands setde both under load and when wettecl
as indicated by the consolidation test results presented on Figs. 4 and 5. The location of the
exploratory boring is shown on Fig. 1. A log of the exploratory boring along with a legend and notes
is shown on Fig. 2.

Fnundation Recommendations: Considering the subsurface conditions encountered in the
exploratory boring and the nature of the proposed construction, we recommend the residence be
founded on spread footings placed on undisturbed natural soils.
The design and construction criteria presented below should be observed for a spread footing
foundation system. The construction details should be considered when preparing project
documents.
(1) Footings placed on the undisturbed natural soils should be designed for an allowable soil
bearing pressure of 2,000 psf.
(2) Based on experience, we estimate total settlement for footings designed and constructed as
discussed in this section will be approximately 1'/z inches. Differential setUements across
the residence are estimated to be approximately one inch.
(3) Spread footings placed on granulaz soils should have a minimum footing width of 16 inches
for continuous footings and of 24 inches for isolated pads.
(4) Exterior footings and footings beneath unheated azeas should be provided with adequate soil
cover above their bearing elevation for frost protection. Placement of foundations at least
30 inches below the exterior grade is typically used in this azea.
(5) Continuous foundation walls should be reinforced top and bottom to span an unsupported
length of at least 10 feet.
(6) Areas of loose material encountered within the foundation excavation should be removed
and the footings extended to adequate natural bearing material. As an altemate, the loose
material may be removed and replaced with non-expansive fill material compacted to 100%
of the maximum standazd Proctor density neaz the optimum moisture content. New fill
should extend down from the edges of the footings at a 1 horizontal to 1 vertical projection.
(7) Granulaz foundation soils should be compacted with a smooth vibratory compactor prior to
placement of concrete.
(8) A representarive of the soil engineer should observe all fooring excavations prior to wncrete
placement.

Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support lightly to
moderately loaded slab-on-gade wnshuction. To reduce the effects of some differential movement,
floor slabs should be separated from all bearing walls and columns with expansion joints which
allow unrestrained vertical movement. Floor slab control joints should be used to reduce damage
due to shrinkage cracking. We suggest joints be provided on the order of 15 feet on centec The
requirements for slab reinforcement should be established by the designer based on experience and
the intended slab use.
A minimum 4-inch layer of free-draining gravel should be placed beneath the slabs. This material
should consist of minus 1'h-inch aggregate with less than 50% passing the No. 4 sieve and less than
5% passing the No. 200 sieve. The granulaz layer will ease construction
Fill placed beneath floor slabs should be a granulaz material similar to the on-site soils. The soil
engineer should evaluate the suitability of proposed fill materials. Fill should be placed and
compacted to at least 95% of the maacimum standard Proctor density neaz the optimum moisture
content. The natural soii encountered at shallow depth is suitable for use in compacted fills beneath
floor slabs.
Surface Drainaigge: The following drainage precautions should be observed during construction and
maintained at all times after the residence has been completed.
(1) Excessive wetting or drying of the foundation excavations should be avoided during
construction.
(2) Exterior backfill should be adjusted to neaz optimum moisriue content and compacted to at
least 95% of the maximum standazd Proctor density in pavement areas and 90% of the
maximum standard Proctor density in landscaped azeas.
(3) The ground surface surrounding the exterior of the residence should be sloped to drain away
from the foundation in all directions. We recommended a minimum slope of 6 inches in the
first 10 feet in unpaved azeas and a minimum slope of 3 inches in the first 10 feet in paved
areas.
(4) Roof downspouts and drains should discharge well beyond the limits of all backfill.

Limitatinns ~ This report has been prepared in accordance with generally accepted soil and foundation
engineering practices in this area for use by the client for design purposes. The conclusions and
recommendations submitted in this report are based upon the data obtained from the exploratory
boring and the proposed type of construction. The nature and extent of subsurface variations across
the site may not become evident until excavation is performed. If during construction, fill, soil, mck,
or water conditions appear to be different from those described herein, this office should be advised
at once so reevaluation of the recommendations may be made. We recommend on-site observation
of excavations and foundation bearing strata by a representative of the soil engineer.

