3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all requ�ired
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, anviolation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

We will be removing bedroom hallway walls and closet to convert
into one large room. One wall is a structural wall (per structural
engineer report) and so we will be placing an LVL support bean.
We will be removing two bedroom closets and a window in one
room. We will be framing out a new door entry that leads to
outside where the window is being removed. We will be
reframing the hallway, now entry into the converted large room.

Dear Mr. Schwartz:
In accordance with your request, I have provided the following letter regarding the removal of
the wall between the two second floor bedrooms, and an addition of a header to accommodate
a new sliding glass door in the first floor bedroom, which I understand will take place as part of
a future interior renovation. This letter does not pertain to any other component or system of the
property.
For the purpose of this report, the front door of this house is directly east.
Following is a report of my findings:
The property is a two-story house, built for single-family occupancy, over a partial crawl space
and partial unfinished basement. Foundation walls are poured concrete. Exterior walls are frame
with siding surfaces. The roof framing consists of pre -manufactured trusses at 24 inches center
to center. The roof framing is supported by the exterior walls.
I understand that the wall that divides the two currently independent bedrooms on the first floor
is to be removed and the conversion of an existing window at the rear wall of the property will
be expanded into a new 72" wide sliding glass door.
Upon review, the wall that divides the two bedrooms is configured to support the second floor
joists. Furthermore, the window to be converted into a sliding glass door is located in a wall that
supports the exterior wall above. Based on the existing configuration and the required loading
conditions which must be satisfied, a new beam and column system will need to be installed in
lieu of the wall to be removed. Additionally, a new header needs to be installed at the window in
order to accommodate the proposed sliding glass modification. Following are the specifications
of these new members:

1. Anew continuous beam, being no greater than 13'-6" in length, consisting of a 3.5" x 11 7/8"

located at the exterior wall, and at the other end is a new column at the eastern portion of
the existing doorway into the bedroom.
a. The new beam needs to be supported by a new triple pack stud column at the west
end that is buried in the existing exterior wall. The new triple pack stud column needs
to be fastened together using (1) row of 12d -16d x 3-1/4" nails at 12" o.c. each side.
The new column shall be placed directly on top of the existing bottom plate of the
existing exterior wall.
b. The east end of the beam needs to be supported by a new triple pack stud column at
that is buried in the existing midspan wall. The new triple pack stud column needs to
be fastened together using (1 ) row of 1 2d - 1 6d x 3-1/4" nails at 12" o.c. each side. The
new column shall be placed directly on top of the existing bottom plate of the existing
midspan wall.

2. At the new sliding glass door, a new header, consisting of (2) 7 Y4" x 1-1/" 2.0E LVL
members, no longer than 6'-6" needs to be installed on top of the new sliding glass door.
a. The members of the new built-up beam need to be fastened together using (2) rows
of 12d -16d x 3-1/4" nails at 12" o.c. one side.
b. Placement of construction adhesive between the two laminates shall occur prior to
fastening the two members together.
c. Each end of the new built up header needs to be supported by a new double pack
stud column. The new double pack stud column needs to be fastened together using
(1) row of 12d -16d x 3-1/4" nails at 12" o.c. each side. The new columns shall be
placed directly on top of the existing bottom plate of the existing exterior wall.
Removal of this wall will potentially require rerouting of existing utilities (e.g. electrical, HVAC,
etc.), which needs to be confirmed with the selected contractor prior to beginning work. If
removal of the interior finishes reveals an assembly that is different from standard conventional
framing, work needs to stop, and the engineer needs to be contacted prior to moving forward.
If any questions arise during construction, or conditions that differ from those that were visible
at the time of the review are encountered, the engineer needs to be contacted immediately
prior to continuing work. All dimensions are approximate and need to be confirmed prior to the
start of work.
This review was limited to the present condition of the visible items discussed above and is not
a prediction of the future behavior of the building components. This report is for your exclusive
use. No guarantee or warranty explicit or implicit is offered as part of this review.
In addition to this report, any verbal communication with the engineer is also a vital part of this
review and is information that you should consider.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection.

All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to stand-
ard practices. All interior damage to buildings is excluded 90 days after completion date and the owner is notified
that any damage to contents will be the responsibility of the owner and his insurance company. All agreements
contingent upon strikes, accidents or delays beyond our control. Owner to carry fire, tornado and other necessary
insurance. Scheduling will be done based upon date signed but limited to availability of materials, crews and weather
conditions.
ACCEPTANCE OF PROPOSAL: THIS PROPOSAL BECOMES A LEGAL BINDING CONTRACT 72 HOURS FROM DATE OF
ACCEPTANCE. Cancellation of this contract can be made with written notice within 72 hours of date of acceptance
and will allow the property owner to rescind the contract for roofing services and obtain a full refund of any deposit.
The property owner may also rescind this contract within 72 hours of notice from the insurance company for a claim
that is denied in whole or part if the work being contracted for is to be funded from the proceeds of a property and
casualty insurance claim. Interest at a rate of 1-1/2% per month (18% per annum) will be added on to all accounts 30
days past due. In addition, costs and reasonable attorney's fees for collections may be charged on any account past
due over 30 days. Payment will be made as outlined above. Failure to pay can constitute a mechanics lien filed
against property. Cancellation of the contract outside of the 72 hour right of rescission period may be achieved by
paying a cancellation fee in the amount of 5% of the contract price. All returned checks are subject to a returned
check charge fee.
