In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

We hereby propose to furnish the materials and perform the labor necessary for the completion of Repair of hail damaged roof
5777 sq. ft.
Roof has major damage but can be repaired as prescribed by the product manufacture.
We will clean and power wash the roof with a special cleaner from Endurtech and remove all debris from the roof.
We will repair all hail damage by caulking all holes.
We will coat the total roof area with 2 coats of Enduratech Roof Coating.
The first coat will be a gray base coat back rolled to ensure total coverage. The second coat will bne a reflective
white top coat
Recoat is $3.00 per s .ft.
Bid includes taxes and permit cost.
All material is guaranteed to be as specified, and the above work to be performed in accordance with the drawings and specifications
submitted for above work, and completed in a substantial workmanlike manner for the sum of sixteen thousand

SUBJECT: Jerry Roach Building Permit Application (7331 West 44"` Avenue)
Although this is only an addition to an existing structure rather than a new development, the proposed
building expansion and parking lot addition will substantially increase the amount of impervious surface
to the site and ultimately change the site drainage. Detention may be required to minimize pollutant
transmission and adverse impact to adjacent properties as a result of the proposed increase in runof£ The
site runoff release is to be released at or below historic rates, and the site drainage must also comply with
the requirements set forth in the Clean Water Act.
Therefore, at a minimum the applicant will need to submit the following docuxnents for review and
approval prior to Public Warks approval of the Building Permit Application. All documents will need to
be signed and sealed by a Professional Civil Engineer licensed in the State of Colorado:
1. Drainage Plan, and a Drainage Letter containing the following information:
a. Identify, compare and discuss historic and proposed runoff values utilizing the Urban
Drainage & Flood Control District Drainage Criteria Manuals,
b. A description of how the site drainage will be addressed, including any measures utilized
to minimize pollutant dischazges from the site (i.e., landscape buffering, etc.).
c. A confirtnation that the proposed site grading will be performed in such a manner as to not
adversely impact the proposed structure or adjacent properties due to the increase in site
runof£
d. The Drainage Letter must be signed and stamped by a Professional Engineer licensed in
the State of Colorado.
e. A signed and stamped Drainage Plan showing the proposed site grading, all proposed
drainage facilities, and the direction of runoff, shall accompany the Drainage Letter.
2. Grading & Erosion Control Plan: A Grading & Erosion Control Plan including details for ail
BMP's to be utilized during construction. Show the type of landscaping in and azound the detention
area, e.g., sod or native seeding. Include the seed mix and the rate of application if native seeding is to
be utilized.
3. Construction Plans (If applicable), for any proposed chanQes/additions to the public
improvements along the 44th Avenue frontage. Include any COWR standard details for all
proposed constructed items (e.g., drivecut, sidewalk, etc.).
Please be advised that if the increase in runoff is deemed to be substantial enough by the Department
of Public Warks to warrant on-site detention, additional information including a comprehensive Fina1
Drainage Report and Plan may be required.
NOTES:
• Information pertaining to the Public Warks Development Rediew requirements, Site Drainage
Requirements, surveying/ROW information, and the City's Standazd Street Construction and
Erosion Control Details in AutoCADO 2000.DWG format, is available on the City of VJheat

SUBJECT: Jerry Roach Buildingl'ermit Application (7331 West 44`h Avenue)
Detenrion is required of all commercial developments. Although this is only an addition to an existing structure
rather than a new development, the proposed building expansion will substanrially increase the amount of
impervious swface to the site and ultimately changing the site drainage. Detention is required to minimize adverse
impaci to adjacent properties as a result of the increase in runoff. This will be accomplished by keeping site mnoff
release at or below historic rates, and is also needed to comply with requirements set forth in the Clean Water Act.
Therefore, the applicant will need to submit the following documents for review and approval prior to Public
Works approval of the Building Permit Application. All documents will need to be signed and sealed by a
Professional Civil Engineer licensed in the State of Colorada
1. Final Drainage Report: As on-site detention is required for all commercial developments (incorporating
water qualiry measures) a Final Drainage Report and Plan shall be required. (Please see attached Site
Drainage Requirements).
2. Grading & Erosion Control Plan: A Crrading & Erosion Control Plan including details for all BMP's to be
utilized during construction. Show the type of landscaping in and azound the detention area, e.g., sod or native
seeding. Include the seed mix and the rate of application if native seeding is to be utilized.
3. Construction Plans (If applicable), for any proposed chaneesJadditions to the public improvements along
the 44th Avenue frontage. Include any COWR standard details for all proposed constructed items (e.g.,
drivecut, sidewalk, etc.).

infotmarion, and the City's Standazd Sheet Construction and Erosion Contro] Details in AutoCADO

A signed and sealed Drainage Certification Letter from the Engineerof-Record (P.E. who did the Final
Drainage ReporUPlan), and As-Built Plans for all dcainage facilities and improvements (if applicable)
