Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding K1/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera,E'TMs installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

I . This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made -be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and_procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4, No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or g ting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code q ordinance eguI tion of this jurisdiction. Approval of work is subject to field inspection.

Dear Mr. Podrasky:
At your request our office performed a structural investigation of the vehicle
impact damage to the subject building. As part of our investigation, we visited
the site on June 29, 2010. Photographs taken during our site visit (attached to
the Appendix of this report) document the observed conditions and are
considered illustrative and corollary to the written portions of this report. The
photo numbers referenced in this report refer to the digits preceding the ".JPG"
portion of the filename adjacent to each photo.
Purpose and Scone
The purpose of this investigation is to provide structural repair recommendations
to return the framing to its pre -loss condition and strength. This report is based
on items that were visible during our site visits. If other damage is discovered,
our office reserves the right to amend this report accordingly. This report is not
to be interpreted as an endorsement of the remaining structure.
Description
The subject building is a three unit single story residential structure with a hip
roof. The exterior walls are clad with brick veneer, and the roof is covered with
asphalt shingles. For orientation purposes the front of the building was assumed
to face west. According to Jefferson County records, the building was
constructed in 1956.

A vehicle impact recently occurred at the west family room wall of Unit 3760,
causing structural damage to the wall framing and brick veneer. The subsequent
sections of this report discuss our observations and the structural repairs
required to return the structure to its pre -loss condition and strength.
Structural Damage and Repair Recommendations
During our site visits we observed the following:
o The exterior wall framing consisted of 2x4's at 16 inches on center, double
top plate, with fiberboard sheathing. The floor consisted of a concrete
slab on grade.
o Prior to our site visit the plaster (at approximately 16 feet of the west
family room wall) had been removed to the expose the wall framing. The
west wall studs were pushed inward, the bottom plate was damaged, and
some of the anchor bolts were bent — this occurred at the south 12 feet of
the wall (photos .18 - .32). This wall provides support for the roof framing,
and possibly ceiling framing also.
o There was cosmetic damage to the plaster at the south window of the
family room (photos .33 -.39). The east window pane was cracked and
the window appeared to be distorted relative to the brick and soffet
(photos .37 and .58). The plaster damage at the upper east and lower
west corners, the broken glass, and the distortion of the window are most
probably the result of the impact.
o The brick veneer at the south 12 feet of the west wall and west 2.5 feet of
the south wall was damaged (photos .17, .46 -.56).
o The west family room window was distorted (photo .57).
o There was a patched north -south crack in the ceiling of the family room,
approximately 5 feet from the west wall (photos .42 and .43). This is not
related to the vehicle impact, but rather due to past minor foundation
movement.
o There was a patched crack that has re- opened at the north wall of the
family room (photos .44 and .45). This is not related to the vehicle impact,
but is rather due to interior floor slab movement, both past and present.
The structural repair plan is included as Attachment 1 to this report. Based on
our observations and evaluation, the following structural repairs are required to

return the framing to its pre -loss condition and strength (See Attachment 1 for
repair details):
1. Temporary shoring to support the roof and ceiling framing.
2. Repair to the south 12 feet of the west family room wall by replacing the
damaged bottom plate, installing new epoxied anchor bolts at 48 inches
on center, replacing the damaged studs, and replacing the exterior wall
sheathing.
3. Removing and replacing the damaged brick veneer.
4. Interior cosmetic repairs.
5. Replacement of the west and south family room windows as necessary.
