Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding AM -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturers€TMs installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for

I. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2, This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3, If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change. of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

1. If the client plans to use the proceeds of a property and casualty insurance policy pursuant to part 1 of
article 4 of title 10, C.R.S., to pay for the roofing work, pursuant to section 6-2-105, the roofing contractor
cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for work on the covered residential property.
2. All Contracts subject to approval by Endeavor Exteriors financial department.
3. Contract price is valid for thirty (30) days.
4. Endeavor Exteriors reserves the right to perform specified work in accordance with generally accepted
construction guidelines, while adhering to local codes in effect at the time of contract.
5. All checks must be made payable to: Endeavor Exteriors.
6. All excess materials remain the property of Endeavor Exteriors.
7. Client is responsible for Homeowner's Association approval of products and colors.
g. Client acknowledges Endeavor Exteriors as a general contractor, as such Endeavor Exteriors will be
entitled to ten percent (10%) overhead, and ten percent (10%) profit, (O&P) if approved by the insurance
company, if this project is part of a claim.
9. An account is defined as past due when 10 days have elapsed from the date of completion of work. Past
due accounts will earn interest at a rate of one and one-half percent (1.5%) per month or eighteen percent
(18%) annually. If Endeavor Exteriors is required to turn the matter over to an attorney for collection, the
client also agrees to pay all costs incurred, including attorney's fees.
10. Should default of payment occur on this contract, Endeavor Exteriors reserves the right to place a
mechanic's lien on the clients' property.
1 l . Endeavor Exteriors shall not be held liable for failure of performance due to labor controversies, strike,
fire, hail, weather, inability to obtain material from usual source, or any other circumstance beyond the
control of the company.
12. Our standard workmanship warranty for roofing is non-transferable limited lifetime, and one (1) year for all
other trades, and the company will provide a service call for roof inspection, as long as the client owns the
property.
13. Endeavor Exteriors is not responsible for any leaks or water damage below the roofline caused by
excessive wind, ice or hail during the period of the warranty.
14. Endeavor Exteriors shall refund any deposit if this contract is cancelled within 72 hours (3 days).
15. If this contract is cancelled by the client, after three (3) or more days of execution, the client agrees to
pay Endeavor Exteriors twenty percent (20%) of the contract price, set by the insurance company, only if
materials have been delivered or work has been performed, as liquidated damages. This payment is not a
penalty, but is fair and just compensation for services rendered to that point.
16. The homeowner may cancel this contract within 72 hours if the insurer has denied, in whole or part, the
homeowners claim that relates to this contract.
17. While we make every effort to ensure there are limited consequences of construction, Endeavor Exteriors
will not be held responsible for slight scratching or denting of gutters, droplets of oil in the driveway,
cracks in concrete, or damage to plants and trees. Furthermore, Endeavor Exteriors will not be held
responsible for exterior damages such as deteriorated trim pieces, crown molding, stucco, solar panels,
evaporative coolers, existing skylights, or interior damage such as falling dust and debris, cracked plaster,
sheet rock or nail pops, as a result of normal re-roofing.
18. Replacement or repair of the deteriorated decking, fascia boards, swamp coolers, ventilators, chimneys,
counter flashing, or any other work completed for the homeowner not explicitly stated in this contract,
are not included in quoted price and will be charged as extra on a time and material basis.
19. Endeavor Exteriors holds the right to photograph all projects in which it is invloved, and the right to
publish throughout all Endeavor Exteriors' social media outlets.

A review of your submittal documents for City of Wheat Ridge Plan #08-0366 for a room addition
above garage at 7005 W. 37'h Avenue, Wheat Ridge, CO, 80033 was conducted and the following
notes, corrections and clarifications are hereby attached to and indicated on the submitted plans:
1. All work shall comply with:
2003 International Residential Code
2005 National Electrical Code
City of Wheat Ridge Municipal Code
2. All foundation inspections shall be conducted by the Engineer-of-Record and verified to the
Building Division by wet-stamped letter of approval prior to rough framing inspection.
3. Garage shall be separated from living space by a minimum 1/2-inch Type X gypsum wallboard
attached with fasteners spaced a maximum of 6 inches on center and by doors with a minimum
of 20-minute fire rating. All structural supporting members shall protected by one layer of 5/8-
inch Type X gypsum wallboard.
4. Underside of stairs and adjacent walls shall be protected with minimum 5/8-inch Type X gypsum
wallboard.
5. Inspection of caissons is required prior to pour.
6. A minimum of 80 inches of headroom is required above stairs at all points.
7. Deck ledger shall be flashed at connection to structure to prohibit entrance of moisture.
8. Rafters shall be provide with uplift protection capable of withstanding 175 pounds of force at
bearing points (Min 1-H2.5 clip)
9. Stair treads shall be a minimum of 10 inches in depth and stair risers shall be a maximum of 7-
3/4 inches in height.
10. Landings measuring a minimum 36 inches in the direction of travel are required at the top and
bottom of stairs.
11. Handrail placed 34-38 inches of the stair nosing is required on one side of stairs.
12. Insulation with minimum values of R-19 in walls, R-38 in ceilings and R21 in floors is required.
13. Heating system with the ability maintain a minimum temperature of 68 degrees is required in a
living areas.
14. Roof shingles shall be nailed with a minimum of 6 nails per shingle. A mid-roof inspection is
required. Ice and water shield IS NOT required.
15. All work is subject to field inspection and approval.
