In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

CONTRACT PER COLORADO SENATE BILL 12-038: According to SB -038 -The Consumer Protection/Roofinr Bill passed Wednesday. 6/6/12, it is illegal to waive, pay or rebate any deductibles This is a binding
contract/agreement when signed by property owner, agent, or insured of property for completion of work listed on this contract. It is the responsibility of the property owner to receive approvals required or acquire information from
their HOA, Property Owners or Associations involved with the property. Frontier Restoration will ONLY complete the work as outlined above or on a Supplement Contract signed by property owner and approved by Frontier
Restoration Management. Frontier Restoration will acquire required permits or the homeowner may acquire the permits if they choose. Frontier will supplement your insurance company for the cost or the property owner may do
so. ALL SUPPLEMENTS, PERMIT COSTS, OVERHEAD & PROFIT AND ANY OTHER ITEMS INVOICED TO YOUR INSURANCE COMPANY BY FRONTIER RESTORATION that are not shown above or on your
insurance summary or contract, will still be owed to Frontier Restoration upon completion of work and payment by the insurance company. Payment in full is due upon completion of each trade. A 3.5% portal fee is applied to

It is the property owner's responsibility to remove any breakable items that they feel could be damaged or broken by the construction work to be performed by Frontier. Interior and Exterior: we are not responsible for broken items
thaf'are not moved away from the construction site by at least 15 feet. It is the property owners responsibility to remove all hanging items from walls and ceilings (paintings, light fixtures, nic nacs, or any other items that could fall
or break due to the heavy pounding that occurs during construction. If not removed or moved and breakage occurs it will not be the responsibility of Frontier Restoration for replacement or correction for these items or damages.
Frontier makes every effort not to damage vegetation; however, it is a construction site and there is the possibility of damaged, vegetation; thus, vegetation damage(s) will not be replaced or paid by reimbursement to the property
owner. Frontier is not responsible for damaged or deteriorated wood during the construction period or thereafter. Property Owner is responsible for notifying, through writing, of any areas without allowable space for fasteners
pursuant to manufactured roofing specs. Company is not responsible for any interior damage or nails that may show through inside the building. Please see front of contract. PAYMENT IS DUE NO LATER THAN 10 DAYS OF
TRADE COMPLETION. All estimated start and completion work dates are determined by weather, availability of material and work crews. THERE IS NO WARRANTY UNTIL ALL MONIES HAVE BEEN PAID IN FULL

ADDITIONAL PROVISIONS: It is unlawful to knowingly provide false, incomplete or misleading facts or information to an insurance company for the purpose of defrauding or attempting to defraud the company.
Penalties may include imprisonment, fines, denial of insurance and civil damages. tfFrontier Restoration is working with insurance proceeds, we will NOT provide any false information for the purpose of defrauding the
insurance company. Any alterations, modifications, changes or additions must be in writing and agreed to between the parties as evidenced by their signatures. Frontier Restoration is not responsible for damage caused by improper
original construction that is not limited to faulty framing, masonry, drywall cracks, bracing and/or roof decks or any type of damage. We are not responsible for decayed, rusty or any type of damaged siding, paint and sheetrock
damage prior to construction, or damages due to vibration when work is being performed. We are not responsible for damaged electrical cable, A/C, or plumbing lines that are installed within 6 inches of the decking. Frontier
Restoration is not responsible for resetting any satellite dishes for homeowner due to repairs on property or building; this is the property owner's responsibility. Frontier Restoration can send a reliable company to reset your satellite
at a cost to the property owner unless otherwise agreed to by both parties that Frontier Restoration will make payment. It is the property owner's responsibility to notify Frontier Restoration of any deed restrictions. If the
undersigned fails to pay Frontier Restoration any amounts due the undersigned agrees to pay all costs involved in the collection of this debt, including fees for property lien costs, mechanic lien costs, and any and all costs
associated with collections such as collection agency fees, sheriffs fees, subpoena fees, court costs, plus attorney fees along with finance, charges at a rate of one and one-half percent (1/1.5%) per month, 18% PER ANNUM or the

