FOR ROOF PITCH 6/12 AND OVER: Midroof & Final Roof inspections for ROOFS with 6/12 PITCH &
OVER: 3rd party inspection will be required for both the midroof and final inspections. The
3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped off to the
Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy
3D-18-001 for Balanced Ventilation Systems, or manufacturer installation instructions,
whichever is more stringent.
MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an
approved mid -roof inspection, conducted when 25-75% of the roof covering is installed,
prior to final approval. Installation of roof sheathing (new or overlay) is required on the
entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists.
Sheathing and mid -roofs may be called in at the same time, one hundred percent of the
sheathing must be complete and 25-75% of the mid -roof may be complete. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required.
LADDER MUST BE SET FOR ALL ROOF INSPECTIONS: A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections.
COMMERCIAL ROOFS: In order to pass a final inspection of elastomeric or similar type roof
coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new RESIDENTIAL roofing permits and for
licensed contractors only. Permits are processed and issued in the order they are
received. You will be notified if your contractor's license or insurance has
expired. The license and insurance must be current before the permit will be
processed.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in
person at City Hall. Revisions to existing permits (for example, to add
redecking) must be completed in person at City Hall. The Building Division
permit counter is open from 7:30 am to 3:30 pm, Monday through Friday to
process these types of requests.

As requested, Family Environmental Compliance Services, Inc. (Family Environmental)
performed a final air clearance for your asbestos abatement project at the above referenced
address
Final clearance air sampling was performed following the removal of the asbestos containing
material and samples were collected and analyzed by a successful AIHA participating member.
Removal and disposal of the asbestos containing material was performed by Asbestos Abatement.
Family Environmental conducted a visual observation of the work area as well as PCM Final Air
Clearance Testing.
Five (5) air samples were then collected using high volume pumps. Samples were collected by
drawing air at a rate of approximately 14.7 liters per minute (LPM) through 25mm mixed
cellulose ester membrane (Millipore 0.8 MCEF) filters, which were housed in three -piece
cassettes, equipped with 50mm electrically- conductive extension cowls. Samples were collected
with variable volume pumps. Flow rates were established and checked with a calibrated flow
meter.
Clearance samples were analyzed for airborne fibers by Phase Contrast Microscopy (PCM) per
NIOSH Method 7400 using a positive Phase- contrast microscope equipped with a Walton -
Beckett graticule (Type G -22 for `A counting rules'). A quarter -wedge was cut from each
sample filter and was examined at a magnification of 400x. Fibers greater than 5 microns in
length with a length -to -width (aspect) ratio equal to or greater than 3:1 were counted. Total fiber
counts for each sample filter were divided by their respective sample volumes. The resulting
concentrations were expressed in terms of total fibers per cubic centimeter of air (fibers /cc).

Phase Contrast Microscopy (PCM) is not asbestos specific because it counts total fibers, which
shows worse case situation because all fibers are counted (carpet, cloth, hair, paper, asbestos,
etc.).
Results from the area sampled during final clearance testing had fiber counts 0.0lVcc (fibers per
cubic centimeter of air) or less. This is the level the FPA and the State of Colorado Air Quality
Control Commission requires all samples to be equal to, or less than, prior to releasing an area to
the public.
Based on visual observations; documentation review; and arc sample results, it is the opinion of
Family Environmental that the air quality within your work area was in compliance with
regulatory and industry standards at the time of this sampling.

Dear Mr. Staab:
At your request we have evaluated the fire damage to the structural
components at the Dirnberger residence located at 4713 Cody, Street, Wheat
Ridge, Colorado. It is our understanding a fire occurred on July 15, 2011, in
the concealed portions of the first floor ceiling joists. The fire spread to several
ceiling joists before being extinguished. At the time of our site visit the
reported cause of the fire was related to a recessed can light located in
bathroom ceiling.
The purpose of our investigation was to determine the extent of structural
damage to the home that resulted from the fire and determine what structural
repairs were warranted. Our work was performed commensurate with Higgins
& Associates, Inc.'s Standard Terms and Conditions dated March 1, 2011.
As ,part of our investigation we conducted a site visit on Wednesday July 20,
2011. The homeowner's son, Mr. Robert Dirnberger, met us at the property
and provided us with a history of the home. A site map of the property has
been included under Exhibit 1. Photographs taken during our site visit have
been included under Exhibit 2 and are considered ancillary to the written
portions of this report.

