Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

1. This permit was issued based on the informati n provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original ermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

ADDITIONAL TERMS AND CONDITIONS
I. Complete Agreement. All the clients listed on the face of the contract must sign. This contract is subject to management approval. This
contract constitutes our entire agreement, and there are no other promises, conditions, representations, warranties (including implied
warranties of fitness or suitability) except as stated herein. This contract shall not be modified except in writing. If you want us to perform
additional work outside the scope of work, including any alteration or deviation from the contract specifications, or in connection with any
defects or problems discovered during the course of the job, then we will require a written change order including a reasonable charge for the
extra work.
2. Payment: We will collect the insurance actual cash value (ACV) before we replace your roof, which you receive in trust for our benefit
under the contract. When the work has been fully performed, we are entitled to immediate payment in full for the balance of the Final
Replacement Cost Value (RCV) contract price. Insurance proceeds included in the terms of this contract and depreciation needs to be
released from the insurer after completion of work. Therefore, final payment must be made within 30 days after the roof installation. In the
event of a default in payment, interest shall accrue from the date of default at the rate of 1.5% per month compounded monthly, and you will
be obligated to pay our reasonable attorney fees and costs of collection. We may place a lien on your property in the event of default in
payment.
3. Safety: You must exercise special care for the safety of yourself and others. The following are examples. You shall not allow anyone to
walk under the work area while roof work is in progress, as falling debris is a hazard in the work area. Roofing work can cause dust, debris,
and small particles from the roof installation to enter the building and attic. Therefore you must use caution the first time the attic access is
used and must not look up the access hatch immediately after opening. We will use a magnetic sweeper and visibly pick up nails upon
completion as permitted by snow and other conditions; however, it is impossible to retrieve every nail. Therefore you must use caution to
avoid injury and property loss and you waive, and release us from, any liability for injury or property loss caused by nails and any other
possible causes of loss which you might avoid by exercise of reasonable care.
4. Scope of Work: Except as indicated specifically on the front of this contract, the following work is not included in this contract:
replacement of roof jacks, ventilators, decking, fascia boards, flashing, gutters or other material not in the Scope of Work. We will not
perform work for which we are not licensed. Therefore, for example, we will not wire electrical accessories because our roofers are not
licensed electricians, nor will we evaluate or replace vents or other components requiring special licensing.
5. Pre-existing and Hidden Conditions: We shall not be obligated to correct or repair pre-existing structural deficiencies or other hidden or
defective conditions including but not limited to problems related to the design or condition of venting, framing, rafters, skylights, flashing
and foundations, irregularities in the underlying roofing system, or problems resulting from such conditions, or the work of others. The
installation of a new roof may intensify the appearance of any deficiencies. If a new nailable surface is required by code and you do not
have ordinance and law provisions on the insurance policy to cover the expense, you will be liable to cover the expense of the new decking.
6. Risks to Property: Building codes require roofing nails to penetrate into decking approximately 3/4". If the other trade services are installed
according to code, all wires, Freon lines, gas pipes and water pipes will not be punctured. Should damage occur to any wires, pipes,
components, and/or personal property as a result of nail punctures, we shall not liable. We suggest you examine your attic before work
begins to ensure these items are not in contact with, or close to the decking. Heating units, hot water heaters and stove vents in the attic
could be dislodged and should be checked after roof installation.
7. Protection of Property: During installation of a new roof, vibrations will most likely be felt throughout the house. You are responsible for
protecting objects hung from interior and exterior walls, and from ceilings and soffits. Driveway access is required for material delivery and
workstations. We will not be held responsible for damage to driveways or curbs from heavy materials and equipment due to normal delivery
procedures and work, or the negligence of independent contractors or others. We shall not be expected to keep gates and doors closed.
Should flashing need replacement, siding adjacent to flashing that has deteriorated may break. We will make every effort to avoid damage
to siding, but will not assume responsibility for any consequential damage to siding.
8. Limited Warranty: You are being provided with a Limited Warranty in a separate document which is part of this contract. We warrant,
subject to the terms and conditions set forth in the Limited Warranty, that the work shall be done in a good and workmanlike manner; that
we will repair roof leaks resulting from defects in workmanship or materials furnished by us occurring within 2 years from the date of
completion on 4.12 or steeper pitch roofs. (Flat roofs have a separate limited warranty and this warranty does not apply to them.) Any
warranty is limited to repairs on roof system we install. We are not liable for any damage to the customer's premises or any contents
thereof, including the roof decking, fascia, rafters, and interior drywall. The warranty excludes any environmental conditions caused by
moisture, leaks, other water intrusions or any contaminant. We are not liable for ice dams; icicles or any damage resulting from these issues.
With regard to completed repair work, we shall have no warranty liability except for the roof areas upon which work was performed
