Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180.days made be granted at the discretion of the Chief Building

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS & CONDITIONS
1. WORK. The estimate defines the Work. The Work shall be completed in a reasonable time given the circumstances. "Substantial Completion" means Owner can
reasonably occupy or use the structure for intended purposes. Unless included in the estimate, the Work does not include repairing pre-existing defects, concealed
conditions, or compliance with current building code requirements. Contractor's obligation does not include any professional services, such as engineering or
architecture. Contractor will use best efforts to match colors and finishes where applicable, but complete matches may not be possible.
2. PAYMENT. Owner shall pay Contractor within fifteen (15) days of invoice. Owner is solely responsible for full payment of "Contract Price" regardless of other
potential source of payment, including insurance. Time is of the essence for all payments. Credit Cards will only accepted for the lesser of the Owner's deductible or
$4,500.00. Deductibles are due at start of work.
3. CHANGES. The Owner may order changes in the Work consisting of additions, deletions or modifications, the Contract Price being adjusted accordingly. Owner and
Contractor shall endeavor to make changes in the Work by written Change Order signed by the Owner and Contractor. However, changes in Work may occur as a result
of a verbal Change Order agreed to by the Owner and Contractor or a signed Potential Change item. If additional work requested is for upgrade of materials or Owner
requested changes not covered by insurance, payment must be received prior to any work being completed. If the change will be priced on a time and material basis, the
Owner shall pay 75% of an estimated amount prior to any Work with the balance paid upon completion of the change.
4. UNFORESEEN CONDITIONS. If concealed or physical conditions are encountered which differ materially from those indicated in the Proposal or there are
unknown physical conditions of an unusual nature, an equitable adjustment in the Contract Price or the time of performance, or both, shall be agreed to pursuant to
written Change Order; or Contractor may at its option terminate.
5. DELAYS. If Contractor is delayed at any time in the progress of the Work by any act or neglect of the Owner or an employee of the Owner, or by a separate contractor
employed by Owner, or by changes in the Work, or by labor disputes, fire, unusual delay in deliveries, unavoidable casualty, or both, or other causes beyond the
Contractur's control, then the Contract Price shall be equitably adjusted pursuant to written or verbal Change Order. However, the Owner shall not be entitled to any
damages caused by such delays.
6. CEASE WORK/TERMINATION. Without limiting Contractor's other remedies, the Contractor is entitled to cease work or terminate upon the Owner's nonpayment
or other breach. In the event the Owner becomes current with the progress payments, and Contractor elects at its discretion to complete the Work, the Contractor shall
be entitled to an equitable adjustment in die Contract Price for a reinobilization charge.
7. WARRANTY. Conditioned upon receipt of Final Payment within thirty (30) days of when Final Payment is invoiced, Contractor shall assign to Owner all warranties of
manufacturers, and warrants to Owner the Work will be free from substantial defects in workmanship not inherent in the quality required or permitted for a period of
one (1) year fi-om Substantial Completion. Owner must provide written notice ofany alleged defects and the location within the one (1) year period, and provide access
to inspect and repair during not -mat business hours. Palace's Warranty excludes damages or defects caused by abuse, modifications, improper or insufficient
maintenance, or normal wear and tear under normal usage, and further excludes any warranty of concrete flatwork against cracking and/or movement. Owner's failure
to maintain or service pursuant to any manual provided by Contractor shall void all warranties. If Owner — does not provide Final Payment within thirty (30) days of
when Final Payment is invoiced, Owner accepts the Project as -is without any warranty. CONTRACTOR DISCLAIMS, AND OWNER WAIVES ANY AND ALL
OTHER EXPRESS, IMPLIED AND STATUTORY WARRANTIES INCLUDING, BUT NOT LIMITED TO: THE WARRANTIES OF HABITABILITY, FREEDOM
FROM STRUCTURAL DEFECTS, WORKMANLIKE CONSTRUCTION, FITNESS FOR A PARTICULAR PURPOSE, AND MERCHANT ABILITY.
S. ACCESS, COOPERATION, UTILITIES. Owner shalt cooperate with Contractor to complete the Work, including by making any selections in a timely manner and
taking all measures to allow prompt payments by any insurerand mortgage holder, including assigning rights to insurance proceeds and signing over to Contractor any
joint checks. Owner shall supply and pay for all necessary utilities during the duration of the project, unless explicitly included in the scope of Contractor's Work.
Owner shall provide all access needed to complete the Work.
