Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Al,z-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufactureraeTs installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer&*39;s technical representative stating that "the application of the roof at

This application is exclusively for new permits for residential roofs and for licensed

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Statement Regarding Deductible : If the Owner plans to use the proceeds of a property and casualty

roofing contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or any part of any

The Scope of Roofing Services and Materials to be Provided:
1. Supply and install ice and water shield at eave lines 2' into the building envelope if
required by code.
2. Supply and install 30# felt underlayment mechanically fastened.
3. Supply and install new pre -finished metal edge, step flashing, 4x5 flashing, and new
valley flashing, replace and paint pipe flashings.
4. Supply and install the roofing system identified above.
5. All work will be done in a professional workman like manor. Shingles will be six (6)
nailed for wind uplift.
6. Mastic & seal all pipes and areas necessary.
7. Jobsite will be supervised daily.
Clean up all debris daily resulting from roof work, sweep yard with magnet to pick up
loose nails.

Owner's Right to Rescind within Seventy -Two (72) Hours: Notwithstanding anything else herein,
Owner may rescind the Roofing Contract and obtain a full refund of any deposit within seventy-
two (72) hours after entering this Roofing Contract.'
1 Written notice of rescission if made in an electronic form, is effective on the date of the electronic transmission; if
made by mail is effective upon deposit in the United States mail, postage prepaid, sent to Roofing Contractor at its
address stated above; or if made by personal delivery to Roofing Contractor, is effective upon delivery. Roofmg
Contractor's contact information for each method of delivery is specified above under "Roofing Contractor's Contact
Information."
Roofing Contractor's Policy Regarding Cancellation of the Contract and Refund of any Deposit:
This Roofing Contract cannot be cancelled after such seventy-two (72) hours as set forth above,
except in accordance with C.R.S. § 6-22-104, as set forth below.
Owner May Rescind this Roofing Contract under Certain Circumstances if Payment is to be made
from Insurance Proceeds if Claim is Denied: If the payment for the Roofing Services will be made
from the proceeds of a property and casualty insurance policy issued pursuant to part 1 of article
4 of title 10, C.R. S., Owner may rescind the contract within seventy-two (72) hours after the Owner
receives written notice from the property and casualty insurer that the claim for payment for
roofing work on the residential property is denied in whole or in part pursuant to C.R.S. § 6-22-
1042, which is incorporated into this contract by reference. The Owner's right of rescission under
this section does not apply when the property and casualty insurer denies, in whole or in part, a
claim related to a request for supplemental roofing services if the damage requiring the
supplemental roofmg services could not have been reasonably foreseen as a necessary and related
roofing service at the time of the initial roofing inspection or the execution of this initial Roofing
Contract. Any notice of rescission pursuant to this section must be given in writing within
2 C.R.S. § 6-22-104. Residential Roofing Contract - (1)(a) A Property Owner who enters into a written contract with
a Roofing Contractor to perform Roofing Work on the Property Owner's Residential Property, the payment for
which will be made from the proceeds of a property and casualty insurance policy issued pursuant to part 1 of article
4 of title 10, C.R.S., may rescind the contract within seventy-two hours after the Property Owner receives written
notice from the property and casualty insurer that the claim for payment for Roofmg Work on the Residential
Property is denied in whole or in part. The Property Owner's right of rescission under this subsection (1) does not
apply when the property and casualty insurer denies, in whole or in part, a claim related to a request for
supplemental Roofing Services if the damage requiring the supplemental Roofmg Services could not have been
reasonably foreseen as a necessary and related Roofmg Service at the time of the initial roofing inspection or the
execution of the initial roofmg contract. (b) The Property Owner shall give written notice of rescission of the
contract to the Roofing Contractor at the physical address provided in the contract within seventy-two hours after he
or she is notified of the denial. The Property Owner may give notice of rescission of the contract: (i) in an electronic
form, which is effective on the date of the electronic transmission; (ii) by mail, which is effective upon deposit in the
United States mail, postage prepaid, sent to the physical address stated in the contract; or (iii) by personal delivery to
the Roofmg Contractor, which is effective upon delivery. (2) Within ten days after rescission of a contract in
accordance with subsection (1) of this section, the Roofing Contractor shall return to the Property Owner any
payments or deposits made by or evidence of indebtedness of the Property Owner in connection with the contract for
Roofing Work on the Residential Property. (3) Nothing in this section precludes a Roofmg Contractor from retaining
all or a portion of any payments or deposits made by a Property Owner to compensate the Roofing Contractor for
Roofing Work actually performed on the Residential Property in a workmanlike manner consistent with standard
roofmg industry practices, but the Roofing Contractor may retain only an amount required to compensate the
Roofing Contractor for the actual work performed. (4) Nothing in this section abrogates the Roofmg Contractor's
right to pursue common law remedies for the reasonable value of roofing materials ordered and actually installed on
the Residential Property pursuant to a contract for Roofing Work before the Property Owner rescinded the contract,
as long as the Roofmg Contractor performed the Roofing Services consistent with roofing industry standards for
workmanlike performance of Roofing Services.