according to this contract. This warranty is not assignable or transferable, unless noted in writing by management. Materials are warranted
as specified by the manufacturer. We shall not be liable for any color or shading variation between shingles and shingle samples.
9. Shared Responsibilities: You agree to read and follow the recommendations contained in the instructions we provide in the documents
entitled, Shared Responsibilities and Homeowner's Checklist, which are part of this contract.
10. Notice: You notify us in writing of any alleged defect within the work performed within seven days of the completion of roof system. You
must notify us within three days of the discovery of a leak. It is your responsibility to take reasonable and immediate action to avoid further
damage. We will send a representative to inspect and investigate any reported leak within 30 days of notification to us; usually we are able
to do repairs within a couple of days. More than one inspection may be necessary to isolate the cause.
11. Weather and Other Risks: We shall not be held responsible for any leaks or other problems caused by lightening, hurricane, tomado, hail,
windstorm, and other unusual movement or settlement, deflection, nails, staples, or other fasteners, or damage due to any roof surface
equipment or traffic of any nature on the roof, or acts of God, war, labor strikes, unavailability of materials, mold, or caused by fires, floods
or other hazards typically covered by homeowner's or builder's risk insurance. We shall not be responsible for incidental, indirect or special
or consequential damages, including but not limited to personal injury to any person as a result of our performance, nonperformance or
termination of this contract, whatever the cause, nor for any reason in an amount in excess of the amount paid to us.
12. Restocking Fees: If you order materials which must be restocked or reordered due to your cancellation, the supplier may charge us a
restocking fee. If that happens, you will pay the restocking fee.
13. Insurance: We maintain automobile, general liability and workers compensation insurance through Western Group Inc. Coverage can be
verified by calling (719) 448-9609, and upon your request we will provide you with copies of certificates of coverage or declarations sheets.
14. Dispute Resolution: If any dispute arises concerning the validity, enforceability, interpretation, performance or breach of this contract (a
"dispute"), the parties shall in good faith attempt to resolve the dispute through confidential mediation with the Better Business Bureau of
Southern Colorado ("BBB"). If mediation is unsuccessful, then you must also comply with the Colorado Construction Defects Reform Act,
and the parties shall resolve the dispute through arbitration with the BBB according to its rules. The arbitrator shall have all rights and
powers of an arbitrator and court of general jurisdiction, and shall in particular have the power to award equitable and declaratory relief
temporary restraining orders, preliminary and permanent injunctions, orders for preservation of confidentiality and sanctions for abuse of the
arbitration process or breach of confidentiality, as well as awarding damages for money damages. Each side shall promptly advance such
deposit toward the mediator's and/or arbitrator's fee as may be required by the BBB. The arbitrator shall have discretion to allocate such
fees among or between the parties and may in his or her discretion award reasonable attorney fees and costs to the substantially prevailing
party. The arbitration award may be made a judgment or court order by any court of competent jurisdiction. The parties waive the right to
trial by jury. Prior to, during, or after the arbitration proceedings, any party may file suit in any court of competent jurisdiction only to
enforce temporary or permanent awards of the arbitrator, or to preserve the confidentiality of information or the status quo, subject to the
arbitrator's ultimate authority to determine all issues of fact and law and to render a final and binding award. In the event that the arbitrator
renders any award granting temporary or permanent legal or equitable relief of any kind, then such award shall be made a judgment, order,
or decree of any court of competent jurisdiction upon the request of any party.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Per your request, we performed limited visual observations at the above referenced residence on June
1, 2009. The purpose of this visit was to observe and evaluate the distress located throughout the
home and provide repair recommendations as needed.
The suspect home is a single family wood framed ranch structure clad intt ck veneer supported on
concrete foundation walls which are founded on spread footings. The h9me has a crawlspace area
behind the garage and a finished garden level basement north of the garage for purposes of this
report the home is assumed to face west.
Site Observations: W
During our visit, we noted the following (partial list of observations):
EXTERIOR:
1: The foundation had been excavated at the sou+ est eornerrof ~etgarage exposing a concrete, s
grade beam founded on a concrete spread footing approximately 'below grade
2. A separation up to 5/6" in width was noted between,the lnok„veneex and,the garage,dgartr
at the south side of the overhead garage door opening: " The separation was widest at the top of.t
the wall.
3. The grading at the south side of the garage was relatively flat and did not appear to provide
adequate drainage away from the home.
4. A stair step brick veneer crack up to 7/8" in width was noted at the south wall of the garage.
The foundation had been excavated in the vicinity of the brick veneer crack revealing a crack
in the foundation wall approximately 1/4" in width.
5. A stair step brick veneer crack approximately 1/4" in width was noted at the south wall of the
crawlspace.
6. A separation approximately 1/8" in width was noted between the brick veneer and the door
trim at the upper east corner of the sliding glass door above the north wall of the crawlspace.
7. A separation up to 1/2" in width was noted between the brick veneer and the garage door trim
at the north side of the overhead garage door opening. The separation was widest at the top of
the wall
8. A separation up to i/4" in width was noted between. the brick veneer and the, door trim at the
upper south corner of the front door. The separation was widest at the top of the walla