9. FEES AND INTEREST. If any amount due to Contractor is not paid when due, Owner agrees to pay interest on the unpaid amount at the rate of 1.5% per month. In
the event Contractor commences an action in court or arbitration, Owner agrees to pay all costs of such action including but not limited to reasonable attorney's fees.
10. HAZARDOUS SUBSTANCES. If the Contractor encounters materials reasonably believed to be asbestos, mold, polychlorinated biphenyl, or other hazardous or toxic
substance, the Contractor shall immediately stop work in the arca affected and report the condition to the Owner. The Work in the affected area shall not thereafter be
resumed except by written agreement of the Owner and Contractor. If the Work is stopped for such reasons for more than one (1) week, Contractor may at its option
terminate the Contract. Upon such event, Owner shall pay Contractor for all Work performed to date, without retainage, as deter- mined by the Contractor's reasonable
estimate and invoice. Such payment shall include ten percent (10%) for overhead and ten percent (10%) for profit, and shall also include a demobilization charge which
shal I be ten percent (10%) of the Work not completed.
11. INSPECTION AND ACCEPTANCE. Owner agrees to inspect the Work at the request of Contractor, and to diligently schedule and expedite any inspections required
by Mortgage Companies, Lenders, or any entity that may be responsible for release of funds due Contractor for the performance of the Work. Owner agrees to execute a
letter of Substantial Completion of any particular portion of the Work when that portion is substantially completed. Owner acknowledges that the approval by the
building department(s) or Owner's architect or engineer shall be conclusive evidence of Contractor's compliance with all applicable codes and this Agreement. Once
work is inspected and approved, Contractor may rely on the approval to cover Work. Owner will be responsible to ensure Owner's architect or engineer inspects work
within two (2) days of a request for inspection. Owner will be responsible to ensure Owner's architect or engineer either approved Work in writing or alternatively
disapproves Work and provides a requirement in writing to obtain approval, within two (2) days of inspection.
12. ARBITRATION. Any controversy or claim arising out of or relating to this Agreement, or the breach thereof, shall, at Contractor's election, be settled by
arbitration before one arbitrator in accordance with the Construction Industry Arbitration Rules of the American Arbitration Association at a location selected by
Contractor, and judgment on the award rendered by the arbitrator may be entered in any court having jurisdiction. Each party agrees the other can properly serve by
regular mail to the address listed in this Agreement. Actual receipt need not be shown. Any requirement for other means or service is waived. Owner's obligation to
arbitrate at Contractor's election extends to Contractor's officers, employees, agents, subcontractors and suppliers, who are intended beneficiaries.
13. LIMITATION OF LIABILITY. TO THE FULL EXTENT PERMITTED BY COLORADO LAW, CONTRACTOR'S TOTAL LIABILITY IS LIMITED TO THE
CONTRACT Price, AND OWNER WAIVES, RELEASES AND COVENANTS NOT TO ASSERT ANY DAMAGES IN EXCESS OF THE CONTRACT PRICE.
14. MISCELLANEOUS. Colorado law governs interpretation and enforcement of this Agreement. This Agreement is the entire agreement of the parties and may not be
modified except by a writing signed by all parties, except as provided in -paragraph 3 above. This Agreement may be executed in counterparts, all of which shall
constitute originals. Facsimile or scanned signatures shall constitute originals. Any Owner that signs wan -ants he/she binds himself/herself to this Agreement and that
he/she is authorized to bind all other Owners jointly and severally. Contractor will remove contents at Owner's expense, which is not part of the Work unless included in
the estimate. Owner shall insure contents, and Owner waives and releases claims against Contractor for loss or damage to any contents.
15. ROOFING ON RESIDENTIAL PROPERTY. Contractor's liability coverage insurer is The Charter Oak Fin; Insurance Comoany, and their contact information is
1-80_0-328-2184. For the roofing portion of Work, Owner may rescind the Contract and obtain a full refund of any deposit for roofing Work within seventy (72) hours of
entering into this Agreement by providing Contractor written notice. Owner may rescind this agreement for roofing Work pursuant to Colorado Rcviscd Statute Section 6-
22-104, which in summary states the Agreement for roofing Work may be rescinded within seventy-two (72) hours after the Owner receives written notice that Owner's
insurance claim has been denied. If the Owner plans to use the proceeds of a property and casualty insurance policy issued pursuant to part I of article 4 of title 10, C.R.S.,
to pay for the roofing Work, pursuant to Section 6-22-105, the Contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance
deductible applicable to the insurance claim for payment for roofing Work on the covered residential property. Except as provided in this paragraph, Contractor's policy is
the contract may not be cancelled and any deposit is non-refundable.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process

TERMS & CONDITIONS
1. WORK. The estimate defines the Work. The Work shall be completed in a reasonable time given the circumstances. "Substantial Completion" means Owner can
reasonably occupy or use the structure for intended purposes. Unless included in the estimate, the Work does not include repairing pre-existing defects, concealed
conditions, or compliance with current building code requirements. Contractor's obligation does not include any professional services, such as engineering or
architecture. Contractor will use best efforts to match colors and finishes where applicable, but complete matches may not be possible.
