1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

As requested, on March 15, 2019 American Veteran Environmental, LLC performed a limited Asbestos
Inspection within the single-family residential dwelling located at 9009 W. 32"d Avenue in Wheatridge,
Colorado. The purpose of the limited inspection was to inspect and sample suspect asbestos containing
materials within the main bathroom of the residence prior to renovation activities.
The Environmental Protection Agency (EPA) and Colorado Department of Public Health and Environment
(CDPHE) Air Quality Control Commission requires suspect Asbestos -Containing Materials (ACM) to be
properly sampled and analyzed prior to any renovation or demolition activities. The NESHAP (National
Emission Standard for Hazardous Air Pollutants) rule requires that suspect regulated asbestos -containing
building materials be identified, classified and quantified prior to renovation or demolition activities.
The Colorado Department of Public Health and Environment (CDPHE) is responsible for administering
the U.S. EPA NESHAP program for Colorado. The table below summarizes the sample(s) collected and
the analytical results.

A total of eight (8) samples were collected and submitted under chain of custody procedures to an
accredited laboratory under the National Voluntary Laboratory Accreditation Program for analysis by
Polarized Light Microscopy (PLM) with dispersion staining techniques per EPA methodology (40 CFR 763,
Subpart F). Microscopic visual estimation was used in obtaining the percentage of asbestos in bulk
samples. Laboratory Results are located in Appendix A at the end of this report.
RESULTS AND RECOMMENDATIONS
The following materials were found to be asbestos containing:
• Gold sheet vinyl flooring- installed as a baseboard on wails within bathroom. Must assume
exists below 12x12 tan peel and stick floor the bathroom floor.
• AVENV recommends that a certified asbestos abatement contractor properly remove and
dispose of the sheet vinyl baseboard if it will be impacted during renovation or demolition
activities.
• If the same material is confirmed below the 12x12 (non -suspect) floor tile, AVENV
recommends that a certified asbestos abatement contractor properly remove and dispose of
the sheet vinyl flooring if it will be impacted during renovation or demolition activities.
• Since the amount of both the sheet vinyl baseboard and the sheet vinyl flooring is below 32
square feet (trigger limit for single-family residential dwelling) Regulation No. 8 Part 13 -
Asbestos does not apply. Refer to Sections III.G.S.a. and IiI.G.5.b. for regulations pertaining to
single-family residential dwellings, owner occupied and the "Opt -out" notification.
• The proper disposal of any asbestos containing material is required regardless of removal
procedures.

AVENV only sampled as requested based on the current renovation plans and limited to the main
bathroom. This inspection and report is not to be considered a comprehensive asbestos survey of this
address. For any materials not listed in this report that may be subject to renovation or demolition,
contact AVENV for additional sampling. As per the Asbestos Hazard Emergency Response Act (AHERA), a
suspect material is to be considered asbestos -containing unless it is proven otherwise by proper
inspection, sampling and analysis,

Enclosed are asbestos analysis results for PLM Bulk samples received at our laboratory on
March 18, 2019. The samples were analyzed for asbestos using polarizing light microscopy
(PLM) per the EPA 600 Method.
Sample results containing >1 % asbestos are considered asbestos -containing materials
(ACMs) per EPA regulatory requirements. The detection limit for the EPA 600 Method is <1%

not covered under NVLAP accreditation. Estimated measurement of uncertainty is available on
request.
This report relates only to the samples tested or analyzed and may not be reproduced, except in full,
without written approval by Eurofins CEI. Eurofins CEI makes no warranty representation regarding
the accuracy of client submitted information in preparing and presenting analytical results.
Interpretation of the analytical results is the sole responsibility of the client. Samples were received in
acceptable condition unless otherwise noted. This report may not be used by the client to claim
product endorsement by NVLAP or any other agency of the U.S. Government.

INTRODUCTION
As requested, on May 6, 2019 American Veteran Environmental, LLC (AVENV) conducted a post
abatement final visual inspection following the removal and disposal of asbestos containing materials
previously identified at 9009 W. 32"d Avenue in Wheatridge, Colorado. The subject property is an owner
occupied, single family residential dwelling (SFRD).
CONFIRMED ASBESTOS CONTANING MATERIAL
• Gold sheet vinyl installed on lower 4 inches of wall as a baseboard in the main bathroom.
Assumed to be under the non -asbestos 12x12 peel and stick tile.
FIELD ACTIVITIES
All asbestos containing sheet vinyl flooring was removed by the homeowner using precautionary
measures and Appendix B guidelines for flooring contractors as outlined in Colorado Department of Public
j Health and Environment (CDPHE) Colorado Regulation No. 8 Part 8 -Asbestos. All materials were removed
by hand using wet methods and placed in 6 -mil polyethylene asbestos waste bags bearing the OSHA
warning label and secured for disposal.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

complete my project. I hope to be done much sooner.
Here's where I am at in construction:
The exterior of the house is nearly complete. (minor details left)
I have all of the framing work done. I am nearly compiete on the rough electric. I just
started the plumbing and mechanical work.
This project has taken me more days to complete than I originally estimated. My day
job has been more demanding than I thought it would be this year as well. I plan on
completing this project as soon as possible and spend all of my free time and weekends
working on it.

For one and two-family dwellings, the first 25' of driveway area from the existing edge of pavement into the site
shall be surfaced with concrete, asphalt, brick pavers or similar materials.
For all uses other than one and hvo family dwellings, no vehicle. entrances or exits may be closer than 25' to any