9. Distress was noted at the east wall of the mud room, south wall of the dining room, east and
west walls of the living room and in the ceiling of the living room adjacent to the fireplace.

BASEMENT:
11. Distress was noted at the south wall of the northwest bedroom, the north wall of the furnace
room, and the west and north walls of the great room in the basement.
12. The interior partition walls are not isolated from the basement slab.
Conclusions / Recommendations:
After performing a structural review of the observed conditions, we have concluded the following:
The majority of the distress noted at the exterior of the home and on the main level of the
home is due to settlement of the footings supporting the garage and crawlspace foundation
walls as well as the footings supporting the south half of the west wall of the basement. We
recommend fourteen (14) helical piers with steel mounting brackets be installed to support the
footings that have experienced settlement. The piers should be installed in the locations
shown on the attached detail SR-1. The piers should be installed as shown in the attached
detail SR-2. Once the piers and pier brackets are properly installed, the piers at the perimeter
2. The maiority of the distress noted in the basement annears to have been caused by onward
vertical movement of the basement slab. The finishes for the basement walls are not properly
isolated from the basement slab so that the movement of the slab is translating into the
basement walls causing the noted distress. The slab movement is likely related to moisture
induced swelling of the expansive soils beneath the slab. The finishes on the basement walls
and the basement wall framing should' be properly isolated from the basement slab by
3. As noted above, it has been reported that water enters the basement at the rear wall of the
home. It is our understanding that a french drain has been installed along the rear of the home
since the time of our site visit to address this issue.
4. Foundation wall cracks 1/16-inch and wider should be epoxy injected per the attached epoxy,
injection specifications.