4. CANCELLATION POLICY: The signer on this contract has the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the contract; however, if the property owner wants material delivered
or work to begin on the property before the legal 72 hours right -to -rescind, any work that has been started on job address of signed contract for work will be paid m Frontier Restoration at our cmmtractee casts and any and all
materials will be paid for in fall that have been delivered or pd to use on said property and Frontier Restoration will have the right to keep any deposits and invoice for 10% of the contract price. If payment is not made within 10 days
after week has been performed and material has been delivered, Frontier Restoration has the legal right to place a lien on this property and take all necessary action to collect monies owed.
5. Frontier Restoration is entitled to compensation for any and all custom or non -returnable materials that may have been ordered prior to any contract rescission. It is the property owner's responsibility for full payment to Frontier Restoration for any
and all of these custom and non -returnable materials. if payment is not made within 10 days of cancellation Frontier Restoration has the legal right to place a lien on this property and take all necessary action to collect monies owed.
& Frontier Restoration Unna be responsible for internal stmauml strength. If you have any concerns or doubts about the strength of your roof substructure to include rafters, beans, etc. and other roof materials, it will be the property owner's
responsibility to either notify us, obtain an engineer's report, and have weakened conditions repaired before roofreplacemen t or repairs are warted. Frontier Restoration can make these repairs at an additional cost.
7. The company shall have no responsibility for damages from Ore, windstorm, other hazards, animal destruction, m Mother Nature as this is normally covered by the property owner's insurance.
8. If you have exposed decking inside your building, i.e.: cathedral ceilings, vaulted ceilings, tongue & groove ceilings, low-pitched roof, open soffits or any other type ceiling or areas with no allowable space for manufactured roofing specs for
fasteners, you should be certain that we know so we can use proper nail length. If we are net told and this portion of the contract is not initialed or we do not this receive information in writing, we are not responsible for any interior damage or nails
that may show through inside the building.
9. Replacement of deteriorated decking, fascia board, rcefjacks, ventilators, flashing or other materials, unless otherwise stated in this contract, are net included and will be charged as m extra or supplement on a time and material basis. This will be
supplemented to your insurance company or will be invoiced to propeny owner at time material is bought and placed on building.
10. I/We acknowledge Frontier Restoration is not responsible for any damages caused to us or to our property as a result of mold andlor mildew resulting from water penetration that may devc1 p as a result of the work per( ed by Frontier Restoration
unless proven to be negligent within a ane -year period