Final Drainage Study and Erosion Control Plan
Dear Mr. Fenwick,
This letter is to inform you that the final drainage study, and
the erosion control plan for the above referenced project have
been reviewed and approved for construction. Please be aware of
the following items regarding the construction of the project:
l. The proposed drainage facilities shall be constructed and
approved by the City of Wheat Ridge prior to issuance of the
Temporary Certificate of Occupancy.
2. Erosion control for the project shall be placed in conformance
with the approved drainage/erosion control plan and shall be
maintained as needed during the course of construction.
3. All drainage improvements shall be constructed in conformance
with the approved drainage report and plan.
4. It will be the responsibility of the contractor for the
project to repair any damage to the existing public
improvements on Lee Street and/or West 44th Avenue as a result
of related construction traffic in the area. Also, the
contractor will be responsible for maintaining Lee Street
and West 44th Avenue on a regular basis such they are free of
construction debris and tracking from construction traffic
accessing the site.
5. Upon completion of the drainage improvements the engineer,
Kenneth Fenwick, P.E. shall provide to the City of Wheat
Ridge a letter of certification stating that the various
improvements, as defined in the drainage report and plan, have
been accurately surveyed to confirm that their construction is
in accordance with these documents. The certification letter
shall be written and stamped by the engineer who prepared the

THIS DEVELOPMENT COOENANT, as authorized by the SOBDIVIBION REGULATIONB

Corporation, hereinafter referred to as City, and 7?~

WHEREAS, Owner is tha titled Owner of a tract of land in the City of
Wheat Ridge, County of Jefferson, State of Colorado, more fully
described as Lot 11, Longview Subdivision, City of Wheat Ridge, County
of Jefferson, State of Colorado, which property is hereinafter referred
to as tha "Property"; and
WHEREAS, Owner is in the process of constructing a new residence and
associated improvements and is desirous of obtaining a building permit;
and
WHEREAS, curb, gutter, sidewalk and street improvements bordering the
Property and neighboring tracts of land are incomplete and/or below
those standards as set forth in the SUBDIVISION REGULATIONS of the City;
and
WHEREAS, to be uniform and consistent with the existing neighboring
improvements, the completion or non-completion of those improvements as
required by SECTION FIVE: IMPROVEMENTS of the SUBDIVISION REGULATIONS
will make no significant change at the present in those streets fronting
the Property and the neighboring tracts of land; and
WHEREAS, in order to conform to those requirements as set out in the
SUBAIVISION REGULATIONS of the City, this Agreement is hereby determined
to be propar.
NOW, THEREFORE, IT ZS HEREHY AGREED THAT:
1. The responsibility for providing improvements along the street
frontage of the Property, as set out in the SUBDIVISION REGULATIONs
of the City, is hereby acknowledged by the Owner.
2. The Owner has caused certain improvements to be constructed on the
Property and associated on-site improvements.
3. The existing improvement of property frontage along 11 q 7v
is below those standards as set forth in the SUBDIVISION
REGULATIONS of the City, and the completion of those improvements
as required by Section Five: Improvements of the SUBDIVISION
REGULATIONS will make no significant change at the present in those
streets fronting the property.
4. In accordance with Section 5-45 of the Code of Laws of the City of
Wheat Ridge, Colorado, subsection E, the Department of Public works
has determined that public improvements along `4i4 70 L ~ , 5`'
need not be immediately constructed and that execution of this
Development Covenant is appropriate. The Owner agrees to
participate in engineering and construction costs for the
property's frontage along yY 7 A 5' when the street is
brought in accordance with City street standards, or as directed by
the Director of Public Works. The engineering and construction
costs will be limited to those reasonable and necessary to
construct Y Sr' to current local street standards,
including earthwork, one half street width paving, curb, gutter and
, walk, and miscellaneous and incidental items.