GENERAL CONDITIONS
The Contractor shall maintain comprehensive general liability, workman's compensation, automobile
liability and/or other insurance as appropriate for the work performed which will provide protection
from any and all construction or completed operations claims. Certificates of Insurance shall be
provided to Hunter Engineering, LLC and/or the Client upon request. Hunter Engineering, LLC
reserves the right to determine the appropriate liability limits for the necessary insurance coverage;
however, the limits will not be less than the minimum mandatory amounts required by the State of
Colorado.
All construction shall be performed in accordance with all applicable codes and standards, and good
construction practices. Contractor shall obtain and pay for any and all permit fees, taxes and licenses
required by governing agencies to complete the work. Contractor shall be responsible for all labor,
materials, equipment, tools, operations and any other item necessary for proper completion of the
work.
The Contractor should visit the site prior to bid submittal to fully understand the scope of the project
and the conditions at the site. By submitting a bid for the work proposed in the attached repair
recommendations, the contractor is implying that they have fully reviewed all associated documents
and that they have a full working understanding of the work to be completed.
The Contractor shall conduct a full review of the attached repair recommendations prior to beginning
work. If during this review or at any time during the construction process the contractor notes any
errors or omissions in the recommendations, work shall be stopped and this office shall be contacted
immediately.
Recommendations and specifications contained herein are based on general assumptions of
conditions, some of which can not be observed during typical nondestructive visual observations. If
unexpected conditions are encountered during the course of work, the Contractor shall contact this
office prior to proceeding.
The Contractor shall field verify all dimensions and existing conditions prior to commencement of
work. The Contractor shall notify Hunter Engineering, LLC upon determination of any significant
variation of information provided on the attached documentation for his/her interpretation and
decision, and such decision shall be final. Information provided regarding existing conditions are
believed to be accurate, however, the Contractor shall be responsible for, and shall only use, his/her
own measurements to verify existing conditions.
The Client shall provide water and electric hook-ups for temporary use. (Contractor must pre-arrange
with Owner.)
All utility lines shall be located, marked and protected from damage throughout the performance of
the work. Contractor shall call for utility locations unless other arrangements had been made. Any
damage to utility lines caused by the Contractor shall be repaired at the Contractor's expense.

The repair recommendations prepared by Hunter Engineering, LLC make no provisions for
construction conditions that may arise during the performance of the work. The Contractor shall be
responsible for maintaining the stability and integrity of existing structures (bracing, shoring,
benching, etc.).
The Contractor shall be responsible for the acts or omissions of his/her agents and subcontractors in
the performance of their work. The Contractor shall be responsible for the means, methods,
techniques, and sequences of construction.

Unless noted otherwise, no substitutions or deviations from the repair plan and specifications are
permitted without the express approval of Hunter Engineering, LLC. Unauthorized modifications to
the repair recommendations and specifications may deem the work unacceptable.
All unacceptable work shall be promptly corrected at the Contractor's expense under the direction of
Hunter Engineering, LLC.
The Contractor shall maintain a safe jobsite. The Contractor is to stack all necessary equipment and
materials in a low traffic area or in an area that will insure the safety of all individuals potentially
present at this site during construction. OSHA standards shall be used as a guideline.
The Contractor shall keep the jobsite thoroughly clean at the end of each day. All surplus materials
shall be safely stacked in a low traffic area or hauled away from the site.
The Contractor agrees to perform the work in a diligent manner and in full conformance with the
repair plan.
If a condition arises during the repair process which requires a change in the scope of work, Hunter
Engineering, LLC may addendums or modifications to the recommendations which may add, delete
or modify the scope of repair. Performance of any extra work, or payment for same, shall only be
considered upon prior written authorization from the Client.
LIMITATIONS
These repair recommendations do not represent a complete analysis of the entire structure. It
addresses only those conditions observed by Hunter Engineering, LLC during our site visits.
The interpretations of the distress to be repaired contained in the attached repair recommendations are
based on the visual observations of the structure. No detailed study was made of the structural
elements of any part of the building. Therefore, no opinion is presented or shall be implied
concerning the structural integrity of those elements not subject to visual inspection. Investigation
and analysis of other portions of the structure are beyond the scope of these repair recommendations
at this time.

