Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Roof Replacement Contractor Agreement
This Agreement is entered into as of this 31st day of July 2017 by and between DARBY
ENTERPRISES, INC., (hereinafter called the "Contractor") and Mark Bradley (hereinafter
called the "Owner")
WITNESSSETH that the Contractor and the Owner, in consideration of the covenants and
conditions set forth herein agree as follows:
Scope of Work: See ATTACHED EXHIBIT "A" which is incorporated herein by reference.
The Contractor shall furnish all materials and perform the work on the property located at:4631
Pierce St. Wheat Ridge, CO 80033
The Owner shall pay the Contractor the sum of $19,762.43 for the material and labor provided by
Contractor, subject to additions or deductions provided in any subsequent change orders, or in any
insurance supplements or requests for supplemental roofing services, payable as follows:
Approximate dates of service: It is anticipated that the work will be commenced on August 2nd, 2017,
and will be completed within 3 days, subject to changes ordered in the Work, unavoidable casualties,
unusual delays in deliveries, or other causes beyond the Contractor's control.
Property Owner is—X is not intending to make payment from the proceeds
ofproperty and casualty insurance policy.
CANCELLATION OF CONTRACT: Owner has the right to rescind this contract and obtain a full
refund of any deposit within 72 hours after entering into the Contract. If Owner plans to use the proceeds
of a property and casualty insurance policy to pay for the roofing work, Owner may rescind this contract
within 72 hours after Owner receives written notice from the property and casualty insurer that Owner's
claim for payment for roofing work has been denied in whole or in part. The Owner's right of rescission
does not apply when the property and casualty insurer denies, in whole or in part, a claim related to a
request for supplemental roofing services if the damage requiring the supplemental roofing services could
not have been reasonably foreseen as a necessary and related roofing service at the time of the initial
roofing inspection or the execution of the initial roofing contract. In order to exercise the right of
rescission, Owner is required to provide Contractor with written notice of rescission of the contract at
the Contractor's address set forth below within 72 hours after entering into the contract, OR within 72
hours after Owner is notified of the denial of the claim for payment. The notice of rescission must be given
by one of the following methods: (a) In an electronic form, which is effective on the date of the electronic
transmission; (b) By mail, which is effective upon deposit in the United States mail, postage prepaid, sent
to Contractor's address stated below; or (c) By personal delivery to Contractor, which is effective upon
delivery. Within io days after rescission, Contractor will return to Owner any payments or deposits made
by or evidence of indebtedness of the Owner in connection with the contract for roofing work. However,
Contractor is entitled to retain all or a portion of payments or deposits made by Owner to compensate
Contractor for roofing work actually performed in a workmanlike manner consistent with standard
roofing industry practices. In addition, nothing set forth herein abrogates Contractor's right to pursue
common law remedies for the reasonable value of roofing materials ordered and actually installed
pursuant to this contract prior to Owner's rescission of the contract, as long as Contractor performed the
services consistent with roofing industry standards for workmanlike performance of roofing services.
Insurance Deductibles: If Owner plans to use the proceeds of a property and casualty insurance policy
to pay for the roofing work, Contractor cannot pay, waive, rebate, or promise to pay, waive or rebate all or

part of any insurance deductible applicable to the insurance claim for payment for roofing work on
Owner's property.
Payments in Trust:
Contractor shall hold in trust any payment from Owner until Contractor has delivered
roofing materials at the property site or has performed a majority of the roofing work on
the Owner's property.
INSURANCE – Contractor shall provide and carry during the entire term of Contract its own insurance,
including general liability insurance {bodily injury and property damage} worker's compensation &
automobile insurance. Contractor agrees to maintain workman's compensation insurance and public
liability insurance in the amount of $1,000,000.00 in full force throughout the term of Contract. The

Warranties and limitation on liability: Contractor warrants to the Owner that the roofing work will
be free from defects for a period of 5 years commencing on substantial completion of the roofing work.
The Contractor's warranty excludes remedy for damage or defect caused by abuse, alterations to the
roofing work not executed by Contractor, improper or insufficient maintenance, or normal wear and tear
and normal usage. In the event of a claim of defective workmanship, the notice of the warranty claim
must be submitted in writing and must describe the claim in sufficient detail to determine the nature of
the problem(s), and must be signed by the Owner. Contractor is not a dealer in roofing materials, and

OTHER WARRANTIES, EXPRESS, IMPLIED OR STATUTORY, AND THERE ARE NO IMPLIED
WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE.
