Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.

The stated "Property Owner", or "Agent", Hereby enters into this contract with CT Contracting LLC, DBA CT Roofing & Restoration hereon

Any permits needed or pulled for specific portions of the project are an agreement between the jurisdiction and contractor to perform professional
workmanship and use approved materials. This agreement will not hold up funds due to contractor while waiting for inspections due to high volume
and weather this can take time to go through inspection process.
Property Owner or Agent, may cancel this contract with in three days or 76 hours prior to midnight on the third business day following signed date of
this contract transaction. Property Owner must cancel in writing and mail certified mail within the allowed time frame without incurring penalties. If
Property Owner cancels this contract after the allowed time, there will be a 25% cancellation, and restocking fee. This 25% is based off of the agreed
total sum of project.. Contractor has the right to use sub -contractors. This contract supersedes all previous agreements, rebates, discounts, or
concessions whether written or oral. Contractor has the right to cancel this contract at any time solely upon contractor discretion, if Contractor feels
Property Owner or Agent, is unwilling to pay fair market value for materials and labor, Property Owner or Agent has read both front and back of this
contract and agree to contract terms and conditions.

Contractor shall be responsible for completing work required as pert of the inspection process, provided it is within the contracted
scope of work -DS
Both sides of this contract, including the terms and conditions set forth below, is between CT Contracting LLC, DBA CT Roofing & Restoration, and the
Property Owner, and are subject to all applicable rules, regulations, laws, and ordinances in effect as of the date of this contract in the State of Colorado and
the local jurisdiction of the property locale.
1. In the event that any provision of this contract is held invalid by an arbitrator, or a court of competent jurisdiction, the remaining provisions shall nonetheless be
enforced in accordance with their terms. Further, in the event that any provision is held to be over broad as written, such provisions shall be enforced as amended. Any
dispute, controversy or claim arising out of relating to the contract, or relationship between parties, other than for non-payment by the Property Owner, shall be settled
by binding arbitration in accordance with the Construction Industry Arbitration Rules of the American Arbitration Association (AAA). Such arbitration shall be
conducted in the city of Denver, State of Colorado, or such other location as the parties may mutually agree. The decision of the arbitrator shall be final and accorded
full faith and credit and entitled to recognition and enforced by both Federal and State Courts of the United States. This contract shall be governed by the laws of the
State of Colorado.
2. If Property Owner cancels this contract after the 76 hours of business allowed from the date on this contract, Property Owner agrees to pay to contractor liquidated
damages of 25% of contract amount or AIA, whichever is applicable for consulting services. In addition to a 15% restocking fee if materials have been ordered or
purchased, and lost profit which Contractor and/ or Project Manager may incur as a result of Property Owners failure to perform any obligations under this contract
3. PLEASE MAKE ALL CHECKS PAYABLE TO CT ROOFING & RESTORATION. Only payments of good funds shall be considered received by Contractor.
Only good funds shall be considered satisfactory by Contractor on Property Owners obligations. Property Owner payment agreements are on the front page of this
contract. Property Owner is to make payments in a timely manner, any payment past 15 days from the time payments are made available to Property Owner or
Agent is considered late a late fee of 15% of agreed total or AIA will be added to invoice per month until paid in full. Property Owner will notify Contractor as soon as
they are aware there may be an issue with a timely payment. Property Owner agrees that the Agreed Insurance Allowance or AIA is the contract amount, including but
not limited to and additional proceeds paid to Property Owner by their insurer as part of or in addition to AIA for the total sum of the project. Failure to comply with
this contract and payment schedule by Property Owner will be considered a default. Should default of payment arise in this contract, Contractor has the right to file a
Mechanic's Lien against the property written on front page of this contract. If Property Owner defaults on payment they will be responsible to pay all expenses, legal
fees, governing filling fees, court cost, and lost profit from contract including lost profit from time taken by Contractor during the process of this default.