it. Frontier Restoration is not responsible for any type of pollution of any kind that may cause health hazards such as allergies, headaches or any other health issue of any kind to members of the household,
visitors, hired contractors, or any other person who tray be present at the time of construction or after construction because dust and pollution is unpreventable.
12. Frontier Restoration is not responsible or liable for any type (solid, environmental, liquid, or any other form of pollutant) of release or escape of pollution or pollutants that may arise or be released from the
construction performed or required by Frontier Restoration.
13. No painting, gutters, downspouts, windows, siding or any other type of work is included with this contract unless otherwise stated on front of contract.
14. The company shall not be liable for failure of performances due to labor disputes, strikes, file, weather, inability to obtain materials from usual sources or any other circumstances beyond the control of the
company whether of a similar nature.
15. If any provision of this agreement should be held to be invalid or unenforceable, the validity, and the enforceability of the restraining provisions of this agreement shall not be affected thereby.
16. Any representatives, statements or other types of communications (such as email, phone conversations, or ANY type of communication), not written in this contract are agreed to be immaterial and not to be
relied on by either party, and do not survive the execution of this contract. Frontier Restoration is NOT responsible for any type of promise, written or verbal, if not in writing and signed by property owner
and Frontier Restoration management.
17. Additional materials are generally ordered for each job. Any material delivered to job site remains the property of Frontier Restoration, this includes left -over material.
18. Frontier Restoration is not responsible for damage to any landscaping materials, brushes, flowers, trees, stone of any kind, pottery, or other outside accessories or furniture and other type of items. It is the
property owner's responsibility to move or remove from construction site by no less than than 15' all breakable items, including furniture. Property owner or resident must cover landscaping and vegetation
while construction is being performed.
19. It is the property owners responsibility for expenses incurred to bring current construction to code per city/county building department specifications if not covered by property owner's insurance policy.
20. Frontier Restoration is not responsible for any (1) faulty or deficiency in or of design, planning, supervision, inspection, construction, or observation of the construction of any improvement to real property (2)
injury to real or personal property caused by any such faulty or deficiency. Any claims for any type of monetary relief or construction relief by the property owner, tenant, predecessor or any other person(s)
21. It is the responsibility of the property owner to maintain the roof, gutters and downspouts from any or all debris that may cause leakage or any other type of damage. Frontier Restoration will invoice the
property owner, tenant or any other person who requests the presence of Frontier Restoration for inspection to the cause of the leakage, fast running water, drainage or any other type of inspection if the
inspection requested is not the fault of Frontier Restoration construction, a minimum charge of $150.00 will be charged for this inspection... if Frontier Restoration is required to perform a "fix" to the result of
this request, property owner, tenant or other such person will be billed accordingly at no less than $95.00 per hour in addition to the inspection fee of $150.00. This must be paid at time of repair and
inspection.
22. NOTE: Shingles may not lay flat when temperatures are below 70 degrees but as soon as the temperature warms, shingles will lay flat.
23. Frontier Restoration is not responsible for any rodents, pests, or any other type of animal damage to work completed by Frontier Restoration.
24. It is the responsibility of the property owner or resident of said property to manage children and adults from playing or gathering around the construction site as flying or falling objects could cause injury.
25. This agreement constitutes the entire agreement between the parties. This agreement may be changed only by written instrument signed by both parties.
26. All these conditions are a part of any contract entered into or authorized to precede, the same as if they were included therein.
27. In the event that any dispute arises under this contract, or there is a problem with the work completed by us, property owner agrees to allow Frontier Restoration an opportunity to cure such defects. In the
event that Frontier Restoration is not allowed an opportunity to cure such defmt(s) then Frontier Restoration is not liable for cost of repair or damages resulting from such defects. In any event, Frontier
Restoration shall not be liable for any consequential damages. Frontier Restoration must be notified within 15 days of work completion of any questions or disputes; otherwise Frontier Restoration will not be
held liable for any portion of any work that the building or property owner finds unsatisfactory.
28. Start and completion time are determined by material delivery, weather conditions and subcontractor work schedules.
29. All contracts are subject to approval by the Management of Frontier Restoration. Frontier Restoration has the right to cancel all contracts at any time.
GENERAL ROOF MAINTENANCE REQUIRED BY THE BUILDING OWNER
The following conditions are unrelated to defects in the roof and therefore are not covered by the warranty.
a. Damage caused by sun, snow, ice, hail, fallen tree branches, other fallen articles, collection of leaves, cones or needles, fire, lightning, windstorm, floods, earthquakes or aha phenomenon of the elements of nature, eta( notify, your insurance
company immediately).
h. Structural settlement, failure, movement, cracking or excess defection of the roof deck; defects or failure of materials used as a roof substrate, aver which the roof system is applied; faulty condition of parapet walls, copings, chimneys, skylights,
vents, supports or other parts of the building, vapor condensation beneath the roof; penetration for pitch boxes, erosion, cracking and porosity of memo and brick; dry rat; stoppage of roof drains and graters penetration of the roof from termites or
any other insects, any type of rodents or any other animals; harmful chemicals, oils, acids and the like that come in contact with the roofing system and cause a leak or otherwise damage the roof system. If the roof fails to maintain a water -tight
condition because ofdamage by reason of any of the foregoing, this warranty shall immediately become null and void for the balance of its term unless such damage required is repaired by Contractor at the expense of the (honer.
c. Accumulation of leaves or pine needles in guttas or roof valley produce what is known ns a "water dam". A water dam can face water under the roofing and produce leaks through the roof structure. These leaks can travel along roofjoints, etc., and
produce interim damage at points far distant from the leak; therefore, provisions should be made for the removal of this debris. Water dams and ice dams are the responsibility of the property owner's maintenance to prevent damage, this pertains to

d. Adequate ventilation will kap attic tempemtme close to those outside, helping to prevent the deck delamination at the roof and the formation of ice dams in guttas and valleys. Adequate ventilation is essential to the life of Asphalt Shingles.
Adequate ventilation (defined by FNA standards) consist of one square fad or more of open arca for every 300 square feet ot attic floor area 50% at or near the peak of the roof. 50% in the eaves or lower pan of the roof. Lack of ventilation for
inadequate ventilation) will result in moisture condensation and can cause telling of the wood deck and/or blistering or curling of asphalt shingles which is net a defect in the shingle but is a direct result of moisture being pulled through the shingle by
the sun.
e. Installation or relocation of -antennas or other equipment on the roof may cause leaks unless proper sealants are used at any point where screws or other anchming devices enter the roof. Avoid unnecessary roof traffic, since it cart greatly diminish the

Regulations and Building Code of Wheat Rid~e, Colorado or any other applicable ordinances of the City.
This permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building authorized is suspended or
abandoned fOf a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing with successive phases of the job.
The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of, any violation of the provisions
of the building codes or any other ordinance, law, rule or regulation.
