The inspection failed, with deficiencies noted in the nailing of the starter course and over-penetrating nails in the shingles. It was noted that re-inspection was required, but work was allowed to continue based on correction and re-inspection of the deficiencies. That required re-inspection was not requested, nor were the deficiencies corrected. The work that continued after the midroof inspection has been found to contain the same deficiencies noted during that inspection. On January 30' 2009, a re-inspection of the roof was conducted, with WR2020 representative Tom Brace present. The existence of the deficiencies was verified to Mr. Brace. In its' current state, the roof covering is not compliant with adopted building codes or manufacturers' installation guidelines and cannot be approved by the Building Division. Additionally, because of the lapse in time between the midroof inspection and the inspection with Mr. Brace, it is believed that correction of the deficiencies in the existing coverings cannot be accomplished without damage to the roof covering and underlayment, thus voiding any stated or implied warranty from the manufacturer. Manufacturer's certification of warranty will be required if the existing covering is to be corrected. Please advise me of your intention concerning correction of the noted deficiencies when determined by your office. A certificate of Occupancy cannot be issued without rectification of this issue. I appreciate the opportunity to hear your perspective on the various building permit projects that we discussed. Both John and I are committed to delivering a building permit and inspection program that 8/4/2008 Page 2 of 4 provides quality, consistent and fair services. We believe the building division is critical to ensuring that the City of Wheat Ridge is a high quality, healthy and safe place to live now and long into the future. Based on our conversation Wednesday, I developed a list of questions pertaining mostly to the project at 4753 Dover Street. I have had an opportunity to discuss those with John as well as research file records and this e-mail summarizes my understanding of the status of each of those areas of concern. It also provides next steps for you to pursue with the building division to reach resolution on this project. Roof. At the mid-roof inspection on 4/24/08, it was noted by the Building Inspector that the starter course of shingles needed a nailing correction and that the nails had over-penetrated a number of the shingles that had been installed and that those needed to be repaired. The inspection card also noted that re-inspection of the work was required and provided a phone number to call to request that re-inspection. It also noted that is was OK to continue roofing provided the corrections were made and the re-inspection was requested. I understand that you believe that you corrected the work as noted on the inspection notice. Unfortunately, you did not call for the requested re-inspection for the City to confirm that you had corrected the two items noted on the inspection card. In the intervening time, the roof shingles have of course sealed, making it more difficult for the City to verify that the work has been corrected. In the spirit of compromise, we are willing to accept a letter from a technical representative of the manufacturer stating that the roofing has been installed in compliance with all warranty requirements. If you are able to obtain that letter, please call for a final roof inspection and provide that letter at that time. Plumbine. I understand that much of the final plumbing work in the house has been completed. Our records indicate that no rough plumbing inspections have been requested nor completed and there are several notes on inspection cards for other types of inspections identifying the need to obtain a rough plumbing inspection and water supply pressure test. You had also mentioned in our conversation that you felt that the inspector had verbally communicated some level of approval, which would of course not be a typical practice. The purpose of the inspection card is to communicate approval and/or necessary corrections. You had also mentioned that you felt the fact that we approved a framing inspection with a note to allow drywall might have implied approval of rough plumbing. In looking at the note on that partial framing inspection, only a limited number of rooms were OK'd to drywall and those did not include the bathroom or kitchen, where additional plumbing work needed to be installed and inspected. Rough plumbing inspections are still required and may necessitate exposing portions of the plumbing through walls that I understand may have been dry-walled and finished. While this is obviously an inconvenience, I assume that will be necessary to approve the plumbing work. In speaking with John, it sounds as though that can be minimally invasive and can likely be done from the opposite side of the bathroom wall to avoid impacting finish work such as tile. The drainage piping will of course also need to be inspected and approved and based on information from both you and John, it appears the location of the new water heater will need to be adjusted to allow for property discharge piping. You can deal with that issue more specifically through the plumbing inspection process. Mechanical L Electrical: The project has received final electrical inspection as you noted in your e-mail. The permit for the new furnace received a rough mechanical inspection and approval on 1/8/08. The required rough mechanical inspection was not requested for the main remodel project. As a result, the new ductwork was not inspected prior to the installation of drywall. 