The Final Development Plan amendment for the property at 8300 W. 38`h Avenue (Exempla
Lutheran Hospital campus) has been reviewed and approved administratively. This amendment
added a driveway and a small parking area to be used by the adjacent property to the south, the
proposed Exempla Lutheran Hospice (8301 & 8321 W. 32°d Ave.). Pursuant to Section 26-311, the
Community Development Director may approve an amendment to a Final Development Plan, given
the listed criteria.

I have completed my initial review of the above-mentioned project, and I have the following
comments:
1. Page One:
A. Please correct the sheet numbering system in the upper-right hand corner of the page.
B. Please add "WZ-05-03" to the Case History box.
C. In the first line of the Owner's Certification, please correct the word "thereof'.
Please make the following corrections. Upon completion, create a mylar copy of each page. The
surveyor should sign and seal the certification. The property owner /agent should sign the document
and have it notarized. Please ensure that each signature is in indelible ink, not ball-point ink. Please
submit the mylar copy along with a check for $22 for recordation with Jefferson County. If you