Hunter Engineering, LLC will not be responsible for 1) knowledge of subsurface conditions away
from the existing borings, if any, without extensive geotechnical information supplied by a competent
geotechnical engineer, 2) knowledge of distresses, or differential displacements that have occurred in
an area which had not been exposed for visual observation during our site visit and 3) any other
elements such as joists or beams and other structural members that are not readily visible.
Hunter Engineering, LLC assumes that all information provided by the Client, Contractor, or other
Consultants is correct, accurate, and may be acted upon accordingly, and shall have no liability for
the validity of information provided.
Hunter Engineering, LLC shall have no liability for acts or omissions of the Contractor or any other
agents performing the work on this project or for the failure of the Contractor to carry out the work in
accordance with the plans and specifications if not inspected as intended. Hunter Engineering, LLC
shall not be responsible for the construction means, methods, techniques, sequence, procedures, or
safety precautions related to this project and its execution that is under the control or direction of the
Contractor.
The repair recommendations included herein shall be used with the accompanying drawings and
specifications. Do not rely on one or the other, all materials contained in the repair recommendations
including but not limited to the cover letter, notes, plans, details, shall be used together to perform the
proposed remedial construction.
SPECIFICATIONS
Helical Piers
1. Piers, brackets, and bolts shall be manufactured by a known replicable manufacturer and
shall be hot dip galvanized and installed per manufactures recommendations.
2. Contractor shall submit manufactures product specifications and installation
recommendations for their proposed pier system to this office for approval, prior to pier
installation. The pier and bracket proposed by the contractor shall meet or exceed
recommendations provided in the attached plans and details.
Epoxy Iniection
1. Concrete crack repair includes the sealing and healing of surface and internal cracks 1/16 inch or
wider.
2. Use epoxy paste sealer to install injection port devices and to caulk and seal cracks. Epoxy paste
sealer shall be Sika Sikadur 33 or Sika Sikadur 31 hi-mod or approved equal.
3. Space metal or plastic injection port tubes, or other approved injection port devices,
approximately twelve (12) inches on center. As required, decrease port spacing to insure
maximum penetration of the injected epoxy adhesive. Increase port spacing only with the
engineer's approval.
4. Epoxy injection adhesive shall be a 2-component, solvent free, moisture-insensitive epoxy
adhesive. Sika Sikadur 52 or Sika Sikadur 35 hi-mod LV or approved equal.

5. Once injection has been started on a crack, it shall be continued uninterrupted until the crack has
been completely injected. Sufficient materials shall be immediately available at the job site before
starting injection.
6. Start injection at the lowest elevation point along a vertical crack and at an end of a horizontal
crack. Continue injection until epoxy appears at the next consecutive port.
7. Cap or seal the injection port and begin injection at the next consecutive port which has not been
capped or sealed. Continue in this manner until the crack is completely injected.
8. Upon completion of the crack injection process, all excess epoxy sealer and injection port devices
shall be ground smooth and flush with the adjacent concrete surfaces.
COMPLIANCE OBSERVATIONS
Compliance observations are not intended to be an exhaustive check or detailed inspection of the
Contractors work but rather are to allow a representative of this office, as an experienced
professional, to become generally familiar with the work in progress and to determine, in general, if
the work is proceeding in accordance with the notes, plans and details provided by this office.
Hunter Engineering, LLC shall be provided with access to all parts of the work and shall be furnished
with such information and assistance by the Contractor as is required to make a complete review of
completed work.
In cases where the Contractor will be using materials differing, in any way, from those materials
specified in the specifications section of this report, the Contractor shall submit all pertinent product
documentation pertaining to the material to be used as a substitute to this office for review and
approval prior to beginning any work. Such information should include material specifications,
design aids, and recommendations for installation and/or usage.
Hunter Engineering, LLC shall determine the quality and acceptability of materials furnished and
work performed. Hunter Engineering, LLC will notify the Contractor of any failure of the work or
materials that do not conform to the requirements set forth in the plans and specifications and reject
such non-conforming materials in question.
If Hunter Engineering, LLC finds the materials famished, work performed or the finished product not
in conformity with the plans and specifications, the affected work or materials shall be removed and
replaced or otherwise corrected by and at the expense of the Contractor in accordance with the
written orders of this office.
Recommended Compliance Observations
The following compliance observations should be completed by a representative of this office.
Hunter Engineering, LLC shall not be held liable for the Contractors interpretation of the notes or
detail, or the performance of the repairs if a representative of this office does not perform the
following compliance observations. Please provide at least 24 hours notice when scheduling
compliance observations.
