1. This permit was issued base on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance w h those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Budding Official and is not guaranteed.

For the purposes of this report, the front entrance of the building is assumed to face west. The structure
observed is a multi -family apartment building. The structure is a single -story building with a cast -in-place
concrete foundation. The exterior wall currently has a brick veneer. The floor and roof framing consists
of wood framing.
Based on our observation, it appears the foundation at the northeast corner of unit 4576 and the
southeast corner of unit 4574 has experienced vertical displacement. Therefore, the foundation at these
locations will require additional vertical support in order to reduce the risk of future foundation movement.
Our office recommends underpinning the foundation walls as specified in the attached pier placement
plan SD1.0. The new piers will add vertical support to the existing foundation. The piers should consist
of SafeBasement, Inc. SafeBase Standard Duty Piers and the associated connection bracket assembly.
Underpinning the foundation walls should provide the building with the required additional vertical
support in order to alleviate the vertical displacement as described above. Our office estimates that a
total of two (2) piers will be required at unit 4576 and seven (7) piers will be required at unit 4574. Each
pier should be designed to support a service load of 20 kips. The piers should be installed as close to
vertical as possible in order to avoid loading the pier eccentrically. The structure may be attempted to
be lifted to the original elevation or close to it prior to the attachment of the piers to the bottom of the
foundation wall. If lifting the structure is achieved, it should be noted that additional cosmetic damage in
the wall and ceiling finishes should be expected and may be repaired after piering operations are
completed. However, lifting is not required to achieve structural stability.
Based on our observation of the current site conditions, the proposed repairs as described above are
structurally adequate. After the piers are installed then any cracks in the wall finishes may be repaired.
The existing cracks in the exterior brick veneer may be tuck -pointed.
The above noted foundation movement has likely occurred due to improper site drainage around the

building and the inability of the water to drain freely away from the foundation wall. Therefore, our office
also recommends the following exterior grade maintenance procedures to be followed.
Exterior Grading Maintenance Procedures
The following finish grade maintenance procedures should be followed in order to keep water runoff away
from the foundation walls and reduce the risk of water infiltration and/or damage to the foundation walls and
superstructure.
1. In areas where site grading would allow ponding of water within ten feet of the structure the grade
should be reworked to provide proper drainage, or that underground drains should be installed in
order to route water runoff away from the structure.
2. Positive drainage should be maintained around the perimeter of the structure, such that water does
not pond at the face of the foundation walls and can drain freely away from the foundation. A minimum
slope of 4 inches in the first ten feet shall be provided around the entire building where possible. If
concrete slabs are located directly adjacent to the foundation walls they should be sloped away from
the foundation approximately 1/8" per foot in order direct water runoff away from the foundation wall.
3. Roof gutters and downspouts should be installed in order to direct roof runoff away from the
foundations. The downspouts should extend at least 10 feet or as far as possible beyond the extents
of the building perimeter with positive slope away from the structure. The downspout extensions
should be fastened to the downspouts. Pre -cast concrete splash blocks may be provided at
downspouts where downspout extensions are not able to be installed. The splash blocks must allow
the water runoff to drain away from the foundation walls and not allow the water to be able to drain
back toward the foundation.
At areas of the building where downspout extensions or splash blocks are unable to or are undesirable to be
installed, then underground drains may be installed as an alternate. The underground drains may be 4"
diameter non -perforated corrugated HDPE pipe and sloped with a minimum of 1/4" per foot to a suitable outlet.
PVC pipe may also be used.
The conclusions and recommendations presented in this report and the opinions expressed are based
on visual observations of the exposed elements as well as our professional knowledge and experience
in the engineering field and construction practices in Colorado. No wall or ceiling surfaces were
penetrated or disturbed during the course of the observation. This report should not be considered a
warranty or guarantee, implied, or expressed, of the building in general, the building superstructure, or
the foundation system.

foundations. The downspouts should extend at least 10 feet or as far as possible beyond the extents
of the building perimeter with positive slope away from the structure. The downspout extensions
should be fastened to the downspouts. Pre -cast concrete splash blocks may be provided at
downspouts where downspout extensions are not able to be installed. The splash blocks must allow
the water runoff to drain away from the foundation walls and not allow the water to be able to drain
back toward the foundation.
At areas of the building where downspout extensions or splash blocks are unable to or are undesirable to be
installed, then underground drains may be installed as an alternate. The underground drains may be 4"
diameter non -perforated corrugated HDPE pipe and sloped with a minimum of 1/4" per foot to a suitable outlet.
PVC pipe may also be used.

mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