2. PAYMENT. Owner shall pay Contractor within fifteen (15) days of invoice. Owner is solely responsible for full payment of "Contract Price" regardless of other
potential source of payment, including insurance. Time is of the essence for all payments. Credit Cards will only accepted for the lesser of the Owner's deductible or
$4,500.00. Deductibles are due at start of work.
3. CHANGES. The Owner may order changes in the Work consisting of additions, deletions or modifications, the Contract Price being adjusted accordingly. Owner and
Contractor shall endeavor to make changes in the Work by written Change Order signed by the Owner and Contractor. However, changes in Work may occur as a result
of a verbal Change Order agreed to by the Owner and Contractor or a signed Potential Change item. If additional work requested is for upgrade of materials or Owner
requested changes not covered by insurance, payment must be received prior to any work being completed. If the change will be priced on a time and material basis, the
Owner shall pay 75% of an estimated amount prior to any Work with the balance paid upon completion of the change.
4. UNFORESEEN CONDITIONS. If concealed or physical conditions are encountered which differ materially from those indicated in the Proposal or there are
unknown physical conditions of an unusual nature, an equitable adjustment in the Contract Price or the time of performance, or both, shall be agreed to pursuant to
written Change Order; or Contractor may at its option terminate.
5. DELAYS. If Contractor is delayed at any time in the progress of the Work by any act or neglect of the Owner or an employee of the Owner, or by a separate contractor
employed by Owner, or by changes in the Work, or by labor disputes, fire, unusual delay in deliveries, unavoidable casualty, or both, or other causes beyond the
Contractur's control, then the Contract Price shall be equitably adjusted pursuant to written or verbal Change Order. However, the Owner shall not be entitled to any
damages caused by such delays.
6. CEASE WORK/TERMINATION. Without limiting Contractor's other remedies, the Contractor is entitled to cease work or terminate upon the Owner's nonpayment
or other breach. In the event the Owner becomes current with the progress payments, and Contractor elects at its discretion to complete the Work, the Contractor shall
be entitled to an equitable adjustment in die Contract Price for a reinobilization charge.
7. WARRANTY. Conditioned upon receipt of Final Payment within thirty (30) days of when Final Payment is invoiced, Contractor shall assign to Owner all warranties of
manufacturers, and warrants to Owner the Work will be free from substantial defects in workmanship not inherent in the quality required or permitted for a period of
one (1) year fi-om Substantial Completion. Owner must provide written notice ofany alleged defects and the location within the one (1) year period, and provide access
to inspect and repair during not -mat business hours. Palace's Warranty excludes damages or defects caused by abuse, modifications, improper or insufficient
maintenance, or normal wear and tear under normal usage, and further excludes any warranty of concrete flatwork against cracking and/or movement. Owner's failure
to maintain or service pursuant to any manual provided by Contractor shall void all warranties. If Owner — does not provide Final Payment within thirty (30) days of
when Final Payment is invoiced, Owner accepts the Project as -is without any warranty. CONTRACTOR DISCLAIMS, AND OWNER WAIVES ANY AND ALL
OTHER EXPRESS, IMPLIED AND STATUTORY WARRANTIES INCLUDING, BUT NOT LIMITED TO: THE WARRANTIES OF HABITABILITY, FREEDOM
FROM STRUCTURAL DEFECTS, WORKMANLIKE CONSTRUCTION, FITNESS FOR A PARTICULAR PURPOSE, AND MERCHANT ABILITY.
S. ACCESS, COOPERATION, UTILITIES. Owner shalt cooperate with Contractor to complete the Work, including by making any selections in a timely manner and
taking all measures to allow prompt payments by any insurerand mortgage holder, including assigning rights to insurance proceeds and signing over to Contractor any
joint checks. Owner shall supply and pay for all necessary utilities during the duration of the project, unless explicitly included in the scope of Contractor's Work.
Owner shall provide all access needed to complete the Work.
