Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A% -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

ACCEPTANCE OF AGREEMENT
By signing this Contingency Contract, you agree to authorize Storm Guard to assist you in your pursuit of a claim under your
insurance policy and your efforts to reach agreement with your insurer on project scope and price. You may cancel this
contract with no penalty by providing written notice before midnight on the third business day after the date of your
signature below, or any time before your insurer approves your claim, whichever is later. You have no obligation to
Storm Guard under this Contract unless your insurer approves your claim and Storm Guard approves the scope and price
identified on the loss statement you receive from your insurer. If your insurer approves your claim, and if Storm Guard approves
the scope and price identified on the loss statement you receive from your insurer, then, by signing below, you authorize Storm
Guard to complete the replacement of damaged property identified on the loss statement with no additional cost to you except for
your insurance deductible, subject to the terms and conditions on the reverse side of this Contract. Payments must be made as
outlined above. If this proposal is not accepted within thirty (30) days, it shall be automatically withdrawn.

TERMS AND CONDITIONS
1. Contingencies. This Contract is contingent upon: 1. your insurance company's approval of your claim; and 2. our approval of
the scope and price identified on the loss statement you receive from your insurer. We may cancel this Contract any time prior
to the start of Work. If your insurer denies your claim, then this contract is not effective.
2. Contract Documents and Changes. This Contract consists of this Contingency Contract and any addenda, change orders,
drawings, plans, or specifications. Any work resulting from alteration or deviation from this Contract, including the replacement of
deteriorated materials (e.g., decking, fascia boards, roof jackets, ventilators, Flashing, etc.), hidden damage, or any other materials not
specified for replacement in this Contract, as well as any additional work required by government inspectors to make the existing
structure code compliant, requires a written change order signed by all parties and may result in extra charges beyond the Contract
price, which shall be your responsibility. This Contract embodies the entire agreement of the parties and supersedes all prior
agreements and understandings. Any promises or representations not included in this Contract are expressly denied.
3. MECHANIC'S LIEN RIGHTS. (A) ANY PERSON OR COMPANY SUPPLYING LABOR OR MATERIALS FOR THIS IMPROVEMENT TO YOUR PROPERTY
MAY FILE A LIEN AGAINST YOUR PROPERTY IF THAT PERSON OR COMPANY IS NOT PAID FOR THE CONTRIBUTIONS. (B) YOU HAVE THE
RIGHT TO PAY PERSONS WHO SUPPLIED LABOR OR MATERIALS FOR THIS IMPROVEMENT DIRECTLY AND DEDUCT THIS AMOUNT FROM OUR
CONTRACT PRICE, OR WITHHOLD THE AMOUNTS DUE THEM FROM US UNTIL 120 DAYS AFTER COMPLETION OF THE IMPROVEMENT, UNLESS
WE GIVE YOU A LIEN WAIVER SIGNED BY PERSONS WHO SUPPLIED ANY LABOR OR MATERIAL FOR THE IMPROVEMENT AND WHO GAVE YOU
TIMELY NOTICE.
4. Warranties. Upon our receipt of full payment for the Work, we provide the following warranties: (a) during the one-year period
from and after completion of the Work, the Work shall be free from defects caused by faulty workmanship or defective materials
due to noncompliance with building standards; (b) during the five-year period from and after completion of the Work, all products
sold in connection with the Work shall be applied according to the manufacturer's written specifications. Notwithstanding any of
the above, we expressly disclaim any and all liability for loss or damage of any kind that is not reported to us in writing within six
(6) months after you discover or should have discovered the loss or damage. These warranties are given in lieu of all other
warranties, express or implied. In no event shall we be liable for consequential or incidental damages of any kind, including any
damage to the building, its contents, or any persons therein.
S. Owner's Obligations and Commitments. You shall: (a) grant free access to work areas for workers and vehicles; (b) allow storage
of materials and rubbish on your property; (c) keep driveways clear and available for movement and parking of vehicles during
normal working hours, including removal of ice and snow; (d) supply, electric, water, and other utilities, including toilet facilities, or
compensate us for the cost of providing them; (e) arrange for identification of underground utilities before we perform any digging;
(f) remove, protect, or secure all satellite dishes, solar panels, sky lights, and other exterior and interior personal property (e.g., shrubs,
flowers, wall hangings, etc.) before the Work begins. You shall hold us harmless from any and all damage to your personal property,
including but not limited to interior fixtures, drywall, plaster wall construction, and decorations, unless caused by our negligence. We
shall not be responsible for realigning satellite dishes or solar panels. The Work may require the use of dumpsters, heavy equipment
vehicles, and construction equipment on your property. You hold us harmless from any damage to your property caused by weight
or movement of such dumpsters, vehicles, or equipment. Dumpsters are for construction debris only. You may be charged additional
fees for disposing other items in our dumpsters. We shall take reasonable efforts to remove construction debris. However, you shall
hold us harmless from any damage or injury caused by debris remaining on your property after completion of the Work. We shall not
be expected to keep gates and doors closed, and you shall hold us harmless from claims arising therefrom. Excess construction material
shall remain our property.
6. Collection Costs. Unless otherwise limited by applicable law, we shall add a 1.5% monthly service fee to any outstanding
balance not paid within thirty (30) days the due date. You agree to pay all reasonable costs we may incur in order to collect amounts
owing under this Contract, including reasonable attorneys' fees, cost, and disbursements. You shall not delay final payment while
waiting for final government inspection of the Work.
7. Events beyond Our Control and Exclusions. You shall hold us harmless from pre-existing conditions on the property discovered
during completion of the Work. Existing out of square and plumb conditions may require similar conditions in the Work. Some
variation may occur in color, texture and planes of materials. We may substitute similar materials if specified materials are
unavailable. We do not evaluate venting and circulation systems. Performance under this Contract is contingent upon strikes,
accidents, or other delays beyond our control. The Contract price may be adjusted due to material or labor price increases caused
by unforeseen problems arising after the Work has begun, market supply shortages, or unusual spikes in market demand. The
Contract price does not include any governmental permit, service, or access charge. We are not responsible for removing hazardous
material (e.g., lead paint or asbestos) from your property and may stop Work until it is removed.
S. Advertising. You authorize us to place advertising yard signs on your property for promotion and identification purposes for workers
and material suppliers. You grant us an unlimited license to record images of the Work in any form and to reproduce those images for
advertising and promotional use.
9. Insurance. To the extent required by law, we maintain current insurance policies covering our general liability and workers
compensation obligations. Prior to commencement of the Work, you shall have insurance to cover the finished cost of improvements.
10. Performance Guidelines. We agree to complete the Work set forth in this Contract in accordance all applicable construction
codes and the manufacturers' specifications for installation of all materials.
11. Cancellation. You may cancel this contract with no penalty by providing written notice before midnight on the
third business day after the date of your signature on the first page of this Contract, or any time before your
insurer approves your claim, whichever is later. If you cancel this Contract after your insurer approves your claim but before
commencement of the Work without legal right to do so, we shall be entitled to the actual costs we incurred in connection with the
Work (including any restocking charges) plus 25% of the Contract price, which you agree is a fair and reasonable amount to
compensate us for our reliance on the Work, and not a penalty. After commencement of the Work, you are responsible for the entire
Contract price.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Adz-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer&€—s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