8/4/2008 Page 3 of 4 You will need to request a rough mechanical inspection and we are hoping that our inspector will be able to adequately review that work through the crawl space, though we cannot guarantee that. Cut Floor Joist. On the furnace rough mechanical inspection card, it was noted that a floor joist had been cut into to allow for the bathtub drain piping and that an engineer's letter would be needed to verify the structural integrity of that joist. To my knowledge, we have not yet received that verification, so please work on that. Permit Fees. I understand that you have some concerns that you may have paid excessive building permit fees on this project as a result of you or your subcontractors applying for multiple permits rather than a single combination permit. In researching our records on this matter, it appears you first applied for (and we approved) a demolition permit in order to get the project started before the full scope of the project had been developed. Your electrical contractor then applied for (and the City approved) a permit for a new 200 Amp service and rough electrical work. Two weeks later you applied for the main permit which noted an interior remodel, addition of bathroom and new roof. We relied on your stated valuation of the work to determine permit fees for the main permit. Lastly, your mechanical subcontractor applied for a furnace and A/C permit, which was not listed in the scope of work as identified on your main permit. I believe we have applied the City's building permit fees properly. I understand you are eager to reach finalization on this project and receive all necessary inspections and ultimately the Certificate of Occupancy. I believe the next steps would be the following: 1. Call in for a rough plumbing inspection. 2. Call in for a rough mechanical inspection. 3. Call in for a roof final inspection provided you have a letter from the manufacturer as discussed earlier. 4. Provide an Engineer's letter regarding the structural issue on the floor joist. 5. Please let John know when you are requesting the inspections so that he can participate in those inspections. You raised some questions on permit fees on other projects during our meeting and I will respond to those questions separately. I trust this letter provides you with the necessary means to reach closure on this project, understanding that some correction measures will presumably be necessary to allow the city to approve all of the required building safety inspections. The results of these inspections and the related conversation were: LATH 1. Staples are required to be installed at a maximum of 6 inches on center both horizontally and vertically. 2. Weep screed is required to be elevated a minimum of 1 inch above solid surfaces below. 3. Proper flashing above garage side and overhead doors is required. PLUMBING 1. Laundry room plumbing is required to be in a conditioned space or all appliances and related piping must be protected from freezing. 2. Most if not all of those projects are being done in partnership with WR2020. Based on our conversation, I have several questions that I would request that you please look into and get me a written response. 4753 Dover Street • Roof. At the mid roof inspection, I understood that the inspector indicated that the over-penetration of the nails had to be corrected prior to proceeding forward to completing the roof. Mr. Blakesley indicated that the inspection card indicated that the over-penetration needed to be corrected but that the work was allowed to proceed to completion. In a more general way, he had some concern that the mid-roof inspection occurs well into the completion of the roof work, which is awkward if corrections are needed. Your thoughts on that issue would be appreciated. • Plumbing. I understood that no rough plumbing inspections occurred on the property and that some work may have been concealed by drywall. Steve indicated that Kirk gave verbal approval of a pressure water test on the plumbing system in the bath and they proceeded to drywall based on that approval. He also indicated that by granting a drywall inspection approval, we had implicitly approved of the rough plumbing. Please respond. • Electrical final inspections. Mr. Blakesley recalled that they have received an electrical final on the hot water heater in the crawl space and wondered whether any code compliance issues associated with that inspection should have been caught at that time. Please elaborate on what some of the potential code compliance issues may be and provide a response on the timing of communicating those concerns. • Separate permits. Mr. Blakesley had some questions regarding the fees associated with the multiple permits that were applied for and approved on the project. Specifically, he