(1) This pemit was issued in accordance with the provisions set foAh in yopur application and is subject to the laws of the State of Coloredo and to the Zoning
Regulations and Building Code of Wheat Ridge, Colorado or any other applicable ordinances of the City.
(2) This pemit shall expire if (A) the work authonzed is not commenced within sixty (60) days from issue date or (B) the building authonzed is suspended or
abandoned for a period of 720 tlays.
(3) If ihis pemit expires, a new pemit may be acquired for a fee of one-half the amount nortnally required, provided no changes have been or will be made in the
original plans and specifcations and any suspension or abandonment has not ezceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid (or a new pertnit. (4) No work of any manner shall be done that will change the naturel flow ofwater causing a drainage problem.
(5) Contractor shall nolify the Building Inspectortwenly-four (24) hours in advance for all inspections and shall receive wriHen approval on inspection card 6efore

approval of, any violation of the provisions of th¢ building codes or any other ordinance, law, rule or regulation.

DEMOLITION APPROVAL NOTICE
This approval notice is granted subject to Colorado Air Quality Control Commission
Regulation No. 8, Part B, adopted December 18, 2003; and effective March 1, 2004, the Air
Quality Control Act C.R,S. (25-7-101 and 25-7-5:01 et seq). This noiice signifies that the
structure was inspected for asbestos and CFCs and the demolirion coritractor has properly
notified the Colorado Department of Public Health and Enviroriment pursuant to Regulation
No. 8, Part B.
As a contractor, you maybe subject to other demolition licenses and permits, depending on
the requirements of the county and municipality in which the work is being performed. The
Colorado Department of Public Health and Environment, Air Pollutiori Control Division,
strongly suggests that you check With county and muriicipal authorifies in order to
determine any other local building/permitting requirements that rnust be met.
THE ORIGINAL PERMIT MUST BE POSTED ON SITE AT ALL TIMES.
Immediately notify the Unit of project modifications by faz (number above) or e-mail (address above) and
the appropriate county health department by fax Project modifications include changes in the scope of
work or the scheduled work dates, etc.
