Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

1. This permit was issued based on the information provided in the permit application and accomranying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regula ions, po ides and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Altitude Contracting agrees to install the following materials selected by the customer. Altitude Contracting will pull all necessary permits and furnish all
labor and materials in a good and workman like manner according to widely accepted standard practices in the industry. Any changes or modifications must
be in writing and agreed to by both parties as evidenced by their signatures. Homeowner acknowledges they have received, read, and understand Altitude
Contracting's "Build Expectations" Form. It is the homeowners responsibility to inform Altitude Contracting of any HOA requirements or restrictions.
Altitude Contracting has it's general liability insurance through Colony Insurance Company and they may be contacted through our agent, Dodrill Insurance
Inc. at 7807 W. Mississippi Avenue - Lakewood, CO 80226 (303)986-1539. Altitude Contracting shall hold in trust any payment received until the
delivery of materials to the job -site or has performed a majority of the roofing work on the property. This contract may be cancelled or rescinded
within 72 hours from the date of signing and Altitude Contracting will issue a full refund of any deposit made. It is illegal for any contractor to waive or pay for
any part of an insurance deductible compliant with Colorado Law SB38. The property owner may rescind the contract for service, the payment for which will
be made from the proceeds of a property insurance claim, within 72 hours after receiving notice from their insurer that the claim is denied in whole or in part.
By signing this agreement I/We agree to release payments to Altitude Contracting per the pay schedule outlined below. I/We agree to not withhold funds
from Altitude Contracting due to city inspections being behind schedule. Altitude Contracting agrees to bill the insurance company directly for any additional
work performed due to hidden site conditions (known as the supplement process). If there is any work performed to the property that is not in the original
scope of loss, Altitude Contracting will bill the insurance company for the required work. I/We agree to pay Altitude Contracting any additional funds
authorized and released by the insurance company. Altitude Contracting will not hold the customer responsible for paying any additional funds not agreed to
in this contract if the insurer denies additional coverage.
Altitude Contracting is not responsible for any deteriorated or rotted items on the home that becomes further damaged due to standard job vibration or
replacement of materials or flashings that are required for the build as this is unavoidable. It is further understood that gas pipes penetrating the roof may
shift during construction and Altitude Contracting recommends the homeowner have these pipes inspected & sealed if necessary, but will not pay for such
an expense. Older (and even newer) homes can have structural issues, if you are unsure as to whether or not your home is structurally sound we
recommend you have a certified engineer assess the property prior to the beginning of the build as we will not be held responsible for any structural issues
that arise due to standard material weight and job vibrations on the project. Any specific concerns must be addressed with Altitude Contracting prior to build.

TERNIS AND CONDITIONS
As applicant for a building permit in the City of Wheat Ridge, I do hereby agree to the terms and
conditions set forth below for the issuance of a CeRificate of Occupancy in connection with this
building permit. The issuance of a Certificate of Occupancy requires:
1. The completion and approval of final inspections and any necessary corrections of
electrical, plumbing, mechanical and framing systems. Inspections aze performed by the
Building Division.
2. The completion of site drainage/grading requirements per the approved site grading
plan and/or final drainage report and plan, if such report and plan is applicable. For all
residential and nonresidential projects with an approved finai drainage repoR and plan, a
letter of certification is required from the project's civil engineer of record. Inspections
aze performed by the Public Works Department.
3. The completio.n and acceptance by the City of Wheat Ridge of all required public
improvements, or a Development Covenant may be processed in lieu of constructing said
improvements. The Public Works Department performs these inspections or provides the
Development Covenant.
4. The completion of all required landscaping and related site improvements.
Inspections aze performed by the Planning and Zoning Division.
All the above itcros must bc complctcd and siencd off by Planning and Zoning, Building
and Public Worl:s before a Ccrtificatc of Occupancy is issucd. Final inspection by the
Building Division does not constitute authorization of a Certificate of Occupancy nor permission
for occupancy. Occupancy of the structure is not permitted utttil the Certificate of
Occupancy is issucd.
The City is not responsibie for any real estate closings or other agreements conceming the
propeRy which may be affected by failure to complete the above items and obtain a CeRificate of
Occupancy.
The City is under no obligntion to issuc a Temporary Certificatc of Occupancy.
