2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

TERMS AND CONDITIONS
THE CONTRACT ON THE FACE HEARS OF AND ANY AGREEMENT MADE PURSUANT THERETO BETWEEN INTEGRITY XTERIORS
(THE "COMPANY") AND THE CUSTOMER (S) (THE "CUSTOMER") WILL BE SUBJECT TO ALL APPROPRIATE LAWS. REGULATIONS AND
ORDINANCES, AND TO THE FOLLOWING SPECIAL TERMS AND CONDITIONS:
All contracts subject to approval of Management & our Credit Department without exception.
ALL CHECKS FOR PAYMENT MUST BE MADE PAYABLE TO INTEGRITY XTERIORS. THERE WILL BE NO EXCEPTIONS.
HOMEOWNER WILL STILL BE LIABLE TO INTEGRITY, INC. FOR PAYMENT OF THIS CONTRACT IF CHECK IS MADE OUT TO ANY
BOTHER ENTLY OR PERSON.
.FIRST (ACV) PAYMENT PLUS COPAY OR DEDUCTIBLE is to be made upon commencement of work. FINAL PAYMENT IS TO BE MADE
UPON SUBSTANTIAL COMPLETION OF WORK. Other arrangements must be discussed with the credit manager. Substantial completion shall
mean that work completed functions to its intended purpose and moisture is not entering the premises due to the workmanship of the company.
Cosmetic, city or county inspection, or "punch list" items will be completed by the Company but shall not hinder payment of contract. All payments
from the Customer or Insurer shall be held in trust by Company until such time as Company has delivered roofing materials to the property
or has performed a majority of the work.
If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of remaining provisions of this
agreement shall not be affected Thereby.
Should default be made in payment of this contract, charges shall be added from the date thereof at a rate of one and one half percent per month
(18%er annum) with a minimum charge of $2.00 per month, and if placed in the hand of an attorney for collection, all attorney's fees and legal
and filing fees shall be paid by the customer accepting said contract.
The Company shall not be liable for failure to perform due to labor controversies, strikes, fires, weather, inability to obtain materials from usual
sources, or any other circumstances beyond the control to the company, whether of a similar or dissimilar nature.
The Company shall have no responsibility for subsequent damages from rain, fire, tornado, windstorm, or other perils, as is normally
contemplated to be covered by HOME OWNERS RISK INSURANCE or BUSINESS RISK INSURANCE, or unless a specified written
agreement be made therefore prior to commencement of the work.
The Company is not responsible for any damage on or below the roof due to leaks caused by excessive wind driven rain, ice, or hail during the period
of the warranty. EXCESSIVE WIND IS 65 OR GREATER MPH. THE STANDARD WARRANTY IS NON -TRANSFERABLE.
If material has to be reordered or restocked because of a cancellation by the customer there will be a RESTOCKING FEE equal to fifteen percent
(15%) of the contract price.

2. The Company shall have no responsibility for the repair of underlying structures that may prevent completion of the specified work, nor for
repair of any hidden or unknown conditions which interfere with the normal installation of the specified work.
3. During the duration of the work the customer's homeowner's insurance will be responsible for any interior damages to the property.
4. The Co. is not responsible for construction problems of the home. If pointed out and notified to the Co., the Co. will try to assist in correcting them
on a time and material basis. Such problems include but are not limited to rotten, swollen or deteriorated siding, fascia, soffits, stucco, masonry
and/or decking, poor ventilation, irregularities in decking due to settling or bowing of framing, rusted, poorly installed or damaged gutters, furnace
stacks, and items that do not meet local code end must be fixed to pass required inspections.
5. Should the work of the Company fail to meet required code inspections, the homeowner agrees to allow the Company to complete the required
repairs in a reasonable time frame.
6. Any alteration or deviation from specifications involving extra cost will be executed only upon written orders, and will become an extra charge over and
above the estimate.
7. This contract and labor warranty shall not be assigned and are nontransferable unless specified in writing.
8. The Company shall revoke labor warranty if contract is not paid in full.
9. Any discounts sign allowance, referral fees, etc will be revoked if amount is past due.
0. This contract constitutes the entire understanding of the parties and no other understanding, collateral or otherwise shall be binding unless in
writing signed by both parties.
I . The Company has the right to order excess materials. All excess materials belong to the Company.
2. The Company reserves the right to file for supplemental insurance claims if insurance adjustor measurements are used and prove to be incorrect
3. At no additional cost to the customer, the Company reserves the right to file for supplemental insurance claim due to an increase in costs.
4. Supplements paid by the insurance company for additional labor and/or materials needed beyond the original scope of repairs are part of this contract
as if contained herein and will be paid to Integrity Xteriors either by the Homeowner or Homeowner's Insurance Company.
5. Homeowner acknowledges Integrity Xteriors as a General Contractor and as such will be entitled to 10% profit and 10% overhead if approved by
insurance company as part of the claim.
6. Homeowner is responsible for Homeowner Association (Covenant approval) of roofing products and color.
7. Integrity Xteriors is not responsible for satellite dish realignment.
8. If contract and/or work order is cancelled or otherwise terminated by customer after the three (3) day grace period mentioned on front, but before the work
has begun, the customer will be responsible and or otherwise liable for a $2,500.00 fee.
9. The prevailing party in any dispute over the terms or performance of this Agreement shall be entitled to recover their court costs, including reasonable
attorneys fees.
0. If the property owner plans to use the proceeds of a property and casualty insurance policy to pay for the roofing work, the Company cannot pay, waive,
rebate, or promise to pay, waive, or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on the
covered residential property.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
