2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Budding
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is

Re: General Flush Mount Roof PV System Letter

This is a. general prescriptive later for flash mount roof PV racks and panels to be installed.
Below are the requirements based on the 2012 IBC that when met mens that the existing roof
structure can support the added dead and live lands from the addition of the new PV array.
1 existing roof stcture is a minimum of 2x4 pre-engineered wood trusses at 24" with a
minitown roof slope of 4/12 and a maximum slope of 12/12.
2 existing roofing material is a single layer of asphalt shingles. Multiple Dyers and other
materials are not covered by this letter:
3 supports from the panel racks are spaced at 48" on the 24" oc trusses) and that.
vertically -adjacent L -feet do not bear on the same truss but are staggered over every other
truss(wherever possible that each truss under the panels supports some lead.
4 The standoffs are attached to tete trusses with 5/16" -diameter stainless steel lag screws with
a minimum: penetration of 2" into the truss trap chords.
The roof snow loading does not exceed 30 psf

field installation of ra;ojid shutdown cables available from SolarE,-w]MRm%Ar#M&--7 need for additional control
systems at the array.
The rapid shutdown cables are shipped in kits, each kit with 5 sets of rapid shutdown cables for the update
of 5 SolarEdge inverters. The kit is available from July 1"' 2014 with a list price of $125 ($25 per inverter),

Rapid Shutdown Functionality
To paraphrase the rapid shutdown requirement, Section 690.12 of the 2014 NEC applies to PV systems
installed on building, roofs and requires that a rapid shutdown function exists to de -energize PV source
circuits from all sources to less than 30Vdc within 10 seconds,
Since SolarEdge systems include power electronics on the roof (typically one power optimizer per module)
and are therefore able to shut off power coming from the roof, these systems are able to de -energize PV
source circuits to less than 30 Vdc In less than 10 seconds, No additional roof mounted devices are
required. The rapid shutdown Initiationis performed by turning the inverter's Safety Switch to the off
position.

The Solarde system Incorporates many additional safety mechanisms, ensuring safety for installers,
maintenance workers and firefighters. One of these mechanisms is the SafeDC"' feature, which ensures an
output of 1V& per power optimizer when the system is short down. This feature, along with the installation
of a rapid shutdown kit in the Inverter Safety Switch, expands the safety mechanisms to include the rapid
shutdown functionality.
The rapid shutdown kit includes a set of cables that should be connected within the Safety Switch,
according to the Instructions supplied with the kit and available here.

Safety Switch Engineering Change
Solarfde will begin shipping inverters with Safety Switches that disconnect the IAC only. These switches
replace the AC/DC Safety Switches that have been supplied in the past, and remove the AC disconnection
redundancy at the AC breaker and at the Safety Switch.:
The installation process and the wiring of the Safety Switch remain unchanged. All previously provided
AC/DC switches and the new DC-only switches support the rapid shutdown kit.
Inverters supplied with DC Safety Switches will be shipping from July Esc 2014.

In the major portion of my career of forty years I have never seen such poor framing of a
residence. Some of the damage to the above noted residence occurred due to the overall
site preparation and addressment of the residence water drainage. Some of the concrete
pads have settled, some have twisted and some have heaved. All of which make a
mishmash of poor structural conditions and then to top this off entire joists were cut away
and oversize holes for plumbing were chopped into the joist. In some cases the joist have
no bearing at the ends of the joist at all.
Where the concrete pads in the crawl space have settled or twisted or heaved new
adjustable pipe columns were installed, shimmed and set in new alignment for structural
adequacy.
In cases where the joist were cut away to satisfy a plumbing or heat duct problem the
joist were properly cut away and headered off with micro lam material. The damaged
floor joist have been cut properly and received new joist hangers to a new micro lam
header. The header shall carry the loads. A new micro lam was sintered to a new full
length joist. This header was set in joist hangers at each end. There are some conditions
where the damaged joist can be strengthened by additional micro stock on each side of
the hole or damaged floor joist.
This work has been completed and was inspected by me and the work did meet with my
approval.

I did visit the above noted site on November 23. 2005. After having seen the training of
the residence it is hard to believe this type of framing could get past the inspection of the
city. There are so many destroyed floorjoist throughout the first floor that the structural
integrity of this whole floor is in jeopardy. As you can see by the enclosed pictures some
of the floorjoist have been cut completely in two before reaching a bearing wall or beam.
This is by virtue of the plumber installing the waste water lines. The heating contractor
was even better than the plumber as he would cut two or three joist in two before they
were headered off into other structural members that carry the loads to existing hearing
elements. They just dangle with no support.
Another structural company has suggested the tile and oak flooring be removed and new
floorjoist be installed. This is a costly and 1 feel an unnecessary procedure. We are
talking about a floor system over a crawl space. New small concrete pads, with columns
and headers can be installed in the crawl space without destroying the first floor finishes
or the foundation vents can be or temporarily removed and new micro lams or floor joist
slipped into the crawl space and carefully and properly he sistered to the damaged floor
joist members or used in other ways in this floor system to once again establish good
structural integrity. This procedure will be less costly and just as effective.