seventy-two (72) hours after Owner is notified of the denial. Such notice is subject to the terms of
footnote 1 of this Roofing Contract and is to be given in accordance with such footnote.
Warranty Transfer: The warranty contained herein is transferable by the Owner to a new owner
of the home, only upon written notice to the Roofing Contractor and payment of a $35.00 transfer
fee. Written notice and payment must be made within 45 days of new owner taking title.
General Terms: Roofing Contractor will make its best effort to clear debris from the site in order
to leave it in a close to the same condition as when the work began. Some residue from construction
may remain. Owner assumes risks associated with such residue. Shingle pattern and paint color
at Roofing Contractor's discretion, unless stated otherwise in this Roofing Contract. Roofing
Contractor recommends a complete tear -off for best results with regard to smoothness. In the event
that the roof has not passed inspection or re -inspection within 30 days of final billing, the balance
of the contract amount is still due in full. In the event of a dispute between the owner and the
Roofing Contractor, the Roofing Contractor shall have the right to legally binding arbitration.
Payment Terms: Payment of final contract amount is due at time of substantial completion of work
and final billing. The total price quoted or estimated herein may not include all supplements and
or additional work performed. If the payment for the Roofing Services will be made from the
proceeds of a property and casualty insurance policy, amounts will attempt to be approved and if
possible billed directly to the insurance carrier, but Owner is ultimately responsible for the final
contract amount, except as expressly provided by applicable law. All accounts not paid in full
within 30 days shall accrue interest at 1 and 1/2% per month (18% per annum) on the outstanding
balance thereafter. Roofing Contractor shall be awarded all attorneys' fees and costs associated
with collection of unpaid balances. No lien wavier is effective unless payment is made in full,
including all above charges, supplements and additional work. To the extent not expressly
prohibited by law, Roofing Contractor shall be entitled to compensation from Customer for any
custom or non -returnable materials that may have been ordered prior to any contract rescission.
Roofing Contractor reserves the right to cancel this contract at any time.
Existing Conditions: Unless specifically included in the scope of work, Roofing Contractor is not
responsible: for installing chimney counter flashing or leaks caused from lack of chimney counter
flashing; for non -roofing related chimney leaks; for non -roofing related leaks from skylights, solar
panels, coolers, windows, siding, gutters for moisture damage due to inadequate attic ventilation
for leaks or damage caused from ice dams; for damages from the result of pre-existing conditions;
for leaks or damage to structure or contents during ordinary loading and application of shingles;
for damage caused by subsequent alterations or repairs by others. Owner is responsible to notify
Roofing Contractor if there are exposed ceilings or area with no attic space or open eaves.
Warranty Limitations: This warranty does not apply to damage caused by events not in control of
Contractor, including, but not limited to: severe winds or weather, fire, traffic on roof, impact of

falling objects, unintended loads existing design or construction of roof / home, inadequate roof
drainage, subsequent structural changes or of roof deck, walls or foundation, variation in color of
shingles due to natural positioning of granular surface material, faulty materials or lack of thermal
sealing of shingles due to insufficient warm sunlight. This warranty may not be modified or altered,
and does not apply if damage is caused by abuse, misuse or failure to properly maintain structure.
As a condition precedent to Contractor's warranty obligations, claims pursuant to this warranty
must be submitted in writing to the contractor within 30 days after discovery of alleged defect in
workmanship. Contractor is not responsible for any damage to the structure or its contents due to
leaks. Contractor's sole liability is to repair the lead if the same occurs during the warranty period.
