Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

On August 27, 2004, Kenneth BoYd of ou[ companY Performed a site visit to review the
aa-built condi,tions on the abovo refucaced project. An as-built survcy aas not providcd
for us to compare with the construction dceuments. This was a good day to observe the
gadmB measures becausc it raiu►ed most of the day. We found that the both duplcxcs
werc complete and the landacaping was in plaae. The grading and dreinage around the
both duplexos appears to substantially conform to the appmved civil sheet Cl Drainage
and Erosion Coi►trol Plaa. The only exception wece the gradiwg assumptions we made
for the lot to the east. It appearod the swale az►d berm as called for on the lot w the east
w,as not constructed. Ponding was evident on the propcrty to the east.
Even with the rain, ponding was mot evident on the above referenced Property. The roof
of the eitha duplex is pitched so that haif the roof runoff goes to the low spot between
the duplex units. From there runoff heads west to Pierce Stmt. The other half of the
south duplex leaves the roof and reveRs to s1►eet flow as at exits the property to the south
and to 32°d Avemue. Tlie temainder af the north duplac flows nozth and then west to
Pieree St.
DrainaSo imPrnvements have been inatalied and fimctiom according tele of feet
BPPe~ the owner tas moved the drainaBe swale betwem the propeYties a couP
to the north. This will allow it to be contained in ona propetty• It will rema3n in the
proposed 10 foot drainage easema►t. New location of the awale is acceptable from a

Dear Mr. McLain,
This letter is to inform you that the Final Drainage Report/Plan and the grading/erosion control plan for
the above referenced project have been reviewed and approved for construction. Please be aware of the
following items regazding the construction of the project:
1. Erosion control for the project shall be placed in conformance with the approved drainage/erosion
control plan and shall be maintained as needed during the course of construction.
2. All drainage improvements shall be constructed in conformance with the approved drainage report
and plan.
Upon completion of the drainage improvements the Engineer-of-Record, Mr. John M. McLain, P.E.,
shail provide to the City of Wheat Ridge a Drainage Certification letter stating that the grading and
the drainage shall function as defined in the Final Drainage Report and Plan, have been accurately
surveyed to confirm their construction is in accordance with these documents. The certification
letter shall be written and stamped by the engineer who prepared the drainage report, and shall be
submitted for review and approval by the City prior to issuance of the Certificate of Occupancy.
4. It will be the responsibility of the of the contractor for the project to repair any damage to the
existing public improvements on Pierce Street or 32"d Avenue as a result of related construction

DESCRIPTION
Materials sampling and testing for public improvements shall be performed by an independent materials
testing company at the expense of the developer, and/or utility district. Unless otherwise designated, all
referenced speci5cations, standards or policies shall be the latest edition as revised or updated by
approved supplements published and issued prior to the date of the building perntit.
REQUIREMENTS
All materials sampling and testing shall be performed by certified, experienced and qualified materials
testing technicians who work under the supervision of a registered professiona] engineer in the State of
Colorado. All materials sampling and testing equipment shall be serviceab]e and calibrated.
Soil classifications and moisture-density curves shall be provided to the Department of Public Works
inspector prior to in-place density testing. Materials testing technicians shall fumish copies of failed test
results to the inspector as prompdy as the results become available. On a weekly basis when testing is
being performed, the developer shall furnish Ihe inspector with copies of all test results taken during the
prior week and a cover letter, signed by the supervising registered professional engineer, which
summarizes the results and discusses any failed tests or inconsistencies.
The City materials testing requirements are provided in Table 720-1. All testing procedures of
verification and central lab requirements shall be as speciFied in the Frequency Guide Schedule of the
Colorado Department of Transportation Field Materials Manual. One test is required for any fraction of
the specified frequency. The reference to Section 203.07 in the table is from the Colorado Department
of Transportation Standard Specifications for Road and Bridge Construction.

All permits shall be applied for through the Public Works Department. An approved permit is effective for a period
of one (1) year from date of issue and may be renewed. Ail fees shall be in accordance with those fees
established by the Uniform Building Code or other applicable City adopted Resolutions or Ordinances.

