inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

GENERAL CONDITIONS: Denver Roof Pros (hereinafter referred to as "DRP") will enter into agreement with the
property owner (hereinafter referred to as "owner"). By signing this agreement, the Owner agrees to use DRP as its
restoration contractor to complete the "private work" or if an "insurance claim", the insurance scope of repairs and
shall be considered a mutual binding agreement between both parties. In regard to "insurance work", this agreement
is for full scope of insurance proceeds by trade. DRP is authorized to invoice the insurance company for additional
repair/supplements, as necessary. Any additional trade specific related proceeds, including but not limited to material
taxes, base service charges and overhead/profit received by Owner from the insurance company shall be paid to
DRP on a pro -rated trade specific basis. The Owner's out of pocket expense will never exceed the insurance
deductible. Any expense over the deductible will only apply in the case where: (1) The Owner wants to upgrade
product; or (2) Owner's insurance policy only covers the actual cash value of the loss (if Owner carries an ACV policy
they will be responsible for the non-recoverable depreciation amount), or (3) Owner's insurance policy does not
include "ordinance and law" coverage. If the Owner does not have "ordinance & law" coverage, the Owner will be
responsible for all costs not covered by the insurance settlement for compliance with current government laws and
codes.
PROPERTY OWNER'S RESPONSIBILITIES: The Owner represents and warrants to DRP that he or she owns the
premises described in this agreement and has the authority to enter into this agreement without any other consents.
It is the responsibility of the Owner to obtain approvals required by their home owner's association or other groups
with jurisdiction over the property.
LIMITED WARRANTY: DRP is not a manufacturer. If any equipment or materials are used or purchased by DRP for
use on-site, the manufacturer's warranties, if any, of the equipment and materials are provided solely by the
manufacturer. DRP provides a Seven year warranty on workmanship, subject to normal use and conditions with the
following exceptions which are unrelated to DRP workmanship: Damage caused by settling of the foundation on the
premises, misuse, maintenance failures, fire, lightning, windstorm, hailstorm or other weather conditions or
casualties. STATE BILL 12-38: DRP does not claim to be or act as a public insurance adjuster adjusting claims for
losses or damages. In accordance with Colorado Revised Statutes (C.R.S.) section 6-22-105, DRP may not pay or
promise to pay, waive or rebate all or part of your insurance deductible. DRP will hold in trust any payments from the
property owner until the DRP has delivered the materials at the property site or has preformed the majority of the
work on the property. Pursuant to C.R.S. 6-22-104, you may rescind this contract within 72 hours following your
receipt of notice from the insurance company that your claim has been denied in whole or in part, unless the denial
was related to a request for supplemental roofing services if the damage requiring the supplemental roofing services
could not have been reasonably foreseen as a necessary and related roofing service at the time of the initial roofing
inspection by DRP or the execution of this contract.
DRP is required by Colorado State Law to carry a $2M/$i M insurance policy for personal and property damage,
which is listed in the top right corner of this document.
DRP agrees to perform the work authorized by the insurance company at the rates that the insurance company
authorizes for each specific trade. The work will be completed to the most recent code standards.
Owner authorizes DRP to work directly with the insurance company when necessary to complete the work as
specified on behalf of the owner/policy owner. DRP recognizes that time is of the essence and the work will be
completed as efficiently as possible.
GOVERNING LAW: The property owner hereby expressly agrees that any and all claims arising from the work by
DRP shall be subject to the rights, remedies and procedures set forth in the Colorado Construction Defect Reform
Act, C.R.S. section 13-20-802 et seq. If any provision of this agreement is held to be invalid or unenforceable by a
court of competent jurisdiction, the validity of the remaining provisions shall not thereby be affected. The terms and
conditions set forth herein are herby integrated into all agreements by and between DRP and the property owner,
whether or not explicitly stated therein.
The customer will provide DRP with the ACV amount listed below, in order that materials can be procured, prior to
beginning work, but as required by State law, will be held in trust until work is initiated.
It is understood that initial insurance estimates are not final numbers and that those numbers will vary due to
supplemental billing to the insurance company based on extra costs incurred, increased reimbursement rates,
supplemented items and overhead and profit from the insurance company. Customer is responsible for permit and
taxes on permit (if not paid by insurance company). The home owner will never be responsible for more than their
deductible. However, as mentioned above, all insurance proceeds will be paid to DRP based on trades that DRP
manages or conducts.

