I. This permit was issued based on the information provided in the permit application and accompanying Flans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required

approval. Installation cf roof sheathing (new or overlay) requires an approved inspection

is required to comply with applicable codes and/or manufacturer&€—s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomr�ric or similar type roof coverings, a letter of inspection and approval from the
manufacturer',s technical representative stating that "the application of the roof at

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in

at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Contingency Agreement
HRR Roofing agrees to invest its time, expertise and expense in
assisting the homeowner with the claim. This proposal is conting-
ent upon the insurance company paying for the roof and/or any
other storm related damage to the property and will be void only if
the claim is not accepted by the insurance company.
The insurance company will determine and set the price for the
claim. By signing below, you agree that HRR Roofing is to perform
the work as specified in the insurance adjuster's estimate. The
homeowner's out of pocket expense will not exceed the deductible
amount unless otherwise noted on the original contract as specified
by HRR Roofing./

Both sides of this document, including the terms and conditions below, and any agreement executed in writing .pursuant thereto, be-
tween Herch Roofing & Renovations and the property owner(s) or property owner's representative(s) hereby referred to as the consu-
mer, are subject to the taws in effect in the State in which it has been signed and executed.
ANY REPRESENTATION, STATEMENT, OR OTHER COMMUNICATIONS NOT WRITTEN iN THIS AGREEMENT, OR MADE IN WRITING PURSU-
ANTTHERETO ARE AGREED TO BE INVALID, AND NOT RELIED ON BY EITHER PARTY AND, DO NOT SURVIVE THE EXECUTION OF THIS
AGREEMENT.
1. The Customer agrees to make payment to the Company in the Actual Cash Value amount provided by the insurance company
once the shingles are installed regardless of other wort: to be competed. i€ there is non -roof related work left to complete and if
depreciation was not withheld from the Customer's settlement then Customer agrees to pay 809- of the settlement amount re-
ceived. Once all work is complete, the balance of ail funds received by the Customer are to be paid to the Company including any
and all supplemer7ts as well as overhead and profit. if there is depreciation held by the Carrier, the Company will submit all docu-
mentation to the Carrier, along with any supplements and agrees to wait until the Customer receives that payment for the Cus-
tomer to pay the Company.
2. Customer should receive their depreciation/supplemental check within four weeks of job completion. It is the Customer's re-
sponsibility to notify the Company that the checl: has been received to schedule a pickup, Additionally, the Customer agrees to
provide the Company copies of all paperwork received from their insurance carrier.
3. Should default be made in payment of this agreement, a lien will be placed on the property and interest charges will be added
from the date of substantial completion at the maximum allowed by law. If placed in the hands of an attorney for collection, the
Customer agrees to pay all attorney and legal fees,
4. The Company and the Customer agree to settle all disputes in accordance with the laws of the state and county in which this
agreement is executed.
5. This agreement does not include any extras such as carpentry repairs, or any other repairs not specified in the insurance adjust-
er's estimate unless agreed to in writing.
6. if the Customer chooses not to pay for a part of the scope of the agreement, an individual line item, or a trade, the Customer
releases the Company of it's obligations for the performance of that component with regard to the integrity of the building sys-
tem as a whole.
7. This agreement can be canceled at any time prior to midnight of the third business day after the date of agreement.
8. If this agreement is canceled by the customer later than three (3) days from execution, Customer agrees to pay to the Company
250,o- of the agreement price for: 1) Consulting Services Rendered, or 2) Liquidating Fees. if materials have to be returned due to
this cancellation, the Customer agrees to pay an additional 15% for re -stocking fees.
9. The Company reserves the right to supplement the Insurance Company for increases in the Full Scope of Work, and or, docu-
mented price increases. Customer agrees to allow these supplements to be paid directly to Company.
10. All excess materials remain the property of the Company.
11. Full Scope of Insurance Proceeds shall be defined as the full price for repairs and replacements allowed by the insurance company
to include any and all supplements as well as overhead and profit submitted by the Company.
12. If any provisions of this agreement should be held invalid or unenforceable, the validity and enforceability of the remaining provi-
sions shall not be affected thereby.
13. The Company will remove all trash from the roof, gutters and yard, protea all landscaping where applicable and roll the yard and