4. Contractor work and material is deemed accepted by Property Owner unless Property Owner gives written notification within ten (10) days of completion of contract
project specifications. Notification of deficiencies needs to be clearly defined by Property Owner. All claims not made in writing in such timeline shall be deemed
released by Property Owner and considered that the Contractors work is complete and to Property Owners satisfaction.
5. As long as Contractor has taken appropriate measure to protect the property on this contract, Contractor is not liable or responsible for interior or exterior damages
during Contractors work including but not limited to Project Specifications of this contract. Damages caused by rain, hail, tomado, windstorm, fire, ice dams or any act
of God and or Mother Nature during the project is not the liability nor is Contractor responsible for these damages during or after the warranty period. Contractor is
not responsible for any pre-existing conditions or deficiencies of property during or after warranty period including but not limited to dry wall nail pops,
decking deflections, wood rot, mold, structural separations or foundation failures. Contractor agrees to perform necessary duties to solve pre-existing
conditions if Property Owners notify Contractor or Contractor notifies Property Owners in writing. Notification will be handled in a professional manner and
Property Owners are responsible for the expense in repair or replacements of conditions. Contractor will charge a fair market value for the repair or replacements,
Contractor has the right to use sub contractors for any portion of project specified in this contract including any pre existing conditions or additional work needed to
complete the project. If excessive damage is caused by Contractor during the work of this contract, it will be the Contractors responsibility to repair or replace damages.
This excludes minimum or slight damages to, drywall, landscape, furniture, doors, windows, screens, gutters, fascia, walkways and driveways. Property Owner agrees
to have property ready for material delivery and work to be performed, this includes but not limited to keeping driveway and yards clear of personal belongings,
vehicles, trailers, animals, and any objects which may inhibit the progress of the project. Contractor is not liable for failure of performance due to, inability to obtain
material from usual suppliers, other trades performance, fire, weather, and acts of God or Mother Nature. Contractor shall not be liable for consequential damages or
incidental damages whatsoever.
6. Microorganisms, including but not limited to, mold, mildew, spores, and other forms of fungi or bacteria occur naturally in the environment, and maybe present
before, during and after repairs or replacement of project specifications. Microorganisms could be in but not limited to, interior wall cavities, indoor air, attics, and
exterior surfaces of the property specified on this contract. Concentration of moisture in property may result from heating, cooling, cooking, showering, or similar
activities inside property, the outside atmosphere, and/or the design, construction means, and/or methods of building materials used in initial construction of property.
This moisture may cause growth, release, discharge, dispersal, or presence of microorganisms, which at certain levels can cause deterioration of materials, damage to
the property, health hazards, and personal injury. Likewise, concentrations of radon released from soil, and chemicals released from household furnishings, appliances,
mechanical equipment, personal possessions, and/or building material, may at certain levels, cause health hazards or personal injury. Because microorganisms and
radon occur naturally Contractor cannot eliminate the possibility that radon and or microorganisms may be present, or that microorganisms may grow in, on, or about
the property. PROPERTY OWNER ACKNOWLEDGES RISKS OF RADON AND MICROORGANISMS AND RELEASES CONTRACTOR OF ANY
LIABILITY OR RESPONSIBILITY, OR ANY WAY CONNECTED WITH THE SAME.
7. This contract shall be binding upon and inure to the benefit of any permitted successors and assigns of CT Contracting LLC, DBA CT Roofing & Restoration, or the
Property Owner. No purported amendments, modifications, or waivers of any provision of this contract shall be binding unless set forth in writing and both parties have
signed (in the case of amendments and modifications), or by the party to be charged there by(in case waivers). Any waiver shall be limited to the circumstances of
events specifically referenced in written waiver document, and shall not be deemed a waiver of any other term of this contract, or of the same circumstance or event
upon any recurrence thereof.
