The purpose of this letter is to confirm proper installation of the header and ceiling framing for the
interior remodel of the single family home located at 6761 West 36t1' Place in Wheat Ridge, Colorado.
Header framing and installation to be installed per the Structural Headers report prepared by Ashworth
Structural, Inc. dated September 21, 2020.
On January 28, 20211 observed the home and installation of the header at the east -west kitchen wall on
the north side of the house. The triple LVL header beam was properly installed, connected and
supported at the end bearing points and the bearing columns have been properly blocked to the support
framing below. In addition to the LVL header for ceiling support, during removal of the secondary wall
north of the beam it was discovered that the ceiling joists were spliced over the secondary wall.
Additional 2x6 ceiling joists were sistered and installed at each of the existing ceiling joists to carry all
ceiling loads to the newly installed beam that was designed to support these loads. Sistered ceiling joists
have been properly installed and no additional work is required. Based on my observations, no
additional modifications are required and the ceiling joists, support beam and columns are properly
installed to support Code required loading in accordance with the Structural Header letter dated
September 21, 2020. No additional work is required and no repairs are required.
Opinions in this report are limited to observations and conclusions for interior remodel of the single
family home located at 6761 West 36th Place in Wheat Ridge, Colorado. We reserve the right to review
any additional information concerning this property and to amend our opinions accordingly. No
warranty is either expressed or implied in this inspection and opinions expressed in this report do not
provide an insurance policy, nor warranty service. In the event of an oversight, maximum liability must
be limited to the fee paid.

RECOMMENDATIONS AND SUMMARY
None of the materials listed above are regulated asbestos containing building materials and do not need
to be removed by a licensed asbestos abatement contractor prior to any demolition or renovation. This
is also a limited survey. If any additional material is found and is suspect all work must stop. Other
areas behind walls and above ceilings and under floors were considered inaccessible and were not
tested. If suspected materials are found during renovation or demolition activities then all work must
stop and materials tested. If any of the materials do contain asbestos then a licensed asbestos abatement
contractor must remove materials according to all Federal, State and Local regulations. The area then
must be cleared before any other renovation or demolition activities can continue.
ANALYTICAL PROCEDURES
The samples collected were delivered to F.R.S. A National Voluntary Laboratory Accreditation
Program (NVLAP) asbestos laboratory, located in Denver Colorado for analysis with a Rush
turnaround time. All samples are archived for 30 days unless otherwise stipulated by the client.
According to the Laboratory, the samples were analyzed in accordance with the E.P.A. Method 600/R
93/116.
This is a limited asbestos survey.
This is not a complete demolition survey.
The laboratory reports are enclosed.

If there is any construction debris that contains any amount of asbestos or is assumed to contain
asbestos you must notify the land fill per the Hazard Communication Act. Such as roofing materials
that are tar impregnated, weather proofing, floor tiles and mastic and any other OSHA material deemed
to be non -friable. These materials must be kept non -friable you can not saw, sand, grind or in any other
way pulverize said materials. Even if the material contains less then 1% asbestos then OSHA standards
and regulations apply. All materials greater than 1 % are considered asbestos containing and regulated
by OSHA, EPA, State of Colorado and NESHAP.
All Samples Are Taken In A Random Sampling Table. Using The EPA Booklet Asbestos In Buildings:
Simplified Sampling Scheme For Friable Surfacing Material.
If point counts are used the point count number shall be used per the EPA. When the point count is
done the lab uses the EPA -600/R-93/116. Where the samples are read and the fiber has to cross the
cross hairs to be counted. ND means that None were Detected. The fibers can be observed but not
counted. If 1 fiber crossed the cross hairs it is counted as 0.25%, 2 fibers are counted as 0.50%, 3 fibers
are counted as 0.75%, 4 fibers are counted as 1% and 5 fibers are counted as 1.25%. Per EPA any
material 1.25% or grater is regulated. Any material I% or less in Non Asbestos containing.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy

MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an
approved mid -roof inspection, conducted when 25-75% of the roof covering is installed,
prior to final approval. Installation of roof sheathing (new or overlay) is required on the
entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists.
Sheathing and mid -roofs may be called in at the same time, one hundred percent of the
sheathing must be complete and 25-75$ of the mid -roof may be complete. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required.
LADDER MUST BE SET FOR ALL ROOF INSPECTIONS: A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections.
COMMERCIAL ROOFS: In order to pass a final inspection of elastomeric or similar type roof
coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Dear Mr. Schat t fer:
On this date, Mark Benjamin, E.I., of our office observed
the proposed basement remodeling project at the above location.
As we understand it, the bearing wall in the basement at the west
end will be removed. This wall bears on a 4" x 4" curb on what
we assume, by our experience of observing similar vintage homes,
to be a 16" wide x 8" thick thickened slab reinforced with two #4
longitudinal rebar. Two new beams will be installed to replace
the wall section removed. One beam starts approximately 4' from
the west end and will span 101-6", the second beam starts
approximately 16" from the first and will span 61-0".
Per our engineering calculations, the 101-6" beam shall be
composed of (3) 1-3/4" x 7-1/2" 2600f laminated veneer lumber
members, such as Micro-Lams manufactured by the Trus Joist
Weyerhaeuser Corporation. As an option, an MC6x12 or C7x9.8
steel channel section may be installed. The second beam should
be composed of (2) 1-3/4° x 7-1/2" 2600f laminated veneer lumber
members. With either beam, install a(2) 2 x 4 standard grade
column under each end.
The 16" wide x 8" thick thickened slab footing section will
be sufficient to bear the total load weight of the beams. Verify
this assumption and inform our firm, J. C. Moore, P. E., P. C.,
of your observations, as we may have to amend this report
depending on what you report. Cut out the curb at the locations
where the wall will be removed. Under the (2) 2 X 4 columns,
install a Simpson Strong-Tie No. BC4 column base on top of the
curb to alleviate concrete spalling of the curb.