GENERAL CONDITIONS: Denver Roof Pros (hereinafter referred to as "DRP") will enter into agreement with the
property owner (hereinafter referred to as "owner"). By signing this agreement, the Owner agrees to use DRP as its
restoration contractor to complete the "private work" or if an "insurance claim", the insurance scope of repairs and
shall be considered a mutual binding agreement between both parties. In regard to "insurance work", this agreement
is for full scope of insurance proceeds by trade. DRP is authorized to invoice the insurance company for additional
repair/supplements, as necessary. Any additional trade specific related proceeds, including but not limited to material
taxes, base service charges and overhead/profit received by Owner from the insurance company shall be paid to
DRP on a pro -rated trade specific basis. The Owner's out of pocket expense will never exceed the insurance
deductible. Any expense over the deductible will only apply in the case where: (1) The Owner wants to upgrade
product; or (2) Owner's insurance policy only covers the actual cash value of the loss (if Owner carries an ACV policy
they will be responsible for the non-recoverable depreciation amount), or (3) Owner's insurance policy does not
include "ordinance and law" coverage. If the Owner does not have "ordinance & law" coverage, the Owner will be
responsible for all costs not covered by the insurance settlement for compliance with current government laws and
codes.
PROPERTY OWNER'S RESPONSIBILITIES: The Owner represents and warrants to DRP that he or she owns the
premises described in this agreement and has the authority to enter into this agreement without any other consents.
It is the responsibility of the Owner to obtain approvals required by their home owner's association or other groups
with jurisdiction over the property.
LIMITED WARRANTY: DRP is not a manufacturer. If any equipment or materials are used or purchased by DRP for
use on-site, the manufacturer's warranties, if any, of the equipment and materials are provided solely by the
manufacturer. DRP provides a Seven year warranty on workmanship, subject to normal use and conditions with the
following exceptions which are unrelated to DRP workmanship: Damage caused by settling of the foundation on the
premises, misuse, maintenance failures, fire, lightning, windstorm, hailstorm or other weather conditions or
casualties. STATE BILL 12-38: DRP does not claim to be or act as a public insurance adjuster adjusting claims for
losses or damages. In accordance with Colorado Revised Statutes (C.R.S.) section 6-22-105, DRP may not pay or
promise to pay, waive or rebate all or part of your insurance deductible. DRP will hold in trust any payments from the
property owner until the DRP has delivered the materials at the property site or has preformed the majority of the
work on the property. Pursuant to C.R.S. 6-22-104, you may rescind this contract within 72 hours following your
receipt of notice from the insurance company that your claim has been denied in whole or in part, unless the denial
was related to a request for supplemental roofing services if the damage requiring the supplemental roofing services
could not have been reasonably foreseen as a necessary and related roofing service at the time of the initial roofing
inspection by DRP or the execution of this contract.
DRP is required by Colorado State Law to carry a $2M/$i M insurance policy for personal and property damage,
which is listed in the top right corner of this document.
DRP agrees to perform the work authorized by the insurance company at the rates that the insurance company
authorizes for each specific trade. The work will be completed to the most recent code standards.
Owner authorizes DRP to work directly with the insurance company when necessary to complete the work as
specified on behalf of the owner/policy owner. DRP recognizes that time is of the essence and the work will be
completed as efficiently as possible.
GOVERNING LAW: The property owner hereby expressly agrees that any and all claims arising from the work by
DRP shall be subject to the rights, remedies and procedures set forth in the Colorado Construction Defect Reform
Act, C.R.S. section 13-20-802 et seq. If any provision of this agreement is held to be invalid or unenforceable by a
court of competent jurisdiction, the validity of the remaining provisions shall not thereby be affected. The terms and
conditions set forth herein are herby integrated into all agreements by and between DRP and the property owner,
whether or not explicitly stated therein.
The customer will provide DRP with the ACV amount listed below, in order that materials can be procured, prior to
beginning work, but as required by State law, will be held in trust until work is initiated.
It is understood that initial insurance estimates are not final numbers and that those numbers will vary due to
supplemental billing to the insurance company based on extra costs incurred, increased reimbursement rates,
supplemented items and overhead and profit from the insurance company. Customer is responsible for permit and
taxes on permit (if not paid by insurance company). The home owner will never be responsible for more than their
deductible. However, as mentioned above, all insurance proceeds will be paid to DRP based on trades that DRP
manages or conducts.

Dear Mr. Cadotte:
This letter is to verify that I have reviewed the repair work to the roof that has been
done by Rincon Partners, LLC. The repair work involved some overframed members
that were not adequate when they were installed sometime in the past. Some 2x6 rafters
were used for long spans that they had insufficient capacity to span. Some of these
rafters had failed in a past snow event. Rincon Partners had shortened the spans by
installing headers and 2x4 stud walls down to the engineered trusses below.
All of the repair work appeared satisfactory. However, I have recommended that the
contractor add a post down to the bearing wall below under the ends of the hips at each
end of the main roof ridge. I was concerned that the load from the overframing might
impose too much load on the first truss of the main roof. The post, which may consist of
2 -2x4's or more, will instead carry the overframing loads directly down to a bearing wall.
Modified in this manner, it is my opinion that the roof framing will be structurally
adequate.

This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must
be made in writing and received prior to the date of expiration. An extension of no more than 180 days made be granted
at the discretion of the Chief Building Official and may be subject to a fee equal to one- half of the original permit fee.
If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to