questioned whether the City could have added both the fumace and the electrical permits to the main permit as subcontractors and included the value of that work within the base valuation that was provided on the main permit, thus saving them some money on permit fees. He indicated that the furnace was specifically listed as part of the scope on the main permit. • Mechanical inspections. Mr. Blakesley indicated he was unaware of the need for a mechanical (duct work) inspection and thought that would be covered at the mechanical rough inspection or some other time. Please elaborate on the timing for the mechanical inspections. • I understand that there are also some potential issues with some cuts that have been made into structural floor joists. Mr. Blakesley seemed to indicate he thought the issue had been addressed. Please give me a status on that matter. • Mr. Blakesley understands that he has a need to call for additional inspections prior to completing the project and receiving a certificate of occupancy. Thank you. 7/9/2008 4753 Dover Street • Roof. The failure of the mid-roof inspection for over-penetrated nails was noted on the correction notice of 4/24/08, with additional, notes that stated "OK to continue roofing" and re-inspection required. The mid-roof inspection is performed in this manner to allow the contractor to correct the deficiencies or improper nailing, while continuing application of the remaining roofing in the corrected manner. This prevents a cessation of work until re-inspection can be performed, usually on the next business day, and also decreases the period of time that the roofing remains incomplete, thus reducing risk to both the contractor and the building owner. It is necessary to have a portion of the roofing in place to inspect code requirements such as nail quantity, spacing and depth, in addition to underlayment thickness, overlap and attachment prior to completion and sealing of the shingles. The installation quantity of twenty-five percent to seventy-five percent is established to ensure that an adequate amount of work is in place for inspection, but also to allow flexibility for the roofer in coordination of inspections without requiring a stoppage in work. In this instance, just less than fifty percent of the roofing was in place at the time that the inspection was performed. Concerning the timing of the mid-roof inspection, to perform an inspection prior to an adequate amount of roofing having been installed would negate the usefulness of the inspection, and to eliminate the mid-roof inspection and check for all code requirements at final inspection would not be possible or would require extensive work or even removal and replacement of roofing materials when corrections are required. Performance of a mid-roof inspection per current policy is designed to identify deficiencies in roofing application prior to completion, thus allowing correction of any deficiencies prior to the sealing of the shingles and minimizing the amount of work requiring correction. Plumbing. It is not the policy of the Building Division to give verbal approval of inspections, and, in fact, when any inspection is performed, results are given in written form. Three separate written correction notices were provided during inspections on which it was noted that plumbing inspections were required but had not been performed. In addition to water or air testing of supply and drainage piping, plumbing systems are required to meet code requirements for proper connection to the supply and drainage system, proper slope, sizing, protection from freezing and breakage, support, and other code requirements. Based on observations made during a July 14, 2008 meeting with Mr. Blakesley at the structure, this testing could not have possibly passed inspection because the plumbing system is still incomplete and cannot be tested in its' current state. As a result of a partial framing inspection performed on 3/25/08, permission was given to drywall in particular areas of the home, specifically "Living Room, NW Bedroom and closet, NE Bedroom." It was also noted that bathroom plumbing needed to be installed and tested. These were the only areas specified because other areas of the structure which contained plumbing and mechanical systems had not received the rough inspections required to permit approval of the framing and installation of insulation and drywall. Permission to drywall particular areas of the structure was given at the request of Mr. Blakesley to facilitate advancement of the project and does not constitute implied approval of the plumbing, and, in fact, the notation of the absence of plumbing inspections was made in an effort to prevent any such inference. Subsequent approval of drywall was partial, and related only to those specific areas approved for drywall installation as listed in the 3/25/08 inspections. Electrical Final Inspections. Electrical inspections are performed by Master Electrician Mike Villano at the request of the project contractor or electrician. Mr. Villano's area of expertise and qualification is electrical, and he does not perform plumbing inspections. A request was made for inspection of the electrical service upgrade, which was performed