Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Al/z-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€lms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Dear Ms. Shuster,
This letter is to follow up a fax received March 19, 2002 from Co solidated Mutual
Water regarding water service tap and service line size for 48 Hillside rive.
The fax states that Consolidated Mutual will not guarantee pressure or flow with regards
to the existing water service to the property. As part of your additi n you are adding
additional fixtures and bathroom facilities, which total 12.5 fixture uni s per the Uniform
Plumbing Code, 1997 Edition.
Pursuant to Tables 6-4, 6-5 of the 1997 Uniform Plumbing Code the s cture has a total
fixture count of 32 with regards to sizing water taps and service line. Because of the
number of total fixture units proposed the water service requirements e as follows, the
0/." water tap and meter can remain but the service lateral from the mete to the house will
be required to be increased to not less than a I-inch line. Please m e the appropriate
corrections or changes prior to requesting a final plumbing or building i spection.

1.0 EXECUTIVE SUMMARY
1.1 Subsurface Couditious
The subsurface conditions at the site are rather consistent. The upper six (6) . ches was topsoil
that supported grass. From six (6) inches to about eight (8) feet consisted of br wn clayey sandy
silt. At about eight (8) feet to fifteen (15) feet brown claystone bedrock was fo d, From fifteen
(IS) feet until the end depth of twenty-five (25) feet was Dawson sandstone.
1.2 Foundations
In our opinion, the most feasible foundation systems for your residence are as foil ws:
a) For shallow footings: spread-footing foundations of twenty-four (24) inches 'de placed below
frost depth (thirty-eight (38) inches) on natural soil. This foundation system, our OpinIOn, is
probably the most suitable foundation alternative for light to moderate loads where structural
movement is not critical. A maximum allowable design bearing pressure of 2, 00 psf should be
used.
b) For deeper footings: spread footing of twenty-four (24) inches wide pia below basement
level (eight (8) feet), on natural soils or rock. This foundation system, in our 0 inion is probably

2.0 SCOPE OF WORK
ATEST, Inc. was contacted by Ms. Schuster about January 15, 2001 fo the purpose of
performing a Soil and Foundation Investigation for a residence at 48 Hillside Drive in eastern
Jefferson County. This is located in the central portion of the northeast Y. 0 the southeast Y.
Section 28, Township 3 South, Range 69 West. This report presents results of the
investigation at the site of the proposed residence.
2.1 Scope of Soil and Foundation Investigation
The purpose of this study was to explore the subsurface conditions, obtain orne data of the
pertinent engineering characteristics of the underlying strata, recommend the most appropriate
foundation system, develop specific foundation design criteria, attempt to eva uate the risks of
slab-on-grade construction, and address other geotechnical factors in the propos development.
It should be understood that economic and practical constraints limit our sampr g and laboratory
testing to only a very small fraction of the total mass of soil and/or bedrock t lies within the
zone of influence of the proposed structure.
Our analyses, conclusions and recommendations are based upon the assumptio that samples of
the subsurface strata, which we observed and tested, are representative of the enf e soil mass.
2.2 Proposed Construction
As we understand, the proposed construction on the site will consist of a two (2 story addition to
an existing singll}-family residence. This will be a wood-framed, brick addition. The addition will
be about twelvl}-hundred (1,200) square feet to an existing fourteen-hun -ninety (1,490)
square foot residence. Utilities will generally be underground with storm being s ace runoff.

3.0 INVESTIGATION
3.1 Field Survey
The field exploration portion of this project took place on March 30, 2001. On (1) exploratory
boring was drilled at the site. The boring was advanced to about twenty-five (25 feet to evaluate
soil conditions for the proposed residence.
At regular intervals, the drilling tools were removed from the bore holes

3.3 Laboratory Testing of Snbgrade Soils
Samples were inspected and classified in the laboratory by the project engin . The laboratory
testing generally confirmed the medium consistency of the natural overburden soils. The plastic
indices ranged from nine (9) to twenty and a half (20.5) feet had approxim ely twenty-seven
(27) to thirty-one (31) percent passing a -200 sieve, and contained between fift (15) to sixteen
(16) percent moisture. No swell/consolidation was performed on the sampl Sulfate results

observations tend to support this report. This water level may be seasonally
higher in wetter months, or due to an increase in lawn irrigation. The slope of e ground towards
this residences backyard encourages the flow of surface water towards this res dence. This would
encourage the use of both a perimeter drain and sump system and an intercept r trench. Standing
water may be present below or above ground. Drainage at the toe of the slope s ould be present.

5.0 RECOMMENDATIONS
5.1 Design Considerations
5.1.1 Foundations (Spread Footings)
Foundation footings should be reinforced with 2 each # 5 grade 60 rebar. This sh uld be placed a
minimum three inches from the bottom of the footing. This is to give them sufficient beam
strength to span isolated loose pockets of soil, which may occur in the strata low foundation
levels.

where structural movement is not critical. A maximum allowable design bearing
pressure of 3,000 psf should be used.
5.1.2 Foundation WaIls
Soil loading conditions present horizontal loads on walls. The at-rest lateral load should be about
45.0 psf per foot of depth. Passive lateral loads sbould be about 250.0 psf per r. of depth in the
overlying soils.

