1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is

received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6'nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT

As of 6/6/12 Colorado Senate Bill 38 - The Consumer Protection/Roofing Bill does not allow a roofing
company to waive, pay or rebate any insurance deductible.
Property Owner is / is not intending to make payment from the proceeds of a property and casualty
insurance company. Rams Roofing & Restoration Inc. (Rams) will provide labor and material as detailed in a
written estimate to be provided to the homeowner prior to work beinffmpleted by R ms and the insurance

This agreement does not obligate the homeowner to Rants in any way unless a scope and price is
agreed upon by Rams and the homeowner's insurance company. Payment in full is due upon completion of
work (per trade). Homeowner will hold no more than 10% of total contracted amount while waiting on a final
building inspection. The homeowner shall have no additional cost, except the policy deductible unless the
following occurs:
(1) The property owner wants to upgrade products; or
(2) The property owner's policy only covers actual cash value ACV in which case the property owner shall be
responsible for the non-recoverable depreciation; or
(3) The property owner's policy does not include "ordinance and law" coverage (code upgrades) in which case
the property owner shall be responsible for all costs not covered by the insurance settlement to comply with
current government laws and codes.
(4) Any decking needs to be replaced due to non-nailable surface. Cost to homeowner is $65.00 per OSB sheet.
Rams reserves the right to supplement the property owners insurance company for any additional materials
and/or labor as needed, Overhead and Profit (O&P) will be paid to Rams when applicable. These proceeds are
due when received from the insurance company.

Dear Mr. Lyons:
At your request, on October 8'4 and 17'h, 1997,.footing and_foundation wall inspections
were performed on the single-family residence located at 3882 Lee Circle,
Wheat Ridge, Colorado. The pertinent information regarding the inspections is as
follows:

The footings were constructed according to the_foundation plans that were on the site.
The. footings had been properly sized and reinforced with two no. 5 bans, grade 60. All
of the bars were properly lapped and tied The, footings were properly bulk-headed qff at
all of the excavation step-downs. All of the interior pads were also properly sized. The
reit forcemeni for the pads was properly tied and sized according to the.foundation plans.
All concrete was placed on undisntrbed natural soils, and all loose debris removed from
the.foolingforms prior to placement of concrete.
FOUNDATION REINFORCING STEEL INSPEU77ON OCTOBER 17, 1997
7he, foundation walls were reinforced horizontally with two no. 5 bars, grade 60, top and
bottom. All of the bars were properly lapped and tied The reinforcing sieel was
properly placed at the foundation wall step-downs and window openings.
The forms were centered on the concrete footing foundation, and vertical dowels were

At the time of our visit on October 23, 1997, the underdrain had been
installed. The perl'orated underdrain was surrounded with gravel, the total depth of
the gravel from below the pipe to above, covering the pipe ranged from 7 to 9 inches,
the drain and gravel were underlain with a plastic membrane and were overlain with
lapped roofing paper. The top of the gravel was 48 to 62 inches below the top of the
foundation wall. A sump was located in the southwest corner of the lower level and
was about 30 inches deep. The invert of the pipe entering the sump was about 70
inches below the top of the foundation wall.
We understand you will add gravel to increase the depth to 12 inches. The
other aspects of the drain installation appear to be substantially in accordance with our
recommendations.

Dear Mr. Lyons:
At your request, oil October 8th and 17th, 1997, footing and foundation wall inspections
were performed on the single-family residence located at 3882 Lee Circle,
Wheat Ridge, Colorado. The pertinent information regarding the inspections is as
follows:
FOOTING FOUNDATIONINSPECTION- OCTOBER 8. 1997
The footings were constructed according to the foundation plans that were on the site.
The footings had been properly sized and reinforced with two no. 5 bars, grade 60. All
of the bars were properly lapped and tied. The footings were properly bulk-headed off at
all of the excavation step-downs. All of the interior pads were also properly sized. The
reinforcement for the pads was properly tied and sized according to the foundation plans.
A11 concrete was placed on undisturbed natural soils, and all loose debris removed from
the footing, forms prior to placement of concrete.
F 0 UNDA TION REINFOR CING STEEL INSPECTION- OCTOBER 17. 1997
The foundation: walls were reinforced horizontally with two no. 5 bars, grade 60, top arid
bottom. All of the bars were properly lapped and tied. The reinforcing steel was
properly placed at the foundation wall step-downs and window openings.
The forms were centered on the concrete footing foundation, and vertical dowels were
spaced at 2'0" to attach the footing to the new foundation walls. All of the reinforcement
was properly placed according to my recommendations.