on 6/27/08. During this inspection, a request was made for an unscheduled inspection of the electrical work associated with the interior remodel, and the inspection was performed and approved. A request for plumbing inspection was never received by the Building Division. It is not common practice for building inspectors to perform inspections in areas for which they are not trained and where inspection is not requested, nor would most contractors desire to have inspectors inspecting and noting violations in disciplines not related to their requested inspection type. It is interesting to note the shift of responsibility to the Building Division for determining the timing and types of inspections to be performed and the disciplines to be included in each requested inspection. Separate permits. Permits were provided in the order requested and at the valuations stated by Mr. Blakesley and his contractors. Permits for this project were purchased as follows: 1/24/08 Permit 4080061 Demolition - requested by Mr. Blakesley 2/01/08 Permit 4080093 New 200 Amp service and rough electrical work - requested by John Sturken 2/19/08 Permit #080150 Interior remodel, addition of bathroom, new roof - requested by Steve Blakesley 4/01/08 Permit #080284 Replace furnace and add A/C - requested by Paul Fortenberry As can be seen above, Mr. Blakesley's electrical contractor requested a permit for the service upgrade and additional interior electrical work prior to the issuance of a permit for the interior remodel. The submitted plans for the interior remodel included an electrical plan, on which there is no indication of an electrical service upgrade. Mr. Blakesley's application for the interior remodel permit states "interior remodel, add of bathroom, new roof'. There is no mention of a new furnace/AC or electrical service upgrade made in the description of work, the section of the application in which BTU's and amps information is requested is blank, despite having been completed on many of Mr. Blakesley's other applications, and no where on the submitted or approved plans is it indicated that he will replacing or adding a furnace/AC or upgrading the service. Since the permit for the electrical service upgrade was purchased prior to the interior remodel permit, if the service upgrade work was to be included the remodel permit, which, from the submitted plans, it appears that it was not, it would have been the responsibility of Mr. Blakesley to exclude this work from his stated valuation, which was used unaltered to determine the permit fees. The request for a furnace permit after the issuance of the remodel permit was processed as submitted, and could have been added to the interior remodel permit, but this was not requested. Based on conversations with Mr. Blakesley, the furnace was a result of a change order and Mr. Blakesley does not feel he should be responsible for permits required for work by his sub contractors, thus the request for a separate permit made by the HVAC contractor. Again, it appears that Mr. Blakesley is attempting to place the responsibility for accurate submission and coordination of his project permits on the Building Division. For the past six weeks it has been suggested that contractors, including Mr. Blakesley, should have the ability to purchase separate permits for the various portions of their remodeling projects to save them fees, but Mr. Blakesley now indicates that he feels that he was in some way over charged or treated improperly by being allowed to do exactly what he has been advocating. It has also been asserted that the Building Division has exercised too much influence in the valuation and permitting of these projects, yet on this particular project, the assertion is that the Building Division failed to act properly when it processed in the exact order at the unaltered valuation each of the permits requested by Mr. Blakesley. • Mechanical Inspections. The need for inspection of mechanical systems, which would include ductwork, is clearly stated in the 2003 International Residential Code Section RI 09.1.2 which states "Rough inspection of plumbing, mechanical, gas and electrical systems shall be made prior to covering or concealment, before fixtures or appliances are set or installed, and prior to frame inspection." Inspection of ductwork as a part of the rough mechanical has been in the codes and a standard part of the inspection process for decades. As stated in the code, rough mechanical inspection and approval is required prior to final approval of rough framing. Rough mechanical inspections were required and performed on another project performed by Mr. Blakesley within the City. If one is to accept Mr. Blakesley's assertion that he was unaware of the need for this mechanical inspection, this demonstrates a lack of knowledge of the most basic requirements and procedures set forth in the codes, is disconcerting, and in my opinion, constitutes reasonable cause for reconsideration of the continuation of unlimited licensing of Mr. Blakesley's firm. • Cut Floor Joist. The cut floor joist and need for repair was noted on the correction notice for the furnace inspection performed on 4/8/08, and remained umepaired, as pictures indicate, at the time of my