MINOR EXCAVATION AND FILL PERMITS
1. 1-50 Cubic Yards: No permit is required for dumping or excavation of earth aterials, which do not
exceed 50 cubic yards, provided, however, that any fill deposited is on natural terrain of less than three
(3) horizontal to one ('I) vertical slope, or such fill is less than three (3) feet in depth and is not intended
to support permanent structures, and in addition, such fill or excavation does not obstruct or otherwise
adversely affect any drainageway. Should any of the above standards be exceeded, or a drainage way
be affected, a permit shall be required under the guidelines for No. 2.
2. 51-500 Cubic Yards: Dumping or excavation of earth materials not exceeding 500 cubic yards may
be allowed with a permit approved by the City Engineer. All applications are to be submitted on the
appropriate completed application form and shall be accompanied by the appropriate fee and sketch
plan indicating the following information before the permit wiil be issued.
A Location and dimensions of all property boundaries and structures on the site.
B location and extent of areas to be filled and/or excavated.
C. Location of existing and proposed drainageways, irrigation ditches, etc., and indication of
how and where historic run-off will be maintained on and through the site.
D. Cross section area to be filled and/or excavated indicating original slope, new slope and
depth of fill.
E. Siatement which indicates the proposed use or purpose for said fill or excavation.
F. Relative elevation of adjacent properties.
G. Erosion control plan showing placement of control devices such as hay bales, etc..

3. 501-20,000 Cubic Yards: Operations in which earth material fill or excavation exceeds 500 cubic
yards but does not exceed 20,000 cubic yards may be allowed by a permit issued by the City Engineer
after review of an application and supporting information. The following information shall be submitted
with the required appiication form:
A. A site plan, prepared and signed by a Colorado registered professional engineer, at a scale
of no less thanl inch to twenty feet ('I :20) which illustrates the following:
1. Location an dimensions of all property boundaries and structures on the site.
2. Location and exRent of areas to be filled and/or excavated.
3. Location of existing waterways and drainage courses indicating any changes. (for a
site containing an established irrigation ditch, a letter of approval from the
appropriate ditch company shall be required).
4. l.ocation of existing and proposed points of ingress.
5. Location and extent of existing vegetation, proposed changes in such vegetation and
methods of rehabilitation on site vegetation after earthwork operations are complete.
6. Erosion control pian.
7. Grading plan with existing (dashed lines) and proposed (solid lines) ground contours
with contour intervals of 2 feet and spot elevations.
8. At least two cross sections (east to west and north to south) through the site
showing depth of fill andlor excavation.
9. A drainage report may be required if site runoff characteristics are changed.
10. Relative elevations of adjacent properties and structures.
11. Statement of purpose of intended flll and/or excavation.
4. Performance Standards: The following provisions shall appiy to all Minor Excavation and Fill
Permits:
A. Rehabilitation: Within thirty(30) days after cessation of filling and/or excavation,
rehabilitation for the site shall have been completed in accordance with the approved plans.
Rehabilitation shall corsist of leveling, grading, landscaping or any combination thereof to
minimize potential erosion and be acceptable to the City Engineer.
B. Debris: Debris and/or contaminants shall not be used except with the approvai of the City
Engineer.
"For the purpose of this Section, the term debris shall have the same meaning as the terms
"garbage, trash or junk" as defined in the Wheat Ridge Code of Laws, Section 15-4."
In no case shall debris or contaminates identified or classified as hazardous waste by locai,
state or federal agencies be used as fill in any fill areas within the City of Wheat Ridge.
C. Excavation and/or fill areas shall be graded to facilitate weed control until final grades are
set and site rehabilitation and use occur (must be leveled with side slopes not to exceed 3
to 1).

the above referenced project have been reviewed and approved for construction. Please be awaze of the
following items regazding the construction of the project:
1. Erosion control for the project shall be placed in conformance with the approved drainage/erosion
control plan and shall be maintained as needed during the course of construction.
2. All drainage improvements shall be constructed in conformance with the approved drainage report
and plan.
Upon completion of the drainage improvements the Engineer-of-Record, Mr. John M. McLain, P.E.,
shall provide to the City of Wheat Ridge a Drainage Certification letter stating that the grading and
the drainage shall function as defined in the Final Drainage Report and Plan, have been accurately
surveyed to confirm their construction is in accordance with these documents. The certification
]etter shall be written and stamped by the engineer who prepazed the drainage report, and shall be

Each lot of the Last Greenhouse Subdivision shall be assessed parkland dedication fees at time of
issuance of building permit. In March of 2002, I sent a memo to the then-owner, Michael Jones stating
that a total of $502.50 would be collected for fee in lieu of pazkland dedication for Lot 1. This figure
was based upon one dwelling unit for Lot 1. Michael Jones has since sold the lot, and it has been
indicated that it will be developed with two dwelling units. In light of this modification, all lots will be
assessed a total of $1,632.75.
This figure was derived by taking the assessed value, as described by Jefferson County ($64,800) and
dividing it by the total subdivision area (1.27 acres). This gives a price per acre of $51,023 for the total
subdivision.
This subdivision valuation was then multiplied by .096 (6 dwelling units), giving an total assessment
needed of $4,898.00. This ftgure was divided by three (3 lots in the subdivision).
These parkland dedication fees shall be paid for each lot prior to issuance of any building permits.
