Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Offcial and is not guaranteed.
4. No work of an manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

ROOFING SYSTEM DESCRIPTION & PROPOSAL
B & M Roofing proposes to do the following work at the above referenced property:
SURFACE PREPARATION
• Remove and dispose ofone layer ofcxisting roofing material.
If additional layers of existing roofing material are found an added labor cost for removal will be necessary.
• Inspect plywood roof deck and re-secure any loose sheets
• Replace rotten/weter damaged plywood after approval by Owner. if this work is required, a separate time
and material price of $65.00 per sheet will be added.
• Detach and reattach de-icing cable.
UNDERLAYMENT
• Instal[ two courses ice and water shield membrane at the eaves.
• Install one layer of 30# felt or synthetic underlayment over entire roofsurface.
• install Tarco valley flash in valley areas, closed style. Install'oped valleys with metal valley liner with
some systems as necessary.
FLASHINGS
• Install new 2"x 4" prefinislied drip edge flashing at the entire perimeter edge of the roof.
• Install new step flashings along sidewall areas.
• Install nav head wall flashings where roof slope meets a vertical wall, as necessary.
• Install and paint new vent flashings on pipe penetrations.
• Install new pre-painted attic vents to facilitate proper attidroof ventilation. Re-uso Power attic vent.
• Replace gutter/downspout per insurance scope. ADD 00.00 to thesyat/ m price belowO

ROOFING SYSTEM
• Install new HERITAGE DIMENSIONAL SHINGLE,
as manufactured by TAhIKO, over underlayment, including all starter, hip and ridge.
We may be able to install a Class 4 H2 (Impact Rated) at no added cost.
• Color and manufacturer will be chosen by Owner, prior to material order by B&M Roofing.
• All shingles to be installed with six nails per shingle.
TAXES AND MISCELLANEOUS
• Debris caused by the roofing operation will be properly disposed ofby B&M Roofing.
• Liability insurance from Continental Western Insurance is carried by our firm for your protection.
• Workers compensation insurance is carried by our firm on all of our employees.
• Applicable sales tax on materials included in our System Price on this project
• The applicable city or county building permit fee is included in our System Price, unless it is being reimburse
by insurance. Permit costs are additional and will be billed separately after all costs are in.
• We will complete the roofing work utilizing the driveway to access the property. A roll-ofPgarbage
container, or trailer will be located in the driveway during the re-roofing project.
Clean roofing debris out of the gutters after the re-roofing process is complete.
• We will provide a 5-year B&M Roofing of Colorado, Inc.'s roofing guarantee on labor upon completion
of the work and receipt of final payment.
• A standard 30yr manufaclurees warranty covering material will be provided upon completion of the
work and receipt of payment
• Please be aware that while working around skylights, some dry-wall dust may be disturbed, and protection fo
property underneath of the skylights is recommended.
• Among other things, painting, carpentry, and soffit venting are excluded from System Price on this project.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
