Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A=5 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturers€—s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

between 7:30am and 8:00am, the morning of the inspection. Please put the address of the inspection in the subject line.
You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after
entering the contract. If you plan to use the proceeds of a property and casualty insurance policy to pay for
the roofing work, you may rescind this contract within 72 hours after you receive written notice from the
property and casualty insurer that your claim has been denied in whole or in part. However, contractor is
entitled to retain payments or deposits to compensate contractor for roofing work actually performed in a
workmanlike manner consistent with standard roofing industry practices.
I/we agree to retain Crown Roofing & Exteriors, Inc. to replace and/or repair the roof with like material on the
property listed on the front of this contract, contingent upon obtaining my insurance company's approval. I
understand and agree that Crown Roofing & Exteriors, Inc. is to contact my insurance carrier and meet with
it's representative in order to discuss the repair or replacement work to be done. Upon agreement between
my insurance carrier and Crown Roofing & Exteriors, Inc. as to the extent of the damages and the cost to fix
them, Crown Roofing & Exteriors, Inc. may begin to work on said property. 1 agree to provide all necessary
documentation for the claim (including all pertinent documentation to facilitate the payment from my insur-
ance carrier/mortgage company). I agree and understand that Crown Roofing & Exteriors, Inc. reserves the
right to file for a supplemental claim due to material or labor increases, incorrect measurements, or for any
standard cost that is relative and reasonable to the industry standards at the time.
It is hereby understood and agreed: Homeowner shall furnish any required electricity; all surplus materials
not used shall remain as the property of Crown Roofing & Exteriors, Inc.; homeowner understands that
roofing work can cause vibrations to the interior walls and sheetrock of the property and homeowner shall
remove any items from the property walls and secure lighting fixtures prior to work in order to eliminate
damages as the contractor shall not be liable for such damage(s). Any alteration or deviation from the above
specifications involving extra costs will be executed only upon written work orders and will become an extra
charge over and above the estimate and incorporated into this agreement.
All labor and material is guaranteed to be as specified. All work is to be completed in a workmanlike manner
according to industry standards and practices. One year Labor Warranty covers roof only and is limited to
roof leaks and replacement of missing shingles, but does not include repair or reimbursement for ice
damming or water damage to the interior of the property. Contractor is not responsible for mold or mildew,
damage to concrete or driveways. Contractor is not responsible for making any aesthetic changes to the
property or surrounding areas as a result of the completed roofing work. Contractor will provide
homeowner(s) with copies of all manufacturers material warranty documents. Failure of payment by the
homeowner(s) of the contract price shall void contractor's one year labor warranty and material warranties.
Upon substantial completion of the roofing work final payment will be due. It not paid within 30 days a
finance charge of 1.5% per month will be charged on my account balance not paid in full. I agree to pay all
expenses of collection, including reasonable attorney fees and court costs and $50.00 for any check
returned from my bank unpaid.
If any provision(s) of this agreement should be held to be invalid or unenforceable, the validity and
enforceability of the remaining provisions of this agreement shall not be affected thereby.
This contract shall become binding only upon written acceptance hereof by the contractor or it's authorized
Agent of the company or upon commencement of the work.
By signing below I acknowledge that I have read the above agreement and that I understand all of the provi-
sions that are outlined in the agreement and agree to the terms as written. I further acknowledge that I have

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 24-48 hours, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Existing permits that require amendments (for example, to add
redecking) must be completed in person at City Hall. All other non -roofing permits
must be completed in person at City Hall. The Building Division will be open from
7:30-10:30 a.m., Monday through Friday to process these types of requests.
