Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A.'A-inch exists. Asphalt shingles are requirea to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Offial and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services

6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This letter will confirm our observations of Lot 75 made on October 3, 1997.
At the time of our visit the excavation had been Cut 6 to 7 feel below the
existing grade. The soils exposed consisted of 1 1/2 leer ol'sandy clay fill overlying
claystone bedrock. There was about 1 1/2 inches of water on the bolton3 of the
excavation on the west side and water was seeping into the excavation on the
southwest side about 4 1/2 feet above the boltom. Water appeared to be coming from
the adjacent irrigation.
In our opinion the house should be founded on a Type 5 foundation, consisting,
of piers. A structural floor is recommended. Slab-on grade floors may be utilized,
realizing there is a high risk of movement. The below grade walls should be
waterproofed with a membrane. Backfill should be free draining material connected
to the underdrain. The top two feet of backflll should consist ol'on-site clay to cap
the free draining material. The other recommendations in our report 296 131, dated
August 30, 1996 should also be observed.

At your request, on October 9th and 22n~ 1997, pier drilling and foundation wall
inspections were performed on the single-family residence located at 4025 Lee Circle,
Wheat Ridge, Colorado. The pertinent information regarding the inspections is as
follows:
PIER DRILLING INSPECTION- OCTOBER 9 1997
Soil Pro file
The soil proftle encountered during drilling consisted of 0' to 3'-0" of fill auul 3' to IV-
0 " of claystone bedrock in the upper level, and 0' to 12'-0" of claystone and 12' to 18'-
0" of blue shale bedrock in the lower level. No water was found in the holes at the time
of drilling. Soils report requirements were met at all pier locations inspected
Reinforcement Concrete
All of the piers were reinforced with three no. 5 bars, grade 60, for their entire length.
The concrete used consisted of a 3, 000 psi, 314 " stone aggregate mix. All of the piers
were properly constructed according to our recommendations and the recommendations
in the soils report.

FOUNDATION WALL STEEL INSPECTIONS- OCTOBER 22 1997
The foundation walls were reinforced horizontally with two no. 5 bars, grade 60, at the
top and bottom, and one no. 5 bar, grade 60, at center. All of the bars were properly
lapped and tied The foundation wall forms were properly centered over the drilled pier
foundation system, and the reinforcing steel from the piers extended to the top row of
reinforcing in the foundation wall. Paper void material was properly installed between
the pier locations in the bottom of the forms. Reinforcing steel was properly placed at
the foundation wall step-downs and window openings. Additional vertical bars were
placed in all walls taller than 8 '-0

Dear Mr. Hobbs:
As requested the undersigned visited the subject house on January 28, 1998.
The purpose of the visit was to observe the seepage conditions and the installation of
an additional underdrain. For the purpose of discussion, it is assumed, the house faces
east.
At the time of my visit, trenches were being dug parallel to the west wall and
an additional sump was being dug in the northwest corner. The house is framed and
has a structural basement floor. The ground surface below the floor is about 9 1h feet
below the outside grade. The ground surface was very muddy and wet with standing
water in several places. The trenches were about 16 inches deep and the sump was
about 3 feet below the bottom of the grade beam. Workers were still cleaning the
drain trenches and sump. Because of the wet conditions, it was not possible to
observe how effective the new drains will be in intercepting seepage, but they should
be. We should make additional observations after conditions improve.