meeting with Mr. Blakesley on July 14, 2008. Additional inspections. Mr. Blakesley will not be able to receive final inspections without first resolving the failure to obtain the required rough inspections and the many code violations that currently exist. Mr. Blakesley did not request all of the necessary rough inspections related to framing, plumbing and mechanical, nor did he request all of the necessary insulation and drywall inspections. Based on the substantial number of violations in this work that is visible, investigative inspections requiring exposure of concealed work are necessary. The corrections and inspections that will be required as a result of those investigative inspections cannot be determined at this time. Miscellaneous comments. Mr. Blakesley has been issued an unlimited general contractors license by the City of Wheat Ridge, has performed a similar project at 3950 Kendall Street, and based on statements made by him, has done a vast number of this type project throughout his career. Despite these facts, Mr. Blakesley has initiated or performed work on at least three projects in the City of Wheat Ridge prior to obtaining the legally required permits, has concealed work without inspection on at least four separate projects, and has been found to have unlicensed subcontractors on every project performed by him that has been legally permitted by the Building Division. Mr. Blakesley made representations in his meeting with you concerning permits, costs, and inspections that are verifiably inaccurate. Mr. Blakesley was permitted to obtain separate permits, as he requested, with fees assessed on the unaltered valuations stated on his permit applications, and now contends that he was treated unfairly by being granted everything exactly as requested. Mr. Blakesley failed to obtain the required inspections and re-inspections on this project, covered work that requires inspection, and has related work that is visibly in violation of the most basic requirements of code. Mr. Blakesley has repeatedly violated city ordinances and building code requirements, without penalty, has committed multiple violations on this project, and fails to acknowledge any responsibility for his action, despite the existence of written documentation of warnings and notations of violations. Mr. Blakesley has been given consideration well beyond that of most contractors, is solely responsible for current state of this project, and is not deserving of any additional consideration for waiver of full compliance with all code requirements. Somerset Condos Valuation. Valuation is based on value added to the structure, including all finish work and painting, in accordance with published Building Valuation Data by ICC, which is used by local jurisdictions such as City of Lakewood. The base added value determined for these projects was taken from the WR2020 Ranch House Renovation: Design Idea Book. The combined average resale value as stated in this document, which is stated as seventy percent or less of cost, for a minor kitchen remodel ($15,410) and bathroom remodel ($10,286) constituted the base valuation ($25,696). To this was added a valuation of $4,400 for the new furnace, AC and ductwork (Contract price was $5200). The installation of a new gas fireplace was valued at $1,500. And an additional valuation of $2,000 for electrical, framing, drywall and trim related to the new walls, alcove, balustrade, smoke detectors and misc work throughout the unit was added, bringing the total valuation to $33,596. Based on Mr. Blakesleys statements that the work in each unit was identical, and his submission of plans also indicating identical work, this valuation was applied to each permit in the complex for which he applied. During the discussion on 6/5/08 with Mr. Blakesley concerning these valuations, he was offered a chance to present his signed contracts for the entire scope of work on the project for consideration of revaluation. At that time, he indicated there were separate contracts on the project for HVAC, for the gas fireplace, and for his work, and that his contract was in excess of $40,000. Based on the proper application of building valuation data and stated values of work by WR2020, with added contract values stated by Mr. Blakesley or obtained from contractors, I believe the valuation to be fair and accurate. • Permit Fees. While the web site was inaccurate concerning the need for siding and window pen-nits, the requirements in the codes for siding have been consistent for years. Additionally, a meeting was held with Mr. Blakesley and Mr. Osborn on February 19, 2008, at their request, to discuss permitting requirements. At that meeting, Community Development staff was specifically questioned as to the need for permits for siding and windows, and Mr. Osborn questioned the erroneous information on the web site. Mr. Blakesley and Mr. Osborn were informed that the information was erroneous and that permits for siding were covered under the code. Two weeks passed from the time of that meeting before the unpermitted work was discovered and stopped. In addition to performing the siding project, Mr. Blakesley completely renovated 6912 W. 48th Avenue and began renovation of 6920 W. 48th Avenue