Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

>k By signing this agreement the homeowner authorizes Beall's Roofing LLC (BRL) to represent the homeowner's best
interest in pursuit of roof replacement due to storm damage. This contract does not obligate the homeowner or BRL until the
claim is approved by the insurance company. Upon approval of the claim, BRL will complete the insurance approved work
at a price agreeable to the insurer and BRL whether the price is higher or lower than our agreed pripe below. The
homeowner will not be charged any additional amount other than the insurance deductible unless the homeowner requests
additional work that was not approved by the insurer to be completed. BRL does not charge a cancellation penalty, but does
charge on a per hour basis of $150 if this contract is canceled after planning for the project has began by BRL. BRL shall
hold in trust any payment froDithe property owner until BRL has delivered materials at the property.

We agree, by signing above, to all the contract conditions on both the front and back of this agreement.
ADDITIONAL PROVISIONS
1. All proposals subject to approval by Beall's Roofing LLC-
) Price of contract is valid for 30 days based on monthly material price increases.
3. The company reserves the right to perform the specified repairs/replacements in accordance
with the generally accepted roofing and construction guidelines adhering to local codes at the
time.
4. All excess materials remain property of Beall's Roofing LLC.
5. Homeowner is responsible for Homeowners Association approval of roofing products and
colors.
6. Past due accounts are defined when all work is completed by Beall's Roofing LLC as defined `
by this contract. Insurance claim has been completed by release of final check and 30 days has
passed, earn interest at 10% per annum (per month) and in addition if Beall's Roofing LLC is
required to turn the matter to an attorney forcollection, the homeowner also agrees to pay all -
costs incurred including reasonable attorney fees.
7. Should defaults be made in payments of this contract. Beall's Roofing LLC reserves the right to
place a mechanic's lien on the contract property.
8. The company shall not be liable for failure of performance due to labor controversies, strike,
fire, hail, weather, liability to obtain material from usual sources, or any other circumstances
beyond the control of the company weather of similar or dissimilar nature.
9. The company is not responsible for any leaks or water damage below the roof line including
excessive wind, ice, or hail during the warranty.
10. Should default be made in payment, a mechanic's lien will be placed upon the property.
11. The company will not be held responsible for slight scratching or denting of gutters, oil droplets
in the driveway, cracked concrete, or damage to plants or trees. Furthermore, the company will
not be held responsible for exterior damages such as broken or fallen crown molding, or stucco,
deteriorated solar panels , evaporative coolers and existing skylights or any interior damages
such as falling dust or debris as a result or re -roofing cracked plaster, sheet rock or nail pops.
12. If material has to be reordered or restocked because of a cancellation by the customer, there will
be a restocking fee equal to twenty percent (20%) of the contract price.
13. Replacement or repair of the deteriorated decking/fascia boards, swamp cool'prs, ventilators..
chimneys, counter flashing or any other work completed for homeowner not stated in the
contract, are not included and will be charged as an extra on a time and material basis.
14. Beall's Roofing LLC reserves the right first (1) and second (2) right to correction or repairs
with ample time for correction. Beall's Roofing LLC will not be responsible financially if
homeowner calls a private contractor.
15. Payment schedule as follows unless other arrangements are made in writing: Deductible owed
at the time this contract is signed. First insurance check due at the time materials are delivered.
Second insurance check paid within 30 days of issuance from insurance company.
16. Homeowner has the right to rescind this contract and receive full refund of any monies paid to
Beall's Roofing LLC within 72 hours of entering this agreement. If homeowner chooses to
rescind this agreement after the 72 hour period, the homeowner is obligated to pay Beall's
Roofing $150/hour for time invested into the planning of the contracted construction project.
17. If homeowner chooses to use money paid by the insurance company, Beall's Roofing LLC
cannot pay, waive, rebate or promise to pay, waive or rebate all or part of any deductible
applicable to the claim.
18. Beall's Roofing LLC currently has liability insurance coverage from AFI (3900 W 38" Ave.
Denver, Co 80211 (303) 433-8888.
