2. This.permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180, days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address)_ has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Terms for insurance work only: This agreement does not obligate the property owner or
Suburban Roofing and Exteriors, LLC to perform work unless it is approved by your
Insurance Company and accepted by Suburban Roofing and Exteriors, LLC. By signing
this agreement you authorize Suburban Roofing and Exteriors, LLC to work directly with
your Insurance Company and complete the work as specified on the Insurance Loss
Sheet at Replacement Cost Value (RCV) without any cost to you except your insurance
deductible. Supplemental claims billed by Suburban Roofing and Exteriors, LLC on your
behalf and approved by your Insurance Company for additional work or cost increases
will become part of this agreement. Any upgrades or additional work requested by you
and not approved by your Insurance Company will be your financial responsibility. By
signing this Agreement, you, the Property Owner acknowledges Suburban Roofing and
Exteriors, LLC as a General Contractor and as such will be entitled to ten percent
overhead and ten percent profit as allowed by insurance industry standards

the terms included on the front and back of this agreement.
THE CONTRACT ON THE FACE HEREOF AND ANY AGREEMENT MADE PURSUANT THERETO BETWEEN SUBURBAN ROOFING AND
EXTERIORS, LLC AND THE HOMEOWNER WILL BE SUBJECT TO ALL APPROPRIATE LAWS, REGULATIONS AND ORDINANCES' AND TO THE
FOLLOWING SPECIAL TERMS AND CONDITIONS. 1. It is agreed that the terms., 'loss sheet", "scope of loss", "insurance estimate" and "insurance
company estimate" are one in the same and represent the paperwork from the insurance company showing an itemized or un -itemized list of
prices of the damages that will be covered by the insurance company. 2. It is agreed that the amount of the contract shall be the amount equal
to the replacement cost value as stated on the Homeowners Insurance company estimate including the amount and all supplements, upgrades
and extras, unless otherwise noted. 3. Homeowner agrees to provide Suburban Roofing with a copy of all insurance company estimates within
five business days of receipt from insurance company in order to facilitate settlement 4. Suburban Roofing has the right to order excess
material. These materials will not be charged above the agreed upon price. All excess material will remain the property of Suburban Roofing.
Suburban Roofing has the right to supplement the Insurance Company's estimate in the event of material or labor increases or omissions or
accuracies on the insurance estimate. Additional supplemental payments paid by the Insurance company for additional labor and materials
needed beyond the original scope of repairs are due and shall be paid directly to Suburban Roofing. 6. All material is guaranteed to be as
specified; or we may substitute similar materials if specified materials are unavailable. Some variation may occur in color, texture, and planes
of materials. All work to be completed in a workmanlike manner according to standard practices. Any alterations or deviation from above
specifications involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate.
7. Suburban Roofing shall not be liable for failure of performances due to labor controversies, strikes, weather, and inabilityto obtain materials
from usual sources, or any other circumstances beyond the control of Suburban Roofing whether of similar or dissimilar nature. 8. Drywall
nail heads may pop and/or drywall seams may crack from vibration of the walls and/or ceiling during roof or siding installation, although
proper care is taken to minimize any problems, Suburban Roofing does not accept any responsibility should this occur. Suburban Roofing is
not responsible for cracked, broken, or loose brick, masonry etc. or the water tightness of the masonry areas. Masonry walls and chimneys
should have proper flashings installed. The condition of the brick, stone, stucco, concrete and mortar needs to be maintained. 9. If you chose
not to have the flashing installed at this time, Suburban Roofing does not assume responsibility for leaks due to the existing flashing. Existing
masonry must be suitable for flashing. 10. Our performance may require the installation and removal of dumpsters, delivery trucks, worker's
vehicles, construction equipment, and vehicles onto your property. These events may cause damage to your property due to weight or
movement, damage from which you hold us harmless. You grant free access to work areas for workers and vehicles and allow storage of
materials and rubbish. You agree to keep driveways clear and available for movement and parking of vehicles during normal working hours,
including removal of ice and snow. We are not expected to keep gates and doors closed and you hold us harmless from claims arising thereof.
11. Skylights: The condition of the skylight itself and the seal of the insulated glass need to be maintained in order for the skylight to remain
water -tight. Suburban Roofing is not responsible for the condition of existing skylights. 12. Replacement of deteriorated decking, roof jacks,
ventilators, flashing or other materials, unless otherwise stated in this contract, any defective rafters, sheathing fascia or sub -fascia necessary
to perform proper roof, siding, window or gutter installation are not included and will be charged as an extra on a time and material basis on
a change order. 13. Suburban Roofing is not responsible for any damage below the roof line due to leaks by excessive wind, ice dams, hail, or
preexisting construction defects during the period of warranty. 14. All proposals are subject to approval of our managers. Suburban Roofing
is allowed to research the credit history of their customers. 15. Limited to the percent, allowable by law, a monthly service charge will be
added to any outstanding balance not paid within thirty (30) days of when invoiced. You agree to pay our reasonable costs to enforce the terms
of this Agreement. Such costs shall include attorneys' fees, costs, and disbursements incurred in pursuing collection, with or without a court
action. The final payment in full shall not be held up while waiting for the city, mortgage company or material manufacturer to inspect the
work. Suburban Roofing agrees to meet all city and state building code requirements. 16. Suburban Roofing will provide the homeowner with
a lifetime warranty covering defects in workmanship on all residential roofing and siding work Suburban Roofing will provide a five-year
workmanship warranty on all other trades. You agree to look solely to product manufacturer's warranties as exclusive coverage for any
product defects. This warranty excludes any claims of consequential damages. The contract and warranty shall not be assigned and are
nonrefundable. In order for any warranties in addition to those mandated by the State of Colorado to be valid, the contract must be paid in full.
17. You hold us harmless from pre-existing construction conditions that may be manifested during the construction process. All repairs needed
due to pre-existing conditions discovered during the construction process are the sole responsibility of the homeowner. Performance under
this contract is contingent upon strikes, accidents or other delays beyond our control. The price of this Contract does not include material or
labor price increases because of unforeseen problems arising after work has begun, market supply shortages or unusual spikes in market
demand. This Contract price does not include any governmental permit, service or access charge. Existing out of square and plumb conditions
may require similar conditions in the new world. You agree to remove and protect any personal property in or near the work area, including
without limitation, shrubs, flowers, wall hangings and other valuables and hold us harmless from damage resulting from failing to so secure.
We are not responsible for hazardous material (lead paint, asbestos, etc.) removal from your existing structure and may stop work until it is
removed. 18. Any representation, statements or other communications not written in this contract are agreed to be immaterial, and not relied
on by either party, and do not survive the execution of this contract. If any provision of this agreement should be held to be invalid or
unenforceable, the validity and enforceability of the remaining provisions of this agreement shall not be affected hereby. This Agreement
constitutes the entire agreement between the parties. It may be changed only by written instrument signed by both parties. 19. THE LAW
REQUIRES THAT THE CONTRACTOR SHALL SUBMIT A SWORN STATEMENT OF PERSONS FURNISHING LABOR & SERVICES, MATERIAL,
FIXTURES, APPARATUS OR MACHINERY, FORMS OR FORM WORK BEFORE ANY PAYMENTS ARE REQUIRED TO BE MADE TO THE
CONTRACTOR.
