F. In residential zone districts, where it is desired to maintain such a restricted vehicle either within six (6) feet of a public
street on private property or within a lawful parking area on a public street abutting the front of the property in excess of
seventy -two (72) hours, the property owner may obtain a temporary parking permit from the planning and development
department. Such temporary parking permit shall be for a time period not to exceed fourteen (14) days and no more than
one (1) such permit shall be issued each six (6) months for the same vehicle. The issuance of a temporary permit is for the
purpose of parking only and not for any other activity. The permit must be placed upon the inside windshield or side window
on the driver's side so as to be visible for inspection.

West 35`fi Avenue in Wheat Ridge, CO on June 21, 2000. The report that follows is based on that
investigation. The inspection was performed by and report written by Mark C. McCafferty P.E.
For your interest, a copy of Mr. McCafferty's resume is attached.
This inspecrion report is limited to observations made from visual evidence. No destructive or
mvasive testing was performed. The report is not to be considered a guarantee of condition and
no warranty is implied.
Tn the course ofthis report, we will discuss the visible distresses that exist in the subject property
on the day ofthe inspection, both on the interior and exterior ofthe building. An e7cplanation and
interpretation ofthese distresses will follow. Finally, ifnecessary, a discussion concerning possible
repairs will be included. The engineering necessary to implement any remedies and repairs is
mcluded in the scope of service of this report.

ORIENTATION
The subject structure is a single story ranch style residence located in the western Denver,
Colorado suburb of Wheat Ridge, Colorado. The residence is finished with and eacterior of brick
veneer and roof surfacing of asphalt composition shingles. The ea4erior load bearing walls consist
of concrete masonry unit 4 inches in width with standard 4 inch bricks on the earterior as a veneer.
The home features a shallow crawlspace under the living quarters with a central girder and brick
column support system.
The lot upon which tlus home is constructed slopes moderately downward toward the front of the
property. It appears that little cut and fill was required during the original construction of the
structure. The foundations are apparently conventional cast in place concrete spread footings
supporting short cast in place concrete stemwall upon wluch the masonry unit bearing walls are
constructed.
For the purposes ofthis report, the home is considered to face north.
OBSERVATIONS
The call to our office was with regard to some interior and exterior distresses in the load bearing
wall system indicative of differential elevarion changes of the foundation system The eacterior
distresses generally consist of stairstep pattern cracldng in the brick veneer. The interior distresses
generally consist of cracking in the mterior plaster or drywall wall finishes.
DISCUSSION
A relative elevation survey was performed throughout the residence using a gas level device
ideally suited for this purpose. Measurements indicate a general loss of elevation toward the front
of the home and some evidence vertical heave on the south (upslope) foundation wall Results of
this survey aze recorded on drawing no. 99-1199A, attached. Evidence indicates that considerable
water has entered the crawlspace in the past, probably as a result of inadequate drainage around
the rear perimeter ofthe home. A maximum differential ofnearly 8 inches was recorded between
the southeast and northwest corners ofthe home. The mortar joints ofthe brick colunms
supporting the central floor girder in the crawlspace have deteriorated (probabiy due to the
moisture ingress) and no longer adequately support the floor girder.
CONCLUSIONS
It is our considered opinion that the subject residential building has suffered from vertical
displacement of its foundation system, as evidenced by elevation readings and building distress
that is common for this condition. This displacement, a general 'mcrease of elevarion, which is
more severe in some locations, has caused a loss of load bearing capacity in the foundation system

in the building. Some elevations change is also likely the result of some settlement ofthe spread
footing system, particularly along the north side ofthe home. Evidence indicates that the
condition is dynamic and unstable, and propagarion ofthe condirion is ]ikely and expected. We
find sufficient cause far the remedy of this condirion with the use of hydraulically driven steel
piers installed to transfer loads from the perimeter spread foundation system as well as the brick
cohiwn system supporting the central floor girder.
REMEDIES
To remedy the condition, we recommend the use of steel piers to provide supplemental vertical
support for the foundation system. These piers will be driven to load bearing strata, and placed 'm
such a mauuer that their total load bearing capacity exceeds the combined loads placed upon it.
After stabilizafion of the system, the piers may be used to provide some elevarion restorarion of
the building. In this case, the elevation restoration will be negative in some azeas and positive in
some others to correct the exaliug uplift and/or settlement condirion.
REPAIR SPECIFICATIONS
Repair specifications for the conection of this building or building component include the
installation of steel piering and subsequent stabilization of three interior basement columns and
supporting piers.
Installation of steel piering shall occur at the eaisting girder column locations as indicated on
Drawings 00-1199B. Contractor shall jack against the framing ofthe building to install steel piers
per GENERAI, NOTES. Contractor shall clip and abandon caissons after preloading steel piers.
Contractor shall use girder loading as follows:

When applying proprietary vertical jaclang system. Calculations are base upon two story
residentiai floor loads, and a snow load appropriate for this azea. Calculations are the property of
the client and Engineer, and can only be requested through the engineer.
Recommended locations for the steel piering is indicated on Drawings 00-1199B. Type of
foundation system shall be visually verified. If caisson and void system, contractor shall clip and
abandon existing caissons in repair areas.
Unless otherwise noted, all conditions of the attached GENERAL NOTES shall be observed.
Contractor shall adhere to the GENERAL NOTES and the Work Description except by written
and prior notice ofthe Engineer.
Client is responsible for restoration ofinterior and exterior distress after installation ofthe piers,
unless otherwise included 'm contract documents by Contractor.
Contractor is responsible for job safety.
Contractor is responsible for obtaining necessary permits and scheduling inspecrions.

GENERAL NOTES
1. All materials, equipment, supplies, consumables, labor, supetvision, and a pro-rata shaze af werhead shall be
provided by the contractor. Temporazy shoring, bracing, and li8ivg or jacking devices shall also be provided by the
contractor. All materials and workmavship aze subject to the approval of the representaYive of Criterium -
McCafferty Engineers, hereafter known as Engineer.
2. All laws of the State of Colorado, including but not limited to Workmau's Compensation laws, shall be adhered
to in [he course of work on this project. Bxcavation support, scaffolding, lifting, and the use of power movement
devices shall be performed in accordance with OSHA Safety and Health Standards for the Construction Industry
(29 CFR Part 1926, Amended June 1, 1989) and applicable local building codes.
3. The Uniform Building Code, with any local amendments, shall be adhered to in the course of new construction
when applicable in the described project.
4. While data, information, condition descriptions, material conditions and/or yuantities, and structural member
sizes contained in tltis repair/construction plan may be used to describe the scope of repairs and provide general
information to the contractor for bid pnrposes, the contractor shall verify and perform an evaluation of conditions
and material condipons prior to submitting bid(s). The enGre extent of the work required and necessary equipment
and materials for completion of the work are the responsibility of the contractor.
5. The contractor shall exercise due caze while locating and avoiding problems with buried utilities when
excavating, if necessary in the course of this project.
6. When backfill of and excavation is necessary, the backfill shall be perFormed in no more than 6" lifts, with
compaction at each lift. Backfill material shall be of non-expansive soils, uniess otherwise noted
7. Concrete shall be provided by the contractor or the contractots supplier. Concrete sball cylinder test at 3500 psi
compressive strength at 28 days. Compressive strength that achieves 3500 psi at less that 28 days shall be
considered acceptable. Crrout used for securing of structural members shall be of nonshrivking variely. Where
concrete or grout is placed onto existing concrete, the existing surFace shall be prepared and a bonding agent shall
be used prior to placement of the new material. Reinforcing bars for cast in place concrete shall conform to ASTM
A615, Grade 60 or better. Welding of reinforcing bars is stricfly prolubited without the consent and supervision of
the Engineer. In this case, welding shall be performed by a welding operator certiffed on the procedure.
8. All welding shall be performed by certified welding operators, and in accordance with the American Welding
Society Dl.l - 94. Welding procedure specifications, procedure qualification records, and operator qualificarion
and continuity records shatl be provided to the engineer upon request. All welding shall incorporate the use of filler
material. The use of short-circuit metal inert gas welding is strictly prohibited
9. Foundation cracks of a size 1/16" and lazger shall be repaired by epoacy injection. Cracks shall be completely
filled with Sikadnr 35 or eqnal, as approved by the Engineer. Resulang surface shall be grinded to original surface
level. For fwndations consisting of stone and mortar, cracks in mortar joims shall be repaired by tuckpointing
with suitable cement mortar.
10. Repair of damaged exterior landscaping shall be the responsibility of the contractor. All exterior
landscaping shall be replaced to original condition, or to the satisfaction of the homeowner. Original
slope away from residences shall be maintained, unless otherwise noted
11. Repair of damaged paint, textures, finishes, equipment, and fixtures, shall be the responsibility of
the contractor unless othercvise noted Replacement for these shall be of eqaal or better quality as those in
