Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Due to the unique nature of repairs related to insurance claims, this contract may not reflect the final price. Multiple factors contribute to the
final price including but not limited to; missed items, discovery, and market conditions. Guardian will notify the carrier of any differences in
price and pursue proper restoration of your project. Reaching an agreement on the full scope of repairs involves considerable time on Guardian's
part; we will not proceed with this phase unless you agree to allow us to perform the work once the scope is agreed upon. By signing this
agreement, you authorize Guardian to reach an agreement on price and scope of repairs on your behalf. Guardian agrees to bid the work using
the primary insurance industry pricing database (Xactimate) based on the scope of work agreed upon with your insurer, including general
contractor markup at customary insurance rates (20% markup on Xactimate line items). Any substantial deductions to the scope of work will
be handled by written change orders.
If the insurer indicates that any line item of the work is not covered at full replacement cost (RCV), the client will be responsible for the difference
between the replacement cost and insurance coverage. To the extent we are informed of those items in a timely manner, we will inform you, and
you can decide whether you want to proceed with the work and pay the portion of the costs not covered by your insurer.
Joint Checks
You agree to have Guardian Exteriors, along with you, on all payments from the insurer that are related to Guardian's work and that are issued
after this contract is signed. If a portion of the insurer's payment is held by a mortgage company, you further agree to have Guardian Exteriors
named, along with you, on all payments from the mortgage company related to Guardian's work. By using joint checks on the payments, neither
party can cash the joint checks without the other party's consent, which protects both parties.

Please reference the attached field r.eport.
The house is a ranch circa 1955. The exterior walls are masonry.
The foundation is poured in place concrete on footings.
There is a concern about settlement of the floor in the kitchen
and bath. The floor has settled in this area due to rotten
flooring. The flooring is rotten because the toilet and washing
machine have leaked. Some of the flooring has been replaced but
only abut 1/3 of the area. A great deal of shoring has been placed
in this area in the crawl space. Most of this shoring is not
necessary. One floor jol.st has been cut for the heating duct
rendering it inadequate. The other joists are 2x8 at 16"OC
spanning 13'-6". This span is at the limit for these joists from a
deflection standpoint. The shoring may have been added because the
floor was bouncy. The existing shoring does not meet code.
Por repair the rotten floor should be replaced. The wllshing
machine should be placed in a tub with a floor drain plumbed into
the exi5ting drain. The cut floer joist should be sistered with 2-
2x8' s. AnothElr option is to shore it with two adjustable pipe
columns on 2'x2'x8" concrete pads. (Reinforce with 3-15 each way
bottom.) Install 2-2x6 cross beams to pIck up three floor joists
each side of the cut. space about 6 feet apart.
~The existing shoring should be removed. If the owner wishes to add
sho:ring copy the eXisting center steel beam. Use the same pad
size. The columns should be 3ft diameter adjustable steel tubing.