Dirnberger Residence
Narrative
The Dirnberger residence is a one -story single family home that was originally
constructed in 1956. According to the Jefferson County Assessor's Office
records, the structure is approximately 1,300 square feet in area with 'a
detached garage. The structure is constructed with wood - framed exterior
walls, first floor, and roof structure. The roof covering consists of asphalt
composition shingles.
Our research indicates the authority having jurisdiction over the repairs is the
City of Wheat Ridge Building Department located at 7500 West 29th Avenue,
Wheat Ridge, Colorado. The Wheat Ridge, Building Department has currently
adopted the 2006 International Residential Code. All repair/ restoration work
should conform to the provisions of the 2009 International Residential Code.
Findings
In general, the smoke damage to the interior is minor.. Similarly, the fire and
heat damage to the structural members is minor and limited to surface
charring on multiple ceiling joists and the top plate of an interior load - bearing
wall. The most severe charring on the ceiling joists was approximately 3/4 to
1 inch deep by approximately 2 feet in length. The observed level of charring
has compromised the structural integrity of those members. According to the
2006 International Residential Code adopted by the Jefferson County Building
Department, Section 8102.7, Existing Structures:
"R102.7.1 Additions, alterations or repairs. Additions, alterations or
repairs to any structure shall conform to the requirements for a new
structure without requiring the existing structure to comply with all of
the requirements of this code, unless otherwise stated. Additions,
alterations or repairs shall not cause an existing structure to become
unsafe or adversely affect the performance of the building."

According to the aforementioned section of the building code, repairs should
not cause a building to become unsafe or adversely affect the unaffected
portions of the building. Therefore, we evaluated the exposed portions of the
wall and ceiling joist framing in the area of the fire to determine what
structural repairs are warranted by the fire damage and prepared our
recommendations for the fire remediation accordingly. The owner, insurance
adjuster, and remediation contractor should be aware that it was beyond our
scope of work to thoroughly inspect all building components; perform a
structural analysis of unaffected items; or certify any structural component not
described below is safe, adequately maintained, or complies with the current
building code.

Dirnberger Residence
At the location of the fire damage, the 2 by 6 ceiling joists support the weight of
the first floor ceiling drywall. The joists are not intended to support any attic
storage loads. The interior wooden studs are not load - bearing, but should be
repaired to their pre -loss condition where they were damaged by the fire event.
We recommend the structural components affected by the fire be repaired. The
following represents our best judgment of the fire damage to the home and may
serve as a guide to the insurance adjuster and the remediation contractor who
repairs the structure to ensure all damaged members are repaired or replaced
to their preloss condition or better. We recommend the fire damage to the
structural components be mitigated as follows:
A. Provide temporary shoring throughout construction as needed. Neither
this report nor the attached plans identify all locations or what types of
shoring are required.
B. Remove the charring from the fire - damaged ceiling joists positioned over
the kitchen and bathroom. Sister all five ceiling joists with a new 2 by 6
Douglas -Fir No. 2 member. The new 2 by 6s should be installed to
provide a continuous tie between the exterior walls and should be
fastened to the existing ceiling joists with PL400 construction adhesive
and two rows of 12 -penny box nails spaced at 6 inches center to center.
The new ceiling joists should be miter cut at the exterior wall to
accommodate the roof profile. The new ceiling joists may be but spliced
with the new and existing 2 by 6s provided a Simpson MSTC28 steel
strap ties is installed over the joint. Fasten the ends of the new ceiling
joists to the exterior wall top plates with three 10 -penny toenails.
C. Remove and replace the upper ply of the 2 by 4 top plate along the non -
load bearing east and north walls of the bathroom. Rebuild the double
top plate with new 2 by 4 studs by fastening the two members together
with a continuous bead of PL400 construction adhesive and one row of
10d nails spaced at 12 inches center to center.
D. Remove all charring from the 2 by 4 ridge board diagonal brace and
sister the member with a new 2 by 4 member. The new 2 by 4 should be
fastened to the existing diagonal brace with PL400 construction adhesive
and one rows of 12 -penny nails spaced at 6 inches center to center.
Attach the lower portion of the diagonal brace to the top plate of the

E. Asbestos inspection shall be conducted by a Colorado Certified Asbestos
Inspector, prior to any demolition or removal of the interior contents.
Inspection services shall include all deliverables required for regulatory
compliance, from the initial inspection through proper permitting for
demolition related to the project. Regulated asbestos containing
materials (RACM) must be properly removed and disposed by a licensed
abatement contractor, prior to any construction activity involving the
demolition, destructive salvage, de- construction, burning, dismantling,
implosion, razing, pulverizing, or wrecking (in whole or in part) of the
structure.
F. All required electrical and /or plumbing repairs should be evaluated by
others and repaired per the City of Wheat Ridge Building Department
requirements. The full nature, extent, and'or /allowance of the cosmetic
repairs and appliance /hardware /textile replacement should be
coordinated with and approved by the owner and the insurance adjuster
prior to construction:
G. Mitigation of non - structural smoke, heat, and /or secondary water
damage to the building should be performed by a contractor with
knowledge and experience in that field and in accordance with industry
standards. The contractor should perform all necessary work to ensure
no residual smoke odors exist in the structure at project completion.
Higgins 8s Associates, Inc. recommends that a contractor specializing in
fire mitigation who is familiar with industry standards perform the
smoke and heat remediation of the building. The fire restoration repairs
and project documentation should be performed in accordance with the
2nd Edition of the Guidelines for Fire and Smoke Damage Repair
published by the Restoration Industry Association (R.I.A.).
H. Damage repair work should be performed only after the contractor has
provided a detailed description of the work, associated costs, and there is
written agreement between all parties identifying the scope of work and
material specifications. The owner and insurance adjuster should be
notified and allowed to inspect all unforeseen conditions or costs not
included in the original estimate prior to incurring additional costs.
Latent damage associated with the fire event is not anticipated. Nevertheless,
should structural distress develop following the completion of the repairs, our
office should be contacted as soon as possible for re inspection of the home.
As with any existing building the owner is responsible for establishing and
maintaining good landscaping and drainage provisions to protect the
foundation and building envelope.

