Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection

installation instructions, whichever is more stringent. In order to pass a final inspection

manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

ACCEPTANCE OF AGREEMENT
By signing this Contingency Contract, you agree to authorize Storm Guard to assist you in your pursuit of a claim under your
insurance policy and your efforts to reach agreement with your insurer on project scope and price. You may cancel this
contract with no penalty by providing written notice before midnight on the third business day after the date of your

Dear Mr. Odeau:
At the time of our visit on December 5, 1997, the underdrain had been
installed. The perforated underdrain was surrounded with gravel, the drain and gravel
were underlain with a plastic membrane and were overlain with geofabric cloth. The
top of the gravel was 89 to 98 inches below the top of the foundation wall. A sump
was located in the center of the north side of the lower level and was about 29 inches
deep. The invert of the pipe entering the sump was about 116 inches below the top of
the foundation wall. At the time of our observation, there was no water in the sump.
There was no underdrain installation along the exterior nor interior of the exterior
crawlspace walls, Mr. I lobbs was informed of our observations and he stated he
would call us to a reinspection, when the crawlspace drainage was complete.
The other aspects of the drain installation appear to be substantially in
accordance with our recommendations.

At your request, on November ll `h and 28'A, and December Is`, 1997, footing and
foundation wall inspections were petfirrmed on the single-family residence located at
3862 Lee Circle, Wheat Ridge, Colorado. The pertinent information regarding the
inspections is as follows:

The footings were constructed according to the foundation plans that were on the site.
The footings had been properly sized and reinforced with two no. 5 bars, grade 60. All
of the bars were properly lapped and tied. The footings were properly bulk-headed off at
all of the excavation step-downs. All of the interior pads were also properly sized The
refit forcen:ent for the pads was properly tied and sized according to the foundation plans.
All concrete was placed oil undisturbed natural soils, and all loose debris removed front
the footing forms prior to placement of concrete.