without the legally required permits. City of Wheat Ridge Municipal Code 5-76 Section 108.4.1 and 108.4.2 state that "whenever any work for which a permit is required by this code has been commenced without first obtaining said permit, a special investigation shall be made before a permit may be issued for such work. An investigative fee, in addition to the permit fee, shall be collected whether or not a permit is then or subsequently issued. The investigation fee shall be equal to the amount of the permit fee required by this code." Essentially, this is the standard "double" fee assessed by most building divisions for starting work without a permit. This fee was initially added to Mr. Blakesley's permits for these two units, but in an effort to foster a more cooperative relationship with Orion Development, and after an hour long discussion with Mr. Blakesley on June 5, 2008, Mr. Blakesley's permits were reprinted with the doubled permit fees removed. This amounted to a total savings to Mr. Blakesley of $3,278.14. Additionally, based on the urging of Mayor DiTullio for consideration of reduction of plan review fees of repetitive plans, Mr. Blakesley was charged a $100.00 plan review fee for 6930 W. 48th Avenue in lieu of the prescribed fee of $407.47, which resulted in a savings of $307.47. This reduction and reasoning for reduction is noted on the printed permit document. The reduction in penalties and plan review fees afforded to Mr. Blakesley is $3,585.61. Mr. Blakesley was clearly informed of the need for a siding permit, but did not obtain one prior to initiating work. He subsequently performed work on two separate projects without required permits, for which penalties were waived. He was afforded a reduced plan review fee, based on requests from Mayor DiTullio and information provided, which may now be erroneous. He has used unlicensed subcontractors on each of these projects, and is currently in violation of municipal and building code requirements on the project at 4753 Dover Street. It is my opinion that Mr. Blakesley has been afforded consideration well beyond reasonable expectation, and that the consideration has not resulted in an improved relationship or increased efforts of compliance on the part of Mr. Blakesley. I do not believe that a refund of any fees is warranted or deserving of consideration. • Miscellaneous comments. Mr. Blakesley's spreadsheet of permit costs includes inaccurate plan review fees for 6918 W. 48`x' Avenue, and reflects a discounted plan review fee of $100, which is indicated on the permit notes as having been reduced based on submission of a repetitive plan. Mr. Blakesley's spreadsheet also lists his costs for work in each of the eight units that have been permitted, and these costs indicate variations on at least four units. Assertions by Mr. Blakesley and others for the past six weeks that he should have been granted discounted plan review fees based on the each of the eight units is contradicted by the variation in costs which is based on the work to be performed. Mr. Blakesley has initiated several projects at this location without the required permits, and has covered work without the required inspections. In fact, the model unit of this complex was completed without permit, is open to the public, was not permitted until June 5, 2008, and to this date has not received any inspections. Mr. Blakesley and others have misrepresented to interested parties the facts and circumstances surrounding this project, have repeatedly and blatantly violated city codes, and are still in violation of codes. The Building Division has offered cooperation and accommodation and has imposed no penalties on Mr. Blakesley, although thoroughly warranted. Mr. Blakesley has, in turn, made no effort to resolve issues surrounding 6912 W. 48'11 Avenue, and has repeated some of the code violations committed at this location on other projects. 3700 Jay Street • Valuations. Mr. Blakesleys assumptions concerning this project are accurate. The contractor for this project was present at a meeting with Mayor DiTullio, Rob Osborn, City Manager Randy Young, you and me on May 1, 2008, at which time requests for leniency in an effort to resolve outstanding issues was made by Mayor DiTullio. Statements of valuation on projects at this address and one other were made on permit applications by this contractor, to which I raised objection. I was asked, in the interest of appeasement and cooperation, to accept these valuations, which were clearly understated. Permits were issued with costs based on these valuations. While these valuations were lower relative to the valuations of Mr. Blakesley's projects, the resulting savings to that contractor were still significantly lower than those afforded to Mr. Blakesley in the waiving of his investigative inspection and plan review fees. This issue serves as an example of the potential problems and perceptions of favoritism that are created due to inconsistent valuation and enforcement of fees and codes, and the need for fair and equal treatment of all Building Division customers. Belleview Avenue Englewood, CO 80111 RE: 4753 Dover Street, Wheat Ridge, CO 80033 Mr. Blakesley, i/ It was brought to the attention of the Building Division that