8. CT Roofing & Restoration. Workmanship Warranty is two (2) years for total replacement and or repairs. All applicable
written manufactures warranties on material shall apply, and may be transferable or assignable for a fee of $250.00, recoverable, or
paid to Contractor, unless specified in writing and signed by both parties. This warranty is limited to, fire, weather in excess force, ice
damming, animals, other trades or company tampering with, and acts of God or Mother Nature. This warranty is only valid if
payments are made in full and Property Owner has not defaulted in payments. Manufactures warranties may have a transfer fee which
Property Owner is responsible for any expense thereof.

to City Hall to submit an application in person. Permits are processed and issued in

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Property Owner cancels this contract after the allowed time, there will be a 25% cancellation, and restocking fee. This 25% is based off of the agreed
total sum of project.. Contractor has the right to use sub -contractors. This contract supersedes all previous agreements, rebates, discounts, or
concessions whether written or oral. Contractor has the right to cancel this contract at any time solely upon contractor discretion, if Contractor feels
Property Owner or Agent, is unwilling to pay fair market value for materials and labor, Property Owner or Agent has read both front and back of this

Contractor shall be responsible for completing work required as pert of the inspection process, provided it is within the contracted
scope of work -DS
Both sides of this contract, including the terms and conditions set forth below, is between CT Contracting LLC, DBA CT Roofing & Restoration, and the
Property Owner, and are subject to all applicable rules, regulations, laws, and ordinances in effect as of the date of this contract in the State of Colorado and
the local jurisdiction of the property locale.
1. In the event that any provision of this contract is held invalid by an arbitrator, or a court of competent jurisdiction, the remaining provisions shall nonetheless be
enforced in accordance with their terms. Further, in the event that any provision is held to be over broad as written, such provisions shall be enforced as amended. Any
dispute, controversy or claim arising out of relating to the contract, or relationship between parties, other than for non-payment by the Property Owner, shall be settled
by binding arbitration in accordance with the Construction Industry Arbitration Rules of the American Arbitration Association (AAA). Such arbitration shall be
conducted in the city of Denver, State of Colorado, or such other location as the parties may mutually agree. The decision of the arbitrator shall be final and accorded
full faith and credit and entitled to recognition and enforced by both Federal and State Courts of the United States. This contract shall be governed by the laws of the
State of Colorado.
2. If Property Owner cancels this contract after the 76 hours of business allowed from the date on this contract, Property Owner agrees to pay to contractor liquidated
damages of 25% of contract amount or AIA, whichever is applicable for consulting services. In addition to a 15% restocking fee if materials have been ordered or
purchased, and lost profit which Contractor and/ or Project Manager may incur as a result of Property Owners failure to perform any obligations under this contract
3. PLEASE MAKE ALL CHECKS PAYABLE TO CT ROOFING & RESTORATION. Only payments of good funds shall be considered received by Contractor.
Only good funds shall be considered satisfactory by Contractor on Property Owners obligations. Property Owner payment agreements are on the front page of this
contract. Property Owner is to make payments in a timely manner, any payment past 15 days from the time payments are made available to Property Owner or
Agent is considered late a late fee of 15% of agreed total or AIA will be added to invoice per month until paid in full. Property Owner will notify Contractor as soon as
they are aware there may be an issue with a timely payment. Property Owner agrees that the Agreed Insurance Allowance or AIA is the contract amount, including but
not limited to and additional proceeds paid to Property Owner by their insurer as part of or in addition to AIA for the total sum of the project. Failure to comply with
this contract and payment schedule by Property Owner will be considered a default. Should default of payment arise in this contract, Contractor has the right to file a
Mechanic's Lien against the property written on front page of this contract. If Property Owner defaults on payment they will be responsible to pay all expenses, legal
fees, governing filling fees, court cost, and lost profit from contract including lost profit from time taken by Contractor during the process of this default.
4. Contractor work and material is deemed accepted by Property Owner unless Property Owner gives written notification within ten (10) days of completion of contract
project specifications. Notification of deficiencies needs to be clearly defined by Property Owner. All claims not made in writing in such timeline shall be deemed
released by Property Owner and considered that the Contractors work is complete and to Property Owners satisfaction.