The Director of Public Works shall give written notice and an
estimate of costs to the Owner 30 days prior to advertisement of
any major improvements to 1IV 70 c.-.. which would affect the
Property and will submit to the Owner a detailed cost breakdown and
billing after Award of the construction contract by City Council.
The detailed cost estimate will include the Owner's portion of the
cost of curb, gutter and sidewalk and associated pavement
improvements and other items listed in Paragraph 4.
6. Upon receipt of the detailed cost estimate or billing, the Owner
hereby agrees to pay to the City, within 45 days, the full amount
of the detailed cost estimate or billing.The Owner also agrees to
pay to the City any extra costs associated with the Public
:mprovements to the Property due to variations in estimated
contract quantities or as a result of change orders within 30 days
of written request for payment by the City. The City will pay to
the Owner any overcharges paid to the City by the Owner within 45
days after final acceptance of the Public Improvements.
7. Owner agrees to pay for all public improvements or billings within
45 days of written notice; Owner further agreeing that any amount
resulting from his failure to pay for any improvement or billing
this improvement completed shall constitute a lien upon the
property above described in the actual amount of the public
improvements or billing completed, less any payments made by Owner,
plus interest on the amount unpaid at 10 percent (10%) per annum
from the date of final acceptance or billing, plus any costs
incurred by the City in collecting same, including court costs and
attorney's fees.
8. In the event an improvement District or a Special Improvement
District is created by the City to improve or provide those
requirements as set out in the SUBDIVISION REGULATIONS as they
exist at the date of execution of this Covenant, for curb, gutter,
sidewalk related street and drainage improvements, solely relating
to those streets fronting on the property and neighboring tracts of
land, the Owner or his assigns and successors agree not to oppose
its creation, or subsequent reasonable assessment of the costs to
the property.
9. In the event an Improvement Diatrict is created by the City to
provide the improvements as provided by the SUBDIVISION REGULA-
TIONS, the costs assessed against the property shall not be
disproportionate with costs assessed to other nearby and like
properties.
10. This COVENANT is executed by the Owner pursuant to the SUBDIVISION
REGULATIONS of the City of Wheat Ridge.
11. The City, in its sole discretion, may require a cash or surety bond
to be held in escrow by the City to secure completion or the public
improvements described herein. Such cash or surety bond shall be
in the sum sufficient to cover the estimated costs of the
construction of the public improvements described herein, said
estimate to be made at or about the time of the notice described in
Paragraph 5 above. Any interest earned on any escrow funds shall
be the sole property of the City.
12. The DEVELOPMENT COVENANT shall be recorded with the Clerk and
Recorder of Jefferson County, Colorado, and constitute a continuing
covenant running with the land, providing however, that this
DEVELOPMENT AGREEMENT shall be released by the City upon the
completion of the public improvements and complete and satisfactory
payment by the Owner of all costs and fees associated therewith of
the public improvements described herein, which release shall be
evidenced by a signed document attested to by the City Clerk and
bearing the City's seal, which document shall likewise be recorded.

BPBCIAL PROVISIONS AND AOREEMENT
The Parties hereto aqree that this DEVELOPMENT COVENANT, by its
terms, shall be binding upon the City and the Owner and upon the
assigns and successors thereof; and shall constitute covenants
running with the property for a period of ten (10). years from the
date of execution by the Owner.

This letter is to inform you of two requirements you will need to
fulfill as a result of my building permit review for the above
referenced address.
First, as part of the City's subdivision regulations, you will
need to execute a Development Covenant. I have included a copy
of this document for you to review. Briefly, the covenants
stipulate that the homeowner will agree to participate in the
engineering and construction costs of roadway improvements along
the property frontage should the City choose to improve Lee
Street within a ten year period. Feel free to take as much time
as is needed to review the terms of the covenant. However,
please be aware that I will need the original document executed
in my presence before a Temporary Certificate of Occupancy can be
issued for the dwelling.
Second, in order to maintain the minimum 50 foot riqht-of-way
requirement for local streets, 10 feet of property will need to
be dedicated along the Lee Street frontage. The City's right-of-
way technician, Mr. Chuck Braden can assist you with the details
