approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€lms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

to drainage. Where abutting the foundation, the hardscape should be well sealed to prevent moisture
penetration. It is not recommended to attach hardscapes to the foundation via rebar pinning.
• Construct or improve landscape swales, as needed, to route the drainage around the foundation. The
flowlines of the swales should have a slope of at least 2% (1/4" per foot). In areas where 2% cannot be
attained, a minimum slope of 1% is acceptable if a French drain is incorporated in the flowline.
• Swale flowlines should be located no closer than 5 feet from the foundation walls. The swales should be
continued to a points where natural drainage is possible.
• Maintain/repair/replace roof gutters and downspouts, as needed, to prevent leakage and/or over spilling
around the foundation. Install and/or repair/ maintain downspout extenders, as needed, to discharge a
minimum distance of 5 feet from the foundation. Determine the discharge locations of any buried
downspout pipes and test with a hose to ensure unimpeded flow and visible (daylight) discharge locations.
If the discharge locations are not evident, and/or if blockage exists, the pipes may need to be extended or
replaced.
• Determine the locations of any and all active sprinkler heads, and move as needed to a distance of 5 feet
from the foundation as a minimum, and orient so that spray does not fall within 5 feet of the foundation.
Drip irrigation and/or "misting" heads are generally considered to be acceptable as long as they are used
sparingly. Maintain or create a 5 -foot -wide non -irrigated landscape buffer around the foundation. Replace
or modify any solid landscape edging to allow drainage to pass thru the edging. Remove any plastic weed
barriers adjacent to the foundation (within 5 feet). If a weed barrier is desired, use a geotextile mesh
landscape fabric that allows the soil to evaporate its excess moisture.
Note: All of the above recommendations or practices are to help prevent further issues to your structure. However, even
with these preventive measures none of them are deemed fail safe and the residence in question may experience issues
beyond anyone's control.

Structural Repair Notes for Homeowner:
-Foundation Stabilization: It should be understood that underpinning/stabilization of part of the foundation will minimize
future movement in the area stabilized, but will not necessarily prevent movement elsewhere. Movement elsewhere (if it
occurs) may in fact become more apparent once part of the foundation is stabilized, as cracking may develop at the
transition points between the stabilized area and the non -stabilized area.
-Foundation Adjustments: Some incremental adjustments (lifting/lowering/realignment) of the affected area may be
attempted with the owner's approval. It must be understood that adjust ments could cause additional damage to finished
surfaces and foundation walls. The owner is encouraged to be present at the time of adjustments in order to authorize the
amount of adjustment to be attempted. StrucSure Engineering, Inc. shall not be responsible for adjustment authorization
or for any damages caused by attempted adjustments.
Good Drainage Practice (for the homeownerl:
• Providing and/or maintaining good drainage is recommended in Colorado, in order to promote
stabilization of moisture content in the foundation bearing soil. Stability of moisture conditions at
foundation depth is considered essential toward promoting long term structural integrity of all homes.
• These drainage remarks are general in nature to all homes in the area, and not necessarily site specific, as
each site has unique constraints. In some cases, consultation with a land surveyor may be necessary in order
to determine a detailed scope of drainage improvement, based on site constraints. Provide/maintain
positive drainage away from the foundation
• Building Codes typically require a minimum of 2% slope (1/4" per foot) and should be considered an
absolute minimum. Ideally, 6" of fall in the first 5' is recommended by most soils engineers in the Colorado
Front Range area with expansive soil conditions, for landscape areas, and then 2% (1/4" per foot
thereafter)
• A minimum of 6" must be maintained from top of foundation wall to top of finished grade to prevent
moisture penetration into the structure. If clearance cannot be maintained, then cutting the soil down and
away from the foundation may be necessary in lieu of adding soil. In this case landscape drainage swales
and/or french drains may need to be created in order to convey the drainage in a continuous manner. I
some cases, addition of sidewalk "skirts" or hadscape patios may be an alternative. city
• Hardscape areas such as driveways, patios, sidewalks, etc. should slope away from the foundation, a o
minimum of 1% slope (1/8" per foot) and should be installed at an elevation that does not act as a aAke-

Mr. Schmidt:
As requested by Pinnacle Structural Services, StrucSure Engineering, Inc. conducted an observation at the above referenced
property on November 9, 2016. Following are conclusions and recommendations by StrucSure Engineering.
General Information:
The residence is a single -story Single Family residential home; east facing.
• The residence is a vinyl siding exterior wall with assumed wood frame wall construction on the interior.
• The residence is roughly 22.1 feet wide and 26.7 feet deep with a 22.1 ft wide by 11.1 foot -deep addition at the
rear (west) of the residence.
• The main floor is 2x construction supported by CivfU foundation and interior bearing.
• No soils, engineering or previous information about the house was provided.
General Observations:
• The north and south side (rear) foundation walls have experienced inward lateral displacement and cracking.
• The cracking noted was horizontal, vertical and diagonal in nature and ranged from hairline to 1/4" in width
with most cracks being around 1/8" in width.
• The homeowner had provided temporary shoring along the south wall to try to stabilize the wall.
• A previous repair had been conducted on the north wall with metal studs bracing the wall and the foundation
wall having epoxy/grout at a date prior to the current homeowner purchasing the residence.
• The topography of the property has positive drainage at the south wall but negs age towards the
residence at the north wall.
• There is i0qi& south of the residence that will✓.b ' `wed to allow for excavation and a new
egress window framed by the homeowner.
• The rear foundation wall at the addition had two vertical cracks noted that the homeowner has made the
decision not to have addressed at this time.
Conclusions:
It appears the foundation walls have seen hydrostatic or inward lateral earth pressure exceeding the foundation capacity to
withstand these forces. Many older houses were not designed to resist the moment created from long foundation walls with
insufficient bracing or reinforcing.
Recommendations:
To reinforce the foundation and to help prevent possible future wall displacement, StrucSure Engineering recommends the
following repair:
• The contractor must ensure all utilities are properly marked and all work is done in compliance with local and
national municipalities safety and construction standards prior to commencement of work.
• Excavate the south wall prior to placement of the wall anchors relieve lateral pressure and help return the south wall
to plumb as much as possible.
• Proh,,he north side foundation wall and long the south side foundation wall per the
attached R1 & R2 drawings.
• Apply a high strength, non -shrink epoxy in all foundation wall cracks after the completion of wall anchors
tensioning has occurred.