Summary and Conclusion
It is our opinion, based upon a reasonable degree of engineering probability,
the fire and heat damage to the structural members is minor and limited to
surface charring on several ceiling joists and a limited section of the non -load
bearing interior wall framing. The most severe charring on the ceiling joists
was approximately 3/4 to 1 inch deep by approximately 2 feet in length.
We believe the repair of the structure and remediation of the fire damage will
be fairly straightforward; however, due to the nature of fire damage, it is often
difficult to determine the precise extent of the damage until demolition is
complete and all concealed spaces are exposed. The framing should be
continuously inspected by the contractor as selective demolition occurs.
Should the repair process reveal deficiencies or damage not anticipated by this
report, the remediation contractor should contact our office and the insurance
adjuster for inspection and guidance. We will share any supplementary
findings with your office. Identification and remediation of all smoke, heat,
and /or secondary water damage are the sole responsibility of the contractor.
The owner and insurance representative are hereby made aware that the
proposed scope of work represents our best judgment as to what is reasonable
based upon the level of damage. The owner, insurance provider, the repair
contractor, and /or the building official may have different interpretations that
could change the scope of work. The preparation of repair recommendations is
not to be construed as an indication of insurance coverage. It should be noted
our scope of work was limited to the affected framing components; specifically,
the fire damage to the ceiling joists.
The scope of our observation was limited to areas of the structure that could be
easily accessed and viewed with a flashlight. Our investigation was completed
to determine the extent of the damage to structural members due to the fire
event and is limited to that subject matter only. This report is not intended to
represent a thorough inspection of all building components or a structural
analysis of items unaffected by the fire event. Our evaluation of the building
does not represent an endorsement of the existing structure or a certification
that any structural component not repaired as outlined above is safe,
adequately maintained, or complies with the current building code.

Specification for Bid /Estimate
• Demo:
1. Remove Plumbing fixtures; Tub, Toilet and sink.
2. Remove drywall from all walls to expose wood studs.
3. Remove flooring and sub - flooring to expose floor joists.
4. Remove 24" door /frame.
• New, construction:
1. Repair floorjoists as needed to support new tub and toilet.
2. Repair /replace waste plumbing lines (ABS /PVC).
a. New waste line for Toilet to main.
b. New waste line for tub and sink.
c. New vent lines (to roof) for toilet, tub and sink.
3. Replace /repair water piping (galvanized to copper or plastic).
a. New water line /with shutoff valves for toilet, shower /tub
and sink.
b. Install new faucets (OP) for shower /tub and sink.
4. Electrical:
a. New light, wall mounted above vanity ($125 allowance).
b. New ground fault outlet above vanity with light /fan switch.
c. New vent fan (Nutone 100 cfm, light and night light, $150
allowance).
5. Flooring:
a. Install new sub - flooring and plywood floor.
b. Install new sheet vinyl floor covering, caulk wall and floor
joint for water tight joint install cove base vinyl /rubber.
6. Walls:
a. Install new moisture resistive drywall (1/2" green board). All
walls, tape and finish smooth.
b. Install water proof HC tub /shower encloser on wall for
tub /shower if required.
c. Paint walls; primer drywall, paint with 2 coats semi gloss,
bathroom paint /color to be determined.
d. Install new 30 "/32" pocketdoor, wall frame and trim.
7. Bathroom Fixtures:
a. Install new Comfort Height (HC) dual flush toilet (Kohler) or
equal with toilet seat ($300 allowance).
b. Install new vanity /sink (OP).
c. Install new tub /shower (OP) combination unit and encloser
with shower door and HC handle.
d. Install new recessed Medicine Cabinet /mirror.
• For the purpose of bid owner (OP) will purchase fixtures:
Owner Provided (OP) fixtures include tub /shower /faucet;
vanity /sink /faucet; floor covering. After award owner will work
with contractor for possible inclusion of OP fixtures as a change
order.
• Contractor's bid will provide for all construction materials, labor
and fixtures not specifically excluded (tub, vanity, floor covering).
Included materials but not limited to:
All misc. electrical wiring, outlets, switches, boxes etc.
All misc. plumbing piping, hangers, fittings, valves etc.
All misc. fixtures (vent fan /light, HC toilet etc.) within the $
allowance, pocket door and all construction
materials /lumber /drywall /paint etc.
• Contractor must complete the entire renovation work of
bathroom during the period of Jan. 28 to Feb.14
• Contractor's bid to list any exclusions.
Contractor is responsible for all permits and licenses required by
the city /county /state.