work on the project at the above address had progressed beyond the point of inspection required by the international Residential Code and was no longer visible for inspection. At the request of the Building Division, a meeting with you was conducted at the property on Tuesday, July 14, 2008, to assess the status of the project and any code violations that had been previously noted for correction and to identify any other code violations that may exist related to the project. The attached list of code violations related to the project was generated. This list includes only the items that were readily visible, and therefore may not be a complete list of all violations that have occurred. As a result of the meeting and inspection, it has been determined that: On April 24, 2008, a mid-roof inspection was performed and required corrections were noted, including "renail all shingles with blown thru nailing." The inspection notice also indicated that work may continue, but that correction and re-inspection was required. During the inspection conducted on July 14, 2008, it was determined that corrections had not been made, that improper nailing of shingles still existed and that a re-inspection had not been requested. Inspection of only a small sampling of the roof was made due to your request that the roof inspection be terminated. On March 25, 2008, and on April 3, 2008, rough framing inspections were performed and corrections were noted. The corresponding correction notices included notes stating "bathroom plumbing to be installed and under test", "need rough plumbing inspection", and that tests for tubs and DW V were required. It was determined during the inspection on July 14, 2008, that the required rough plumbing inspections had not been performed, that drywall and wall coverings were in place, and that work had been concealed without inspection. It was also determined that substantial violations of code requirements for plumbing systems and appliances exist in work that is visible, and that a licensed plumber as required by state and municipal statutes has not been identified. Photographs of these violations were taken. On April 8, 2008, a rough mechanical inspection was performed, and one framing correction was noted. This inspection was related only to the installation of a new furnace and air conditioner, as stated on that particular permit and application. It was determined during the inspection on July 14, 2008, that the required rough mechanical inspections related to the interior remodel project had not been performed, that drywall and wall coverings were in place, and that work had been concealed without inspection. It was also determined that violations of code requirements for mechanical systems exist in work that is visible, and that the required correction or engineer approval of an over-notched floor joist noted on the April 8, 2008 inspection was not accomplished. Finally, it was determined that work has been completed in violation of building codes and without inspection and cannot be verified due to the fact that it has been concealed, and that removal of completed work as necessary to provide exposure of uninspected work areas will be required. Please be advised that correction of all items noted on the attached list of violations, in addition to any violations noted during the investigative inspection process, will be required and must be inspected and verified. Also, it is requested that you contact the Building Division no later than July 31, 2008, to discuss the process for obtaining the required investigative inspections of the concealed work and correction of the existing deficiencies in order to reach a resolution of this matter. Garages shall be separated from the residence and its' attic area by not less than one-half inch drywall applied to the garage side. Openings from a garage into a residence shall be made only through the use of doors. No rough plumbing inspections have been performed on the project and work has been concealed (2003 IRC Sec 109.4) Additional plumbing violations visible from the crawl space are: a. temperature and pressure relief piping on water heater is required to be piped to the exterior or an approved drain (2003 IRC P2803.6.1) - Discharge piping drains directly into water heater pit in crawl space and is not visible to occupants b. piping is required to be fully insulated and supported in accordance with Table 2605.1 and sections P2603.6 and P3001.2 of the 2003 IRC - All supply and waste piping and the water heater are required to be insulated in crawl space c. drain lines shall be sloped as required by Sec P3005.3 of the 2003 IRC - Drain piping in crawl space has reverse slope and/or does not have code required slope d. water supply and drain, waste and vent piping shall be tested as required by 2003 IRC Secs P2503.5 and P2503.6 - No testing has been performed on any plumbing systems e. drain piping shall be connected to the sanitary drainage system (2003 IRC P2601.2) - Bathtub is not currently connected to drainage system and is draining into crawl space £ plumbing fixtures shall be spaced in accordance with IRC 2003 Figure 307.1 - Fixtures do not meet minimum spacing requirements in figure 307.1 of the 2003 IRC 6.