9. FEES AND INTEREST. If any amount due to Contractor is not paid when due, Owner agrees to pay interest on the unpaid amount at the rate of 1.5% per month. In
the event Contractor commences an action in court or arbitration, Owner agrees to pay all costs of such action including but not limited to reasonable attorney's fees.
10. HAZARDOUS SUBSTANCES. If the Contractor encounters materials reasonably believed to be asbestos, mold, polychlorinated biphenyl, or other hazardous or toxic
substance, the Contractor shall immediately stop work in the arca affected and report the condition to the Owner. The Work in the affected area shall not thereafter be
resumed except by written agreement of the Owner and Contractor. If the Work is stopped for such reasons for more than one (1) week, Contractor may at its option
terminate the Contract. Upon such event, Owner shall pay Contractor for all Work performed to date, without retainage, as deter- mined by the Contractor's reasonable
estimate and invoice. Such payment shall include ten percent (10%) for overhead and ten percent (10%) for profit, and shall also include a demobilization charge which
shal I be ten percent (10%) of the Work not completed.
11. INSPECTION AND ACCEPTANCE. Owner agrees to inspect the Work at the request of Contractor, and to diligently schedule and expedite any inspections required
by Mortgage Companies, Lenders, or any entity that may be responsible for release of funds due Contractor for the performance of the Work. Owner agrees to execute a
letter of Substantial Completion of any particular portion of the Work when that portion is substantially completed. Owner acknowledges that the approval by the
building department(s) or Owner's architect or engineer shall be conclusive evidence of Contractor's compliance with all applicable codes and this Agreement. Once
work is inspected and approved, Contractor may rely on the approval to cover Work. Owner will be responsible to ensure Owner's architect or engineer inspects work
within two (2) days of a request for inspection. Owner will be responsible to ensure Owner's architect or engineer either approved Work in writing or alternatively
disapproves Work and provides a requirement in writing to obtain approval, within two (2) days of inspection.
12. ARBITRATION. Any controversy or claim arising out of or relating to this Agreement, or the breach thereof, shall, at Contractor's election, be settled by
arbitration before one arbitrator in accordance with the Construction Industry Arbitration Rules of the American Arbitration Association at a location selected by
Contractor, and judgment on the award rendered by the arbitrator may be entered in any court having jurisdiction. Each party agrees the other can properly serve by
regular mail to the address listed in this Agreement. Actual receipt need not be shown. Any requirement for other means or service is waived. Owner's obligation to
arbitrate at Contractor's election extends to Contractor's officers, employees, agents, subcontractors and suppliers, who are intended beneficiaries.
13. LIMITATION OF LIABILITY. TO THE FULL EXTENT PERMITTED BY COLORADO LAW, CONTRACTOR'S TOTAL LIABILITY IS LIMITED TO THE
CONTRACT Price, AND OWNER WAIVES, RELEASES AND COVENANTS NOT TO ASSERT ANY DAMAGES IN EXCESS OF THE CONTRACT PRICE.
14. MISCELLANEOUS. Colorado law governs interpretation and enforcement of this Agreement. This Agreement is the entire agreement of the parties and may not be
modified except by a writing signed by all parties, except as provided in -paragraph 3 above. This Agreement may be executed in counterparts, all of which shall
constitute originals. Facsimile or scanned signatures shall constitute originals. Any Owner that signs wan -ants he/she binds himself/herself to this Agreement and that
he/she is authorized to bind all other Owners jointly and severally. Contractor will remove contents at Owner's expense, which is not part of the Work unless included in
the estimate. Owner shall insure contents, and Owner waives and releases claims against Contractor for loss or damage to any contents.
15. ROOFING ON RESIDENTIAL PROPERTY. Contractor's liability coverage insurer is The Charter Oak Fin; Insurance Comoany, and their contact information is
1-80_0-328-2184. For the roofing portion of Work, Owner may rescind the Contract and obtain a full refund of any deposit for roofing Work within seventy (72) hours of
entering into this Agreement by providing Contractor written notice. Owner may rescind this agreement for roofing Work pursuant to Colorado Rcviscd Statute Section 6-
22-104, which in summary states the Agreement for roofing Work may be rescinded within seventy-two (72) hours after the Owner receives written notice that Owner's
insurance claim has been denied. If the Owner plans to use the proceeds of a property and casualty insurance policy issued pursuant to part I of article 4 of title 10, C.R.S.,
to pay for the roofing Work, pursuant to Section 6-22-105, the Contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance
deductible applicable to the insurance claim for payment for roofing Work on the covered residential property. Except as provided in this paragraph, Contractor's policy is
the contract may not be cancelled and any deposit is non-refundable.
