Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A'/z-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer&Oms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalppermit fee.
3. If this permit expires, a new permit may be required to be obtained, issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Both sides of this document, including the terms and conditions below, and any agreement executed in writing, pursuant thereto, between
Johnson Construction Company LLC. (the "Company") and the property owner(s) or property owner's representative (s)
hereby referred to as the "Customer", are subject to the laws in effect in the State in which it has been signed and executed.
ANY REPRESENTATION, STATEMENT, OR OTHER COMMUNICATIONS NOT WRITTEN IN THIS AGREEMENT, OR MADE IN WRITING
PURSUANT THERETO ARE AGREED TO BE INVALID, AND NOT RELIED ON BY EITHER PARTY AND, DO NOT SURVIVE THE EXECUTION OF
THIS AGREEMENT.
1. This agreement is subject to the credit and pricing approval of the management of Johnson Construction Company LLC.
2. Payment is due upon job site delivery of materials. Payment issue for the remainder of the agreement with completion of each individual trade,
including applicable overhead & profit, such as: Roofing, Siding & Trim, Gutters, Awnings & Carports, Interior Repairs and or Contained Punch List.
In the event a final inspection is required before the "Customer" chooses to pay, the "Customer" shall agree to pay the total balance due less 10% as
hold -back, until the final inspection and any subsequent punch list is completed.
3. Should default be made in payment of this agreement, a lien will be placed on the property and charges will be added from the date of restoration
completion at the maximum allowed by law. If placed in the hands of an attorney for collection, the "Customer" will pay all attorney and legal fees.
Colorado: Any person or company supplying labor or materials for this improvement to your property may tile a lien against your property if that
person or company is not paid for their contributions.
4. The "Company" and the "Customer" agree to settle all disputes in accordance with the laws of Colorado and the county in which the agreement is
executed.
5. The full amount of all money as specified by an agreed price, or as specified by "Full Scope of Insurance Proceeds" does not include any extras such
as carpentry repairs, or any other repairs not specified, and not covered by the "Full Scope of Insurance Proceeds", that is necessary to complete the
repair process as required by local building codes, and building officials, or to satisfy cosmetically, the "Customer".
6. If the "Customer" chooses not to pay for a part of the scope of the agreement, an individual line item or a trade, the "Customer" releases the
"Company" of its obligations for the performance of that component with regard to the integrity of the building system as a whole.
7. The "Company" is not responsible for damage below the roof due to leaks caused by excessive wind greater than 55mph, ice damming or hail, at any
time during the construction process, or warranty period.
8. "Company" will not be responsible for soft, de -laminated, rotten or poorly installed decking below roofing material. "Customer" i s responsible for
cost of decking replacement to satisfy "Company's" workmanship warranty.
9. The "Company" will not be responsible for slight scratching and denting of gutters, oil droplets in driveways, hairline fractures in concrete, Flowers,
or minor broken branches on plants and shrubbery.
10. You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the contract. If you plan to use the
proceeds of a property and casualty insurance policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive
written notice from the property and casualty insurer that your claim has been denied in whole or in part. However, Johnson Construction
Company LLC. is entitled to retain payments or deposits to compensate Johnson Construction Company LLC. for roofing work actually performed in
a workmanlike manner consistent with standard roofing industry practices. Johnson Construction Company LLC, cannot pay, waive, rebate, or
promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on your
property.
11. This agreement cannot be canceled once negotiations begin with your insurance company, and/or material has been delivered to your job site
without written mutual agreement of both parties.
12. If this agreement is canceled by the "Customer" later than three (3) days from execution, 'Customer" agrees to pay to the "Company" 15%
restocking fees if materials have to be returned due to cancelation.
13. The "Company" reserves the right to supplement the Insurance Company for increases in the "Full Scope of Project Work", and or documented price
increases from suppliers. "Customer" agrees to allow these supplements to be paid directly to the 'Company".
14. All excess materials remain the property of the "Company".
15. "Full Scope of Insurance Proceeds" shall be defined as the total price for repairs and replacements allowed by the insurance company.
16. If any provisions of this agreement should be held invalid or unenforceable, the validity and enforceability of the remaining provisions shall not be
affected thereby.
17. We will not honor our warranty on any agreements that are not paid in full specified by "Full Scope of Insurance Proceeds". Johnson Construction
Company LLC. carries workers' compensation, automobile and commercial general liability (bodily injury and property damage) insurance. A
Certificate of Insurance identifying our insurance carrier and the types and amounts of coverage and our insurance contact information is attached
or will be provided to Customer and is incorporated herein by reference and should be considered as part of this contract.
WARRANTY: OUR STANDARD WORKMANSHIP WARRANTY IS FOR TWO YEARS. IN ADDITION, WE WILL PROVIDE YOU A WRITTEN WARRANTY
UPON RECEIPT AND VERIFICATION OF YOUR PAYMENT IN FULL AGREEMENT. COPIES ARE AVAILABLE FOR YOUR PERUSAL.

How to Use this Guide

1. Review this Building Guide — Review the requirements set forth in this guide for
submittal and compliance with applicable codes.
2. Provide 2 Floor Plans, minimum I V x 17" in size, drawn to scale, indicating room sizes
and types. Identify fumace, water heater, plumbing fixtures, electrical outlets, lights, smoke,
and carbon monoxide detectors, and windows, including size and type, and any other
pertinent information.
3. Fill out a Building Permit Application.
The majority of permit applications are processed in I to 2 weeks. The submitted documenj