We were quite disappointed that your client was unwilling to agree to our proposal to buy
back her residence at fair market value. As we discussed, I proposed that idea because I thought
it would be quite beneficial to your client.
Cambridge Builders, LLC, hereby offers under C.R.S. § 13-20-803.5 to take the
following steps to resolve the issues raised by your client:
1. We will perform all work necessary to install helical piers and new concrete pads
for the pipe columns that were not centered on the original pads.
2. We will perform all work necessary to repair the TJIs that were cut by sintering
them with full span micro-lams.
3. We will install a helical pier under the rear patio where it supports the roof and
also mud jack the patio to return it to its original position.
4. We will perform cosmetic repairs as required.

Cambridge Builders, LLC, is currently unwilling to install helical piers under the front
patio because they were neither called for by the original design nor required now. Your expert,
Martin & Martin, concluded that the front porch settled at the northeast corner on the order of 1"
and has a hairline crack. However, they further concluded that the front patio "is not causing any
damage to the structure or problems which inhibits use of the patio." They also concluded that
no caisson was installed under the column on that porch. Finally, Martin & Martin further
opined that "the caissons are not required to structurally support the piers" at the front porch and
currently "no damaging movement of the piers was observed" there. Thus, your own expert
confirms that good engineering practice does not require such piers.
Similarly, Cambridge Builders, LLC is unwilling to install helical piers under the entire
foundation. We are aware of no justification for such work whatsoever. Indeed, your own
expert, Martin &Martin, concluded that the lack of any observed distress indicates the perimeter
foundation wall is not moving. At most, according to Church & Associates, there is a "risk" for
future movement. Under Colorado law, as you know, builders need only remedy actual damage
rather than "probable" damage.
To ensure that this offer is more than fair to your client, we are not demanding a release
in exchange for this proposed work. Moreover, we hereby offer to extend the tolling agreement
if your client agrees to let us perform this work. Such an extension will allow the parties to
confirm that no future problems occur with the foundation. Thus, your client may seek
additional damages at a later time.
Finally, we are willing to participate in mediation to resolve any disputes between the
parties. Please let me know who you would like to use to mediate these issues. We believe that
this proposed work should be scheduled immediately while the mediation is being scheduled.
This letter constitutes Cambridge Builders, LLC's efforts to resolve these issues under
the 2003 Construction ]Defect Reform Act. We have previously provided you with our findings
and results of our inspection of Ms. Brewer's residence. Cambridge will arrange to remedy all of
the issues set forth in this response within 90 days of Mrs. Brewer's agreement to move forward
with these repairs. We look forward to your prompt response.

Dear Ms. Tighe:
As requested, I performed a structural investigation of the residence located at 3909 Lee Circle in
Wheat Ridge, Colorado. The purpose of my visit was to evaluate structural movement and
construction defects at the residence. This report presents the findings and conclusions reached
as a result of my investigation.
Background: The residence is a single-story, wood framed structure with an attached two-car
garage. For the purpose of this report, the front of the residence is assumed to face east. The
roof is framed with pre-engineered wood trusses spaced at 24 inches. The floor is framed with
9-1/2" TJU150 wood I -joists spaced at 16 inches. The floor joists are supported on the perimeter
foundation walls and two interior steel beams running the length of the crawlspace. The steel
beams are supported on the exterior foundation walls and four interior adjustable pipe columns.
The perimeter foundation walls are supported on 16 inch wide footings. The footings are skip
voided at the front and rear walls and garage. The four interior pipe columns are supported on
spread footings.
I performed visual inspections at the residence on February 2, May 19, May 27 and June 1, 2004.
In addition, I reviewed the following documents:

Observations and Recommendations:
Foundation Movement: The following indicators of foundation movement were observed:
• A large drywall crack at the upper right (south) comer of the opening from the dining
room to the kitchen (Photos 1 and 2).
• A large drywall crack at the east wall of the living room and hallway (Photo 3).
• A drywall crack at the upper right corner of the door to the laundry room (Photo 4). The
door is slightly skewed and does not latch properly (Photo 5).
• A hairline crack at the opening from the hall to the kitchen (Photo 6).
• A crack in the ceiling of the breakfast nook (Photo 7).
• The pantry closet is racked (Photo 8).
• The kitchen countertop is 1/4" in 4'-0" out of level and is separating from the wall.
An elevation survey performed on the underside of the floor joists indicates that the foundation is
out of level by approximately 1-1/2 inches. The elevation measurements along with the noted
distress indicate that the four interior pad footings in the crawlspace are settling. It is unknown
at this time if movement of the perimeter foundation walls is occurring. Elevation measurements
indicate that the perimeter foundation wall is out of level as well; however, no associated distress
was observed. Large fissures exist in the soil in the crawlspace indicting that the soil in the
crawlspace had been very moist at one time and shrank as it dried out.
The geotechnical investigation performed by Church and Associates contains the following
statements:
In our opinion, it appears that the drying of the crawlspace soils may have caused
shrinkage in the clay soil, which is causing settlement of the interior pad footings.
The continuous perimeter footings may be more stable since relatively moist soil
conditions are present on the residence exterior and moisture conditions below
the footing are stable.
Given the present soil moisture conditions in the crawlspace, there is a risk for
future foundation movement. Continued settlement may occur if additional drying
occurs and the shrinkage conditions migrate toward the perimeter footings.

Additionally, there is a risk for foundation heave should moisture contents in the
crawlspace soil increase resulting in soil expansion. The heave from expansive
soils may be most severe at interior pad locations.
If crawlspace conditions remain unchanged and there is a potential for additional
drying and shrinkage or heave should unplanned moisture intrusion occur, there
will be continued potential for differential movement. A deep foundation system,
such as steel helical piers, would be recommended to repair foundations. Under
this scenario, it may be possible to use helical piers at interior column locations
and then monitor the perimeter elevations to determine it the drying of the soil is
causing foundation distress and settlement.
Recommendations: Support the four interior adjustable steel posts on new helical piers
installed to bedrock. Refer to the attached detail. The posts should be located at the
positions shown on the original structural drawings. Monitor the perimeter foundations
for a period of one year after the interior posts are underpinned. If movement of the
perimeter foundations is found to be occurring, the exterior foundation walls may require
underpinning as well.
Offset Posts: The adjustable steel posts supporting the floor structure are significantly offset
from the centerline of the pad footings in the crawlspace (Photos 9 through 12). One post is at
the very edge of the footing. The offset posts result in increased pressures under the footings and
are likely contributing to the previously noted settlement. The footings with the most severe
offsets exhibit out-of-levelness consistent with settlement of the more heavily loaded side of the
footing.
Recommendations: The offset condition of the posts will be corrected with the installation
of the new helical piers as previously described.
Cut Floor Joists: Four floor joists have been cut clear through for installation of plumbing and
mechanical. A fourth floor joists has been improperly notched. The notched joist and three of
the cut joists are located adjacent each other and are intended to support a bathtub, furnace and
hot water heater. The cut joists have deflected 1/2 to 1 inch.
Recommendations: The cut and notched floor joists are to be repaired. The method for
repairing the joists will depend on the location of mechanical and plumbing obstructions in
the area. Joists may be sistered with a 1-3/4" x 9-1/2" Micro-Lam if the one side of the
joists is unobstructed. The Micro-Lams are to be full span, bearing on the steel beam or
foundation wall at each end. Provide plywood web fillers at sistered joists. If obstructions
exist that prevent sintering, 1-3/4" x 9-1/2" Micro-Lams should be installed so that the
spacing between Micro-Lams and/or undamaged joists is 16 inches maximum. Provide
1-3/4" x 9-1/2" Micro-Lam headers with joist hangers around the mechanical duct opening.
The mechanical, plumbing and electrical in this area will have to be removed in order to
install the new framing.