5.1.3 Floors (Slab-on-grade)
It is recommended that a slab-on-grade floor system be used in conjunction with the spread
footings. Slabs should have joints spaced at no more than one hundred (100) uare foot panels
(about 10 foot by 10 foot).
5.1.4 Drainage
A. Surface Drainage
Adequate drainage provision should be made so as to minimize water flowin to the subgrade
soils beneath the beneath the buildings. The life of any structure is grea y diminished by
improper drainage. Backfill around the buildings should be moistened and well compacted
during construction. The fmal ground surface should be given a good slo away from the
building. A minimum slope of six (6) inches in the fIrst fIVe (5) feet (10% slope) is
recommended. Paved surfaces may be sloped a minimum of three and a half ( .5) inches in the
frrst ten (10) feet (3% slope).
A swale should be provided to allow positive drainage away from the back ard towards the
drainage ditch in the front yard. Roof downspouts should discharge onto spla h blocks. Down
spouts and/or pavements should extend to beyond the limit offoundation backfill.
B. Subsurface Draiuage
The phreatic surface lies many feet below the bottom of the footings. Seaso I fluctuations in
precipitation or lawn irrigation apparently have resulted in a temporary perch

months for snow accumulation. The silty clayey soil may occasionally make co struction difficult
because the soil is somewhat moisture sensitive.
An interceptor drain at a minimum of two and a half (2.5) feet below surface sloped to daylight
towards a swale in front of the home should be installed. This could be in rated into the
swale recommended above. The drain should be a minimum four (4) inc
covered with a geotextile "sock." It should slope a minimum one-quarter ('A) inch per foot (2.5
percent slope) and not be steeper than two (2) horizontal to one (I) vertical. e trench should be
backfilled with coarse sand. This drain could be located near the toe of the slop of the back yard.
A rodent guard should cover the end.

The need for permanent de-watering is highly encouraged. Sprayed on water p oofmg should be
used as a minimum, with consideration given to using an impermeable mem rane. This drain
should be sloped to a sump perhaps at the southwest portion of the addition. S opes in the drain
should be a minimum one-quarter (Y<) inch per foot (2.5 percent slope) and no steeper than two
(2) horizontal to one (I) vertical. A small size pump should be consider however, if the
seasollal flow rates are higher than anticipated this pump should be reviewed for dequacy.
5.1.5 Utilities
Utility backfill should be compacted as appropriate for the proposed surface uses (landscape,
building, pavement, etc.). Cobbles (3 inches plus) and boulders should not be resent in bedding
fill within one (1) foot of the utility lines. Cobbles up to eight inches may be sed in the utility
backfill outside this one-foot bedding. Cobbles over eight inches should not allowed in any
utility backfill. On-site materials, with organic materials removed, are suitable ackfill if properly
screened/processed to meet gradation requirements. Backfill around electric lin should be clean
sand material.
5.2 Construction Operations
5.2.1 Foundations
A geotechnical engineer should inspect soils exposed in the bottom of the:li ting excavations.
Pockets of loose, soft, or other undesirable material should be replaced with p operly compacted
structural fill approved by the engineer.
The natural soils exposed in the bottom of footing excavations may need to be compacted by
hand operated equipment to help consolidate soils distributed during co ction. These soils
should be compacted to ninety-five (95) percent maximum Standard Proctor
698). Any structural backfill used on-site or over natural soils should als be compacted to
ninety-five (95) percent Standard Proctor density.
Any excessively clayey material or pockets encountered during excavation for footings should be
removed and replaced with suitable material.
5.2.2 Earthwork
We recommend that cut and fill slopes over four (4) feet in height be no st than two (2)
horizontal to One (1) vertical. Any excavation must comply with OSHA irements (29 CFR
part 1926).

Structural fill should consist of either inherently non swelling granular material, t an increased
risk, material which can be placed and maintained in such manner that its s II potential is
minimized. Any imported material should be approved prior to its use. On site sa dy material is
acceptable. Structural fill should compacted to at least ninety-five (95) percent 0 the maximum
Standard Proctor density (per ASTM D-698) at moisture content appropriate fo the particular
material. We would expect that on-site clays would require a moisture cont t at or some
percentage above the optimum (0 to + 4% OMC). The necessary moisture
imported material would be determined at the time of approval. The mo.
essentially granular material such as sand would not be critical.
5.2.3 Utilities
Caving of the natural sandy soils could occur during construction. Shoring, and! r trench-boxes
might not be necessary. Compliance with 29 CFR 1926 is required.

6.0 CLOSURE
The geotechnical findings, recommendations, professional judgment, and c . eria on design
alternatives provided herein were developed from the information obtained from the test borings
that reflect the subsurface conditions only at the specific locations and at th particular time
designated on the test boring. Soil and groundwater conditions may differ from conditions
occurring at the test boring locations, The nature and extent of any variation' the test borings
may not become evident until during the course of construction. If variations appear, it may
become necessary to reevaluate the recommendations of this report after observing the variation.

At least four completed documents must be provided to apply for a building permit:
1. Complete this Building Guide by filling in the blanks on p ge two and indicating
which construction details will be used,
2. Provide a Separate Site Plan showing dimensions of yo r project or addition
and its relationship to existing buildings or structures on the roperty. Show all
easements and the distance to existing property lines.
3. Provide a Aoorplan. See example on page 4,
4. Fill out a Building Permit Applicadon.

processed with little delay. The submitted
documents will help determine if the
project is in compliance with building
safety codes, zoning ordinances and other
applicable laws.

This permit shall expire If (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the b ilding authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no ch nges have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
No work of any manner shall be done that will change the natural flow of water causin~ a drainage problem.