5. As long as Contractor has taken appropriate measure to protect the property on this contract, Contractor is not liable or responsible for interior or exterior damages
during Contractors work including but not limited to Project Specifications of this contract. Damages caused by rain, hail, tomado, windstorm, fire, ice dams or any act
of God and or Mother Nature during the project is not the liability nor is Contractor responsible for these damages during or after the warranty period. Contractor is
not responsible for any pre-existing conditions or deficiencies of property during or after warranty period including but not limited to dry wall nail pops,
decking deflections, wood rot, mold, structural separations or foundation failures. Contractor agrees to perform necessary duties to solve pre-existing
conditions if Property Owners notify Contractor or Contractor notifies Property Owners in writing. Notification will be handled in a professional manner and
Property Owners are responsible for the expense in repair or replacements of conditions. Contractor will charge a fair market value for the repair or replacements,
Contractor has the right to use sub contractors for any portion of project specified in this contract including any pre existing conditions or additional work needed to
complete the project. If excessive damage is caused by Contractor during the work of this contract, it will be the Contractors responsibility to repair or replace damages.
This excludes minimum or slight damages to, drywall, landscape, furniture, doors, windows, screens, gutters, fascia, walkways and driveways. Property Owner agrees
to have property ready for material delivery and work to be performed, this includes but not limited to keeping driveway and yards clear of personal belongings,
vehicles, trailers, animals, and any objects which may inhibit the progress of the project. Contractor is not liable for failure of performance due to, inability to obtain
material from usual suppliers, other trades performance, fire, weather, and acts of God or Mother Nature. Contractor shall not be liable for consequential damages or
incidental damages whatsoever.
6. Microorganisms, including but not limited to, mold, mildew, spores, and other forms of fungi or bacteria occur naturally in the environment, and maybe present
before, during and after repairs or replacement of project specifications. Microorganisms could be in but not limited to, interior wall cavities, indoor air, attics, and
exterior surfaces of the property specified on this contract. Concentration of moisture in property may result from heating, cooling, cooking, showering, or similar
activities inside property, the outside atmosphere, and/or the design, construction means, and/or methods of building materials used in initial construction of property.
This moisture may cause growth, release, discharge, dispersal, or presence of microorganisms, which at certain levels can cause deterioration of materials, damage to
the property, health hazards, and personal injury. Likewise, concentrations of radon released from soil, and chemicals released from household furnishings, appliances,
mechanical equipment, personal possessions, and/or building material, may at certain levels, cause health hazards or personal injury. Because microorganisms and
radon occur naturally Contractor cannot eliminate the possibility that radon and or microorganisms may be present, or that microorganisms may grow in, on, or about
the property. PROPERTY OWNER ACKNOWLEDGES RISKS OF RADON AND MICROORGANISMS AND RELEASES CONTRACTOR OF ANY
LIABILITY OR RESPONSIBILITY, OR ANY WAY CONNECTED WITH THE SAME.
7. This contract shall be binding upon and inure to the benefit of any permitted successors and assigns of CT Contracting LLC, DBA CT Roofing & Restoration, or the
Property Owner. No purported amendments, modifications, or waivers of any provision of this contract shall be binding unless set forth in writing and both parties have
signed (in the case of amendments and modifications), or by the party to be charged there by(in case waivers). Any waiver shall be limited to the circumstances of
events specifically referenced in written waiver document, and shall not be deemed a waiver of any other term of this contract, or of the same circumstance or event
upon any recurrence thereof.
8. CT Roofing & Restoration. Workmanship Warranty is two (2) years for total replacement and or repairs. All applicable
written manufactures warranties on material shall apply, and may be transferable or assignable for a fee of $250.00, recoverable, or
paid to Contractor, unless specified in writing and signed by both parties. This warranty is limited to, fire, weather in excess force, ice
damming, animals, other trades or company tampering with, and acts of God or Mother Nature. This warranty is only valid if
payments are made in full and Property Owner has not defaulted in payments. Manufactures warranties may have a transfer fee which
Property Owner is responsible for any expense thereof.