Rear Patio Slab Settlement: The rear patio has settled approximately 2 inches causing the post at
the corner of the patio to separate from the roof structure. It appears that the post at the corner of
the patio is intended to support the roof trusses. The foundation plan indicates that a 10"
diameter by 10'-0" long concrete pier was to be installed under the post. The amount of
settlement exhibited by the post indicates that the pier was likely not installed and the post is
supported solely by the slab. The slab settlement is likely due to consolidation of inadequately
compacted backfill soil. Movement of the patio slab is also creating pressure on the wood trim
under the patio door. The trim is buckled and the patio door is skewed and difficult to operate.
Recommendations: The settlement of patio exceeds the criteria established in the
Residential Construction Performance Guidelines established by the National Association
of Homebuilders. The patio slab should be removed and replaced on properly excavated
and re-compacted SII. Alternately, the slab could be lifted with urethane or grout injection
with the understanding that future settlement may occur. The slab should be isolated from
the house and post to prevent any movement of the slab from affecting adjacent elements.
Soil at the northwest corner of the patio should be excavated to verify the existence of the
required caisson under the post. The builder should provide structural drawings including
truss plans indicating the bearing location of the trusses. If the post is intended to support
the trusses, the required foundation should be installed to support the post. A helical pier
installed to bedrock may be used as an alternative. If the post is architectural, rather than
structural, i.e. not intended to support the roof trusses, a pier would not be structurally
required under the post. It should be noted, however, that without the pier specified on the
original structural plans, the post is more likely to move and create cosmetic distress in the
future.
Front Patio: The original structural plans require 10" diameter x 10'-0" long piers to be installed
at each of the four masonry piers at the front patio and the column supporting the northeast
corner of the roof (Photo 20). The patio slab adjacent the northeast corner of the residence has
settled on the order of I inch (Photos 21 and 22). A hairline crack has developed in the slab due
to the settlement (Photo 23). Inspection and elevation measurements indicate that the column
and roof structure have settled by the same amount. The settlement of the structure indicates that
the required caisson was likely not installed under the column and the column bears solely on the
patio slab. No isolation pours were observed at the masonry piers indicating that the required
caissons were not likely installed at these locations either.
Recommendations: Soil adjacent the column at the northeast corner of the residence
should be excavated to verify the existence of the required caisson under the column. If the
required caisson is absent, one should be installed. A helical pier installed to bedrock may
be used as an alternative. The caisson and column should be isolated from the patio slab.
The settlement of the patio slab is approaching the Performance Guidelines established by
the National Association of Homebuilders. At this time, the settlement of the slab itself
(excluding the settlement's affect on the improperly supported column) is not causing any

damage to the structure or problems which inhibit use of the patio. It should be noted,
however, that settlement may continue in the future. If settlement continues, removal and
replacement of the slab on properly excavated and re-compacted soil may be required.
Soil adjacent the architectural masonry piers at the corners of the patio should be
excavated to determine if the caissons specified on the original structural plans were
installed. It should be noted that the caissons are not required to structurally support the
piers. Rather, the caissons are intended to minimize movement of the piers. At this time,
however, no damaging movement of the piers was observed.
Limitations: This report is based on conditions of structural elements that were readily
observable at the time of investigation. No invasive testing or inspections were performed.
Martin/Martin, Inc. does not accept responsibility for hidden structural deficiencies not evident
during an investigation of this type. Conditions observed on the date of investigation may
change if noted deficiencies are not corrected. Repair recommendations provided in this report
are conceptual in nature and are not intended for construction. Martin/Martin is able to provide
construction details and specifications for repair at your request.

To Whom It May Concern,
It has been brought to our attention there are structural concerns at the above referenced address.
Upon inspection of the above referenced address it was noted there are numerous TJI floor joist that
have been cut and damaged as a result of plumbing and mechanical installations. There is also a
problem with settling at the concrete porch on the west side due to the possibility of no supporting
caisson being installed per the approved foundation plan.
Due to these structural issues the city must require you to contact our office no later than
February 9, 2004 to schedule an inspection of the above referenced address. Upon completion of
this inspection a time frame will be determined for the required repairs.

The footings were constructed according to the foundationplos that were on the site
2,he footings had been properly stied and rvb*ced with two no. 5 bars, grade 60. All
of the bars wereproperiy kpped and tied The footings wareproperly bulk-headed Off at
all of the excavation step-downy. All of the interior pads were dso properly sized The
reinfa=ment for the pads was properly tied and sized accoAft to thefoundmion plans.
All cwwrete was placed on undisturbed natural solls, and all loose debris removed from
thefoodngforms prior to placement of concrete.

The foundation walls were reinforced horizontally with twa no. S bars, grade 60, top and

The forms were centered on the concrete footing foundation, and vertical dowels were
spaced at 2'0" to attach the footing to the new joiondatron walls- All of the reinforcement
was properly placed according to my recommendations.

Subject: Observation of Excavation for House at Lot 15, 3909 Lee Cr.,
Cambridge Park, Wheat Ridge, Colorado.
Dear Mr. Collins:
This letter will confirm our visual observations made on March 5, 1998. The
proposed house will have a crawl space.
At the time of our visit the excavation had been dug to a depth of ifa to 2 Ih
feet below the adjacent ground level. The soil exposed on the bottom of the
excavation consisted of stiff sandy clay. A small area of snow and wet soil was
present on the south side. These soils are similar to those encountered in our nearby
borings.
In our opinion the house may be founded on Type 3 spread footings designed
for a maximum soil pressure of 4,000 psf and a minimum dead'load pressure of 2,000
psf. The bearing soils below the footings must not be disturbed and all loose and wet
soils should be removed from the area below the footings prior to placing concrete. A
structural floor is recommended. A slab on grade floor may be utilized, realizing
there is a moderate to high risk of movement. The other recommendations in our
report 296 131, dated August 8, 1996 also should be observed.

I . This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This, permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in w; iting and
received prior to the date of expiration. An extension of no more than 180 days made -be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior andspecific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, anviolation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to fyeld inspection.
