Both the front and back of this permit are required to be posted on the job site at all
times. If the complete permit is not present, inspections WILL NOT be performed.
Work shall comply with 2012 IRC & 2017 NEC. Per IRC Sec. R314, smoke detectors are required
to be installed in every sleeping room, in hallways outside of sleeping rooms, and on every
level of the structure.

General Notes:
(1) All dimensions and slopes shall be verified by the contractor prior to construction.
(2) Helical piers shall be ICC -ES certified 2 7/8" dia (or equiv).
(3) Minimum required allowable axial load capacity = 24 k per pier, min pier depth = 24',
or to refusal. Contractor shall ensure each pier achieves minimum required capacity.

Subject: Construction Observation for Foundation repairs at 3745 Garland
St., Wheat Ridge, CO.
Ref: March 3, 2018 Letter and Engineering Drawings
Dear Mr. Farland,
Per your request, I visited the home at the above referenced address
on March 16, 2018. The purpose of my visit was to observe the
foundation repairs.
The stem wall foundation at the south end of the house was exposed
during the repair and observed to have what appeared to be an
exterior concrete sister wall along part of the wall. It also appeared this
part of the foundation had been piered in the past.
The eight (8) helical piers were advanced to a depth of 20 -feet before
achieving refusal.
In my opinion, the piers appeared to have been installed in general
conformance with the requirements set forth in the engineering
documents.

Other Items (Beyond the scope of the structural repairs)
1) The interior floor system above the crawlspace area should be
monitored for additional movement. It may become necessary to
remove and replace some or all of the column supports in the
crawlspace.
2) Cracks in the brick may be repaired by a qualified masonry
contactor.
3) If applicable, racked doors and windows should be reset, and
cosmetic repairs can should be made three (3) months after the
structural repairs are complete.
4) It is very important to maintain proper drainage away from the
garage and house. I do not recommend irrigating within 5 -feet of
the foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home,
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, if they exist, should
function properly.
I observed the structural repairs to the foundation locations described
above. Other unknown concerns may also exist. The Lepetsos
Group, Inc. makes no representations regarding the suitability of other
areas, including, but not limited to mechanical, electrical, plumbing,
previous foundation repairs, and other systems. The homeowner
should understand additional unforeseen and unforeseeable issues
related to soil heaving and or settlement, and exposure to unwanted
water may cause additional foundation movement in the future. My
evaluation and recommendations are based on my education,
experience and observations made on the date of my visit. The
Lepetsos Group, Inc. makes no representations regarding the ability of
the property to meet current or past building code requirements.
Please note it was not possible to observe every structural detail.

based on my education, experience and observations made on the
date of my visit. The Lepetsos Group, Inc. makes no representations

code requirements. Please note it was not possible to observe every

Some of the interior floor supports observed in the crawlspace at the
northeast corner of the house were constructed with wood columns on
CMU block. Other supports were constructed with concrete footings
and concrete pedestals. These elements were located near the short
foundation wall on the west side of the crawlspace. The soil near the
footings was subsided.
Recommendations
1) I recommend adding eight (8) helical piers to the northeast and
southeast corners of the house per the attached engineering
drawings.
2) The interior floor system should be monitored for additional
movement. It may become necessary to remove and replace some
or all of the column supports in the crawlspace.
3) Cracks in the brick may be repaired by a qualified masonry
contactor.
4) If applicable, racked doors and windows should be reset, and
cosmetic repairs can should be made three (3) months after the
structural repairs are complete.
5) It is very important to maintain proper drainage away from the
garage and house. I do not recommend irrigating within 5 -feet of
the foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home,
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, if they exist, should
function properly.
I observed the structural condition of the visible portions of the
foundation described above. Other unknown concerns may also exist.

Per your request, I visited the home at the above referenced address
on February 23, 2018. The purposes of my visit were to observe the
foundation damage, and provide repair recommendations as required.
Please note: My investigation was based on observation of accessible,
visible items. Issues/concerns concealed by finishes, if any exist, were
not able to be observed, and were therefore not evaluated as part of
this investigation. Issues and repairs, if required, regarding other parts
of the home were not part of the scope of this investigation.
Observations
For purposes of this report, the front of the home faced east. The two
single brick house had experienced an addition in the past. It was
possible a basement on the north side was excavated as part of the
addition.
The northeast and southwest exterior corners (front corners) of the
house were cracked and settled.

1. This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the
State of Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable
ordinances of the City.
2. This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at the discretion of the Building Official.
3. If this permit expires, a new permit may be acquired for a fee of one -half the amount normally required, provided no
changes have been or will be made in the original plans and specifications and any suspension or abandonment has not
exceeded one (1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall
be paid for a new permit.
4. No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
5. Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive
written approval on inspection card before proceeding with successive phases of the job.
6. The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor
an approval of, any violation of the provisions of the building codes or any other ordinance, law, rule or regulation